- 0:37I'm absolutely thrilled to have
- 0:38with me today Dr. Ralph Yates,
- 0:41chief Medical Officer at Salem Health.
- 0:43Doctor, it's very nice to have you on the
- 0:45podcast today. How are you?
- 0:47- I, I'm well, and thank you for having me.
- 0:49- Of course. Well, to get us started,
- 0:51would you mind please introducing yourself
- 0:52and telling us a bit about your background?
- 0:54- Of course. Uh, Ralph Yates.
- 0:56I'm, uh, in my 44th year clinically in practice by,
- 1:01by training and certification.
- 1:03I'm a family physician.
- 1:04I'm Chief medical officer of Salem Health in Salem, Oregon.
- 1:09We are a two hospital health system, 6,000 employees
- 1:14and medical staff.
- 1:16We're located in the mid Willamette Valley area of Oregon.
- 1:21Beautiful, beautiful area. Our main hospital is 644 beds.
- 1:26We're a level two trauma and nicu,
- 1:29and we service a catchment area of,
- 1:31of roughly 500,000 citizens,
- 1:34and we are the main hospital for that area.
- 1:37- Wonderful, wonderful.
- 1:38Thank you so much for giving us that background.
- 1:40So to get us started,
- 1:42could you tell us about your most successful project
- 1:45or initiative from the last year?
- 1:47What issues were you trying to solve?
- 1:49And also what drove the Success
- 1:52- Sale Health is, is, is focused around the whole concepts
- 1:55of teams and teams solving problems.
- 1:59Our initiative that I would, would address
- 2:02and speak to involves staff development,
- 2:06emotional and uh, clinical support development for
- 2:11all members of our, our staff,
- 2:13both nursing and medical staff.
- 2:16There are three main initiatives that I've been involved
- 2:18with over this last year.
- 2:22One of them involves a very innovative program and,
- 2:25and one that we have not modeled after anyone else,
- 2:30and it's, it is called Ours Program Staff Engagement
- 2:35and Resiliency Advocates,
- 2:37and there are five of them that are,
- 2:39are embedded within the organization right now.
- 2:42These are are counselors.
- 2:45They are crisis trained individuals.
- 2:50Four of the five are, are actually in the, come from the
- 2:55ministry profession and they're embedded within our,
- 2:59our main health system soon, other areas as well.
- 3:03And they're there to provide daily support for our nursing
- 3:08and physician staff.
- 3:09This, all this all was started initially in our emergency
- 3:13department, it spreading out to others,
- 3:15other areas of the hospital.
- 3:17And then we, we survey how they're doing
- 3:20and how our staff's doing using validated questions.
- 3:25That friend of ours
- 3:26and someone we work with closely,
- 3:28Brian Sexton at Duke developed.
- 3:31We look at emotional exhaustion, emotional thriving
- 3:35and emotional recovery for the three areas and,
- 3:39and the, the teams have had a direct impact on all three
- 3:43of those parameters within our institution.
- 3:46The other, the other program I had mentioned, second
- 3:49of three is our peer support program
- 3:52and some, some other institutions.
- 3:55It's called Second Victims Program.
- 3:57This is a unique program for us in that it has developed out
- 4:02of or in concert with our Sierra a program,
- 4:05and these are trained individuals that we train internally
- 4:09and they're there to reach out to staff, both physician
- 4:13and nursing staff when, when there are issues when,
- 4:19when a case goes badly
- 4:22and they're there to, they, they carry beepers
- 4:25and they're there literally to reach the individual
- 4:27and say, how are you doing?
- 4:29The third program is one that we've just entered our,
- 4:32our 13th year and it's PLI
- 4:37and PLI stands for a Physician Leadership Institute.
- 4:42It, it's a anachronistic in that it, the program
- 4:46reaches more than just our physicians.
- 4:48It involves our nursing staff, all
- 4:52of our senior executives board.
- 4:55We've, we've now graduated over 900
- 4:58leaders through this program.
- 5:00It's a four month long program, two days a month, Friday
- 5:03and Saturday, once a month.
- 5:05They work on quality improvement projects through their,
- 5:10their four month experience
- 5:11and then present them at the end of the program.
- 5:15What's important about this program is it develops culture
- 5:20and is an investment in people that go through it.
- 5:23We have a 18 month waiting list to get into the program
- 5:27is been a remarkable program in terms
- 5:29of shaping the conversation here within our health system.
- 5:33So it's cultural investment that, that, that I look to
- 5:38and that I spend my time involved with here at Salem.
- 5:42There's a cost to this and we have showed that cost
- 5:46and the results are showing the benefits of that.
- 5:52- Wonderful. Thank you so much for giving us all
- 5:54that information about a very successful project.
- 5:57It seems, as you know,
- 5:59the clinical workforce has changed a lot
- 6:00in the last few years.
- 6:02What challenges are you still facing
- 6:04and how do you see the clinical workforce evolving?
- 6:07- Everything has changed as a result
- 6:10of the last three and a half years.
- 6:13I'm, and I know I don't speak alone
- 6:18in this regard, but our, our nursing staff overnight, many
- 6:21of them changed.
- 6:24The senior experienced nurses chose outpatient work
- 6:28or in some cases left the profession.
- 6:31We have just a whole lot
- 6:35of new grads on our floors
- 6:38and my concern for the medical staff is
- 6:42that we not see the same thing happen,
- 6:45and that's a, a big effort of mine
- 6:49and the staff I work with here to support them
- 6:53to round, to be with them
- 6:56and hopefully we keep them within the practice of medicine.
- 7:01But I think what we're seeing as we come out of Covid is
- 7:05that this really is the most complex chapter in this book
- 7:09of Covid, the one we're in right now.
- 7:12There's just such significant change
- 7:15that's taken place culturally within the nursing and,
- 7:19and medical staffs.
- 7:21- Absolutely. Thank you so much for giving us that insight.
- 7:24And Doctor, before I let you go, last thing I wanted
- 7:26to ask you is, what is your best advice
- 7:29for aspiring physician nurse leaders?
- 7:32- Get outta your office, get out on the floor round.
- 7:38Spend time with staff.
- 7:40Actually talk to them and take the time to hear
- 7:42what they tell you and listen to their stories.
- 7:47They, their stories have meaning
- 7:49and it's important for us as leaders
- 7:52to take the time to hear those stories.
- 7:55- Yeah, absolutely.
- 7:56Thank you so much for sharing
- 7:58and giving us those final thoughts.
- 7:59Doctor, this has been an amazing
