Transcript
- 0:00Hello, everyone. This is Jacob Emerson with the
- 0:02Becker's Pay Your Issues podcast.
- 0:04Thrilled today to be joined by Zach Myers,
- 0:06who is the national general manager at ScanHealth
- 0:08Plan. Zach, thanks so much for taking the
- 0:10time to be with me on the podcast
- 0:12today.
- 0:13Yeah. Thank you.
- 0:14So before we dive into everything we wanna
- 0:16talk with you about, Zach, and all the
- 0:17latest going on at SCAN, can you tell
- 0:19us a little bit more about yourself, your
- 0:21background in health care, and what it is
- 0:23that you do at the company?
- 0:25Yeah. Sure. Well, as you shared, I'm SCAN's
- 0:27national general manager. I'm responsible for the performance
- 0:30and operations
- 0:31of our markets across the country, and and
- 0:33we're a growing health plan. We're now in
- 0:35five states, soon to be six,
- 0:38across the country with over 300,000 lives that
- 0:40we are responsible for the care of. You
- 0:43know, I started my career in health care
- 0:46technology. I kind of grew up in it,
- 0:48Jacob. I my family owned a company back
- 0:50in the day called CECity,
- 0:53terribly named business. We could have used scans
- 0:55marketing team, but,
- 0:58we built
- 0:59interesting technology in the early days of healthcare
- 1:02tech that helped primary care physicians measure the
- 1:06way
- 1:07they delivered care and the quality of care
- 1:08they were delivering and then helped them to
- 1:10get paid under the earliest pay for performance
- 1:13programs that CMS,
- 1:15put out.
- 1:16I helped lead the sale of that business
- 1:19to premiere in 2014
- 1:21and then spent really the next decade of
- 1:23my career in the world of Southern California.
- 1:26Senior care.
- 1:28Really devoted to the mission of how to
- 1:31improve the quality of life for seniors. First
- 1:33first at a company
- 1:34called CareMore Health, where I ran their largest
- 1:37P and L for a number of years.
- 1:39And then here at SCAN. So excited to
- 1:42to chat with you about all things
- 1:44payers, providers and and how to improve care
- 1:46for seniors.
- 1:47Yeah. Absolutely. So you've been in the industry
- 1:50for a long time, Zach. It sounds like
- 1:51it's personal for you as well. You've seen
- 1:54a lot of different trends and and different,
- 1:57directions the industry has gone over over the
- 1:59last decade or so. So you you mentioned
- 2:02Skin is going to be expanding into its
- 2:05sixth states. The the company continues to expand
- 2:07into new markets.
- 2:08Talk to us about some of the factors
- 2:10that you see driving future growth for Medicare
- 2:13Advantage right now. There's obviously so many headlines,
- 2:16so many changes we've been hearing about over
- 2:18the last few years within the program.
- 2:20So where do you see SCAN positioning itself
- 2:23to address some of the challenges and the
- 2:24opportunities in what what is, you know, is
- 2:27a very evolving market?
- 2:29Yeah. Yeah. Well,
- 2:31you know, despite all of the various headlines
- 2:34we've seen in the industry, I think the
- 2:35reality is that seniors have really
- 2:38voted for the MA program with their feet.
- 2:41Now we now have more than half of
- 2:43all seniors in the Medicare Advantage program in
- 2:46large part because they've seen the real value
- 2:48proposition right around having access to
- 2:51additional benefits, comprehensive
- 2:53coverage
- 2:54in particular
- 2:56for those seniors living on fixed incomes,
- 2:59who really depend on stable, affordable access to
- 3:01care that's not possible
- 3:03under
- 3:04a Medigap policy or Medicare fee for service
- 3:07alone.
- 3:08You know,
- 3:09I think even in the face of that
- 3:11growth, what we've seen is some real headwinds
- 3:13in the industry,
- 3:15margin compression,
- 3:16changes to the payment model in part because
- 3:18of v twenty eight,
- 3:20and a general decline in national stars ratings,
- 3:24across many plans.
- 3:27You know, most health plans have reacted to
- 3:29that
- 3:30by pulling out of the markets. I think
- 3:31we're going to see hundreds of thousands of
- 3:33seniors
- 3:34lose access to their health plan, this AEP,
- 3:38or really squeezing their provider partners.
- 3:41You know, we've seen real growth in claim
- 3:43denials,
- 3:44increasing
- 3:45use of burdensome prior authorization.
- 3:47And the result is really a vicious cycle.
- 3:50Right? You see
- 3:51a provider's less happy. You see plans pulling
- 3:54back on benefits. The result is those plans
- 3:56have worsening member experience, worsening star scores, and
- 3:59ultimately,
- 4:00you know,
- 4:02the the circle kind of continues. And I
- 4:04think where SCAN
- 4:06as a nonprofit
- 4:07health plan who's been in this industry for
- 4:09decades,
- 4:10we're positioned to take a different approach.
- 4:13We have the opportunity to reinvest our margin
- 4:17back into our members and our provider partners.
- 4:20And our vision is really to be the
- 4:23most preferred partner for providers,
- 4:25and health systems across the country.
- 4:28And what that allows us to do is
- 4:29create a virtuous cycle instead of
- 4:32the sort of downward spiral I was just
- 4:34describing. So so you see fair payment for
- 4:37providers
- 4:38not playing games with claim denials and prior
- 4:41auth, you know, creates better alignment with your
- 4:43groups.
- 4:44The result is a better member of experience
- 4:47and improved STARS performance.
- 4:49You know, at SCAN, we've seen that we've
- 4:51had,
- 4:52four plus star performance for for over a
- 4:54decade in in our core market in California
- 4:58and, ultimately,
- 4:59higher revenue that we can re invert reinvest
- 5:01in member experience and growth. And
- 5:04so we see a different way of approaching
- 5:07the market that will allow us to win
- 5:09in in the face of of all the
- 5:11change in the industry.
- 5:12Does does that make sense, Jake? It it
- 5:14does make sense, and it's it really aligns
- 5:16with everything that we consistently hear both from
- 5:19the hospital leaders and from the health plan
- 5:21leadership pretty much every day at this point,
- 5:23Zach, in terms of the squeezing of profit
- 5:25margins of of providers,
- 5:28the turmoil that everyone is expecting membership wise
- 5:31heading into AEP.
- 5:32But I know, you know, speaking from with
- 5:34SCAN leadership over the years, it's it's really
- 5:36great to consistently hear how the company acknowledges
- 5:39these industry problems and then tackles them head
- 5:42on,
- 5:43and and wants to to fix them. I'll
- 5:45just mention, you know, we the proof point
- 5:48for that from a scam perspective is we're
- 5:50now not really pulling out of any markets.
- 5:53We're expanding into the markets as AEP. And
- 5:55so you're gonna see us sort of with
- 5:57with hopefully market leading benefits,
- 6:00expanding into our sixth state, continue to grow
- 6:03in the states that we operate in. And,
- 6:05so we're excited about that. Yeah. No. It's
- 6:07really great to hear, and it definitely is
- 6:09in contrast to, obviously, a lot of other
- 6:11big players out out there. I wonder you
- 6:13know, I know there's a lot going into
- 6:15just this enrollment period, but if you were
- 6:17looking at the health care landscape in terms
- 6:19of how it's been evolving and how it
- 6:20will evolve over the next three to five
- 6:22years or so,
- 6:24is there anything in particular that stands out
- 6:26when it comes to Medicare Advantage? Any predictions,
- 6:28that you would wanna share? It's
- 6:33hard to predict where this I know. This
- 6:34you know, is it there's so many you
- 6:36know, it's at the whims of CMS in
- 6:38many ways. But, you know, I think we're
- 6:40we're gonna see
- 6:42Medicare Advantage
- 6:43continue to play an important role for seniors
- 6:47and for the government.
- 6:49I think it will evolve in pretty significant
- 6:51ways. I think plans are gonna have to
- 6:54adapt to tighter margins,
- 6:56which is gonna require
- 6:58being very thoughtful about how do you deliver
- 7:00meaningful value to seniors through the right benefits,
- 7:04not gimmicks,
- 7:06and then build the kind of provider partnerships
- 7:08that can actually bend the cost curve and
- 7:11keep patients independent and and healthy, which is,
- 7:14you know, fundamentally
- 7:15our mission.
- 7:16And I think part of that's gonna be
- 7:19the reality of changing technology. Of course, AI,
- 7:22But also, you know, we're seeing real novel
- 7:25medications, therapies, testing modalities.
- 7:28And at scan, we're thinking very creatively about
- 7:31how do you build sort of the care
- 7:32models of the future that can wrap around
- 7:35and support our provider partners
- 7:37to improve quality of life and create savings.
- 7:41But I think that's gonna be a really,
- 7:43like, narrow,
- 7:44line to to tiptoe here over the next
- 7:46few years. As a nonprofit, I think SCAN's
- 7:49pretty well positioned to
- 7:51to lead
- 7:52in in across all of those different changes
- 7:56that I that I just talked about. And,
- 7:58you know, we really wanna demonstrate how mission
- 8:01driven organizations
- 8:02can
- 8:04help set the standard
- 8:06for what Medicare
- 8:08becomes and looks like.
- 8:10Yeah. No. Absolutely. I mean and I appreciate
- 8:12your candidness around that topic as well there,
- 8:15Zach. And I I wonder if we're if
- 8:16we're sticking on the topic of caring for
- 8:18the most vulnerable, and certainly that applies to
- 8:21a big swath of the Medicare
- 8:23population.
- 8:24One area we've really seen a lot of
- 8:26growth, more than traditional Medicare Advantage, in terms
- 8:30of growth over the last few years is
- 8:32within special needs plans.
- 8:34And so can you talk to us about
- 8:36institutional special needs plans specifically? Because that's another
- 8:40segment, like I mentioned. We've seen a lot
- 8:41of growth there higher than normal MA.
- 8:44Where do you think is the opportunity there
- 8:46in terms of its continued expansion in that
- 8:49segment, continuing to see some of the growth
- 8:51margins we've seen there.
- 8:53What what happens there in the next few
- 8:55years?
- 8:56Yeah. I'm I I'm really excited to talk
- 8:58about the iSNP world because we view iSNP
- 9:01as the real maybe the best embodiment of
- 9:05SCAN's mission to keep seniors healthy
- 9:08and independent.
- 9:09You know, our our Embrace product is
- 9:13now the largest and fastest growing iSNP in
- 9:16California,
- 9:17and
- 9:18it's delivering markedly better patient experience
- 9:22to the members who it serves. Right? Facility's
- 9:25instinct
- 9:26often is to to call 911 at the
- 9:28first sign of any any trouble for their
- 9:31patients,
- 9:32because it's impossible for them to get the
- 9:34kind of rapid response they need to keep
- 9:37their,
- 9:38you know, the the folks living in their
- 9:40communities,
- 9:41at home.
- 9:42And our ISP
- 9:44program, which we call embrace
- 9:46has really developed a better model
- 9:48delivering care directly into the community available 20
- 9:52fourseven
- 9:53there to deliver,
- 9:54you know, higher level urgent care.
- 9:57And and even before it gets there to
- 9:59prevent
- 10:00exacerbations of chronic illness before they ever happen.
- 10:04You know, part of the reason we're seeing
- 10:05that level of growth
- 10:07is because there's
- 10:09been,
- 10:10an increasing recognition from community operators,
- 10:15that there's an important intersection between health care
- 10:18and the community,
- 10:20that keeping folks healthy,
- 10:23and in their building
- 10:24is both right for the for those members
- 10:27and also
- 10:28important for the economics of their,
- 10:31operations.
- 10:32And so we we think where ISNIP will
- 10:35go in the next number of years is
- 10:37is continued growth around
- 10:40community operators who see
- 10:42who see this vision and want to build
- 10:44the kind of transformational partnerships with SCAN,
- 10:47that can that can bring this to their
- 10:49community. So, you know, for example,
- 10:52we recently
- 10:54announced
- 10:55a partnership with Brookdale Senior Living, where we're
- 10:58expanding,
- 10:59across their 33 communities in California and Arizona
- 11:03and partnering
- 11:04around how can we keep
- 11:07their communities healthy in in our iSNP product.
- 11:10So we hope to replicate that model with
- 11:12many other operators in the years to come.
- 11:14Understood. So you foresee continued growth in this
- 11:17segment. You're partnering and want to continue partnering
- 11:20with these senior living facilities.
- 11:22And what was that, statistic you cited, Zach,
- 11:25about I SNP at the beginning being one
- 11:26of or the fastest growing for you in
- 11:29California?
- 11:30Embrace is the largest and fastest growing I
- 11:33SNP,
- 11:34in California.
- 11:36Fantastic. Well, Zach, as we, wind down on
- 11:39on time here, you've got the ears of
- 11:41of a lot of other health plan leaders
- 11:43listening in from all over the country.
- 11:45Everyone's, I think, feeling a little bit,
- 11:48nervous,
- 11:48excited going into AEP.
- 11:51So
- 11:52if you're speaking to them all directly,
- 11:54any final thoughts, final bits of advice as
- 11:56we we enter this, this period of the
- 11:59year?
- 12:00You know, I think,
- 12:01historically, plans and and providers have often
- 12:05seen
- 12:07ourselves as in competition that that that this
- 12:11health plan industry is a bit of a
- 12:13zero sum game. And and we
- 12:15take a different approach, and I personally believe
- 12:17that there's
- 12:18an opportunity to build the kinds of deep
- 12:21long term partnerships with providers that ultimately result
- 12:25in better health care and better economics for
- 12:27everyone. And
- 12:29that's
- 12:30the kind of partnerships that we're building
- 12:33now. We we recently announced our partnership with
- 12:36Sutter to do just that and think about
- 12:38how can we co design
- 12:40health plans and benefits that improve the life
- 12:42of providers
- 12:44as well as patients,
- 12:46and then get really deep into the way
- 12:48we deliver care to improve outcomes fundamentally for
- 12:51everyone. And so I would just say that
- 12:54there's an opportunity to do that for anyone
- 12:56who operates in this industry
- 12:59and to think about how we can make
- 13:01the system stronger as a whole by working
- 13:02together.
- 13:03Wonderful. Well, Zach, I really appreciate you taking
- 13:06the time to chat with us and for
- 13:08sharing your insights with our listeners. We really
- 13:09appreciate it. Thank you. I enjoyed the conversation.
- 13:13Yeah. Likewise. And and to our listeners, if
