- 0:00Good morning, everyone, and welcome to another episode of Morning Medical with Dr.
- 0:03Chow.
- 0:04I'm Dr.
- 0:05Chi-Ming Chow, your host for today, and as always, we're here to bring you the latest in
- 0:09medical and health news from around the globe, powered by artificial intelligence.
- 0:14Today, we're diving into a range of topics that are making headlines, and first up, we're
- 0:19discussing the current status of cyclosporiasis cases in Canada amidst a significant outbreak
- 0:25in the United States.
- 0:29Cyclosporiasis is an intestinal illness caused by the parasite cyclospora, which is typically
- 0:35transmitted through the consumption of contaminated food or water.
- 0:39This condition is characterized by symptoms such as watery diarrhea, abdominal bloating,
- 0:45gas, and an overall loss of appetite.
- 0:48While the United States is experiencing a notable outbreak, particularly in Michigan,
- 0:53it's important to understand how this situation is affecting Canada.
- 0:58In Quebec, health officials have reported an uptick in cyclosporiasis cases this year, with
- 1:03107 cases identified as of July 11th.
- 1:07This is a significant increase from the same period last year, which saw only 30 cases.
- 1:13However, it's crucial to note that most of these cases are linked to travel, primarily to
- 1:18Mexico, and not connected to the U.S.
- 1:20outbreak.
- 1:21Less than five of these cases involved individuals who had traveled to the United
- 1:26States.
- 1:28Similarly, British Columbia has reported 93 cases this year, which is considered typical as
- 1:34cases are reported annually.
- 1:38Most of these are related to travel, and while the situation is being monitored closely due to
- 1:43the U.S.
- 1:44outbreak, there isn't a direct link identified yet.
- 1:48Health officials in British Columbia have noted an increase in non-travel related infections,
- 1:54often due to the consumption of contaminated, imported raw produce during the spring and
- 1:59summer months.
- 2:01In Alberta, there have been six reported cases, with five identified within the past six weeks.
- 2:08These cases also coincide with the timeline of the U.S.
- 2:11outbreak, but investigations have shown no direct connections to travel to the United
- 2:15States.
- 2:17Instead, most cases were linked to travel to Mexico.
- 2:21Routine surveillance activities are ongoing to ensure any potential links to ongoing outbreaks
- 2:26are identified and addressed.
- 2:29It's interesting to note that provinces such as the Northwest Territories, Nova Scotia, New
- 2:35Brunswick, Saskatchewan, and Prince Edward Island have reported no cases of cyclosporiasis
- 2:40this year.
- 2:42Meanwhile, Nunavut has confirmed five cases, also with no evidence linking them to the U.S.
- 2:48outbreak or imported produce from the U.S.
- 2:52As of now, the Public Health Agency of Canada is not investigating any specific outbreaks of
- 2:57cyclosporiasis within the country.
- 3:00However, they do acknowledge that there is typically an increase in infections during the
- 3:05spring and summer months due to the consumption of fresh fruits and vegetables imported from
- 3:09other countries.
- 3:11It's important for Canadians to stay informed and cautious, especially during this time of
- 3:16the year when the risk of such infections can be higher due to imported produce.
- 3:22As always, thorough washing of fruits and vegetables and being mindful of travel-related
- 3:27exposures can help mitigate the risk of cyclosporiasis.
- 3:33In exciting news for cardiology, the Food and Drug Administration has approved the first ever
- 3:38oral PCSK9 inhibitor pill for lowering cholesterol.
- 3:43The drug, known as enlicitide and marketed under the name Lipfendra, provides a new
- 3:49non-invasive option for patients with high cholesterol or heterozygous familial
- 3:55hypercholesterolemia.
- 3:57This marks a significant advancement in the management of cholesterol, offering an
- 4:02alternative to the injectable PCSK9 inhibitors that have been available up until now.
- 4:08Enlicitide is taken as a 20-milligram tablet, once daily, and is recommended to be consumed
- 4:14after a six to eight-hour fast with no food intake for 30 minutes following the dosage.
- 4:20This oral administration offers a more convenient option for patients who may have
- 4:25been hesitant about injection-based treatments.
- 4:28The approval of enlicitide was based on the results from two pivotal phase three clinical
- 4:33trials, CORALreef Lipids and CORALreef HeFH.
- 4:37These trials demonstrated impressive reductions in low-density lipoprotein cholesterol, with
- 4:42enlicitide lowering levels by 56 percent and 59 percent, respectively, compared to placebo
- 4:48after 24 weeks.
- 4:50The trials collectively involved 3,207 participants, covering individuals with severe
- 4:56hypercholesterolemia, including those with a history or at high risk of atherosclerotic
- 5:01cardiovascular disease, as well as patients with heterozygous familial
- 5:05hypercholesterolemia.
- 5:07Furthermore, an additional phase three study, known as CORALreef AddOn, highlighted
- 5:13enlicitide's efficacy when compared with other oral nonstatin therapies such as ezetimibe and
- 5:20bempedoic acid, both alone and in combination.
- 5:25This positions enlicitide as a strong contender in the landscape of nonstatin
- 5:30cholesterol-lowering treatments.
- 5:33The introduction of an oral PCSK9 inhibitor is particularly noteworthy given the challenges
- 5:39faced with existing injectable therapies.
- 5:42Injectable PCSK9 inhibitors, including evolocumab, alirocumab, and inclisiran, though
- 5:49effective, have been underutilized partly due to their mode of administration.
- 5:54Enlicitide's pill form could potentially increase accessibility and adherence to
- 5:58treatment, thereby helping more patients achieve their cholesterol-lowering goals.
- 6:04Statins remain the cornerstone of cholesterol management, but they are not without their
- 6:08drawbacks, such as muscle pain and liver function abnormalities.
- 6:13Enlicitide offers an alternative with a different side effect profile.
- 6:17In the CORALreef trials, diarrhea and dizziness were more common with enlicitide than with
- 6:23placebo, but importantly, no excess safety events were noted in comparison to other
- 6:29treatments.
- 6:32Montreal Public Health has issued a warning regarding an alarming increase in overdose
- 6:36deaths linked to carfentanil, a synthetic opioid significantly more potent than fentanyl.
- 6:43Since April, carfentanil has been confirmed in eight suspected overdose deaths, though the
- 6:48actual number may be higher due to the time required for toxicology testing.
- 6:53This spike in overdoses has led to a public health alert, highlighting the ongoing crisis
- 6:58fueled by the presence of fentanyl and its analogues in the drug supply.
- 7:03The situation has become critical, with ambulance crews administering as much naloxone
- 7:08in the first half of 2026 as they did throughout the entire previous year.
- 7:13This underscores a sustained surge in overdoses, with non-fatal cases remaining
- 7:17unusually high since March.
- 7:21Dr.
- 7:21David Kaiser, a public health physician with Montreal Public Health, emphasized that while
- 7:27these substances are highly toxic, they still respond to naloxone, a medication that can
- 7:32reverse opioid overdoses.
- 7:35Public health officials are raising awareness not only among individuals who use drugs but
- 7:41also within the general public.
- 7:43During the summer months, more overdoses are occurring in public spaces such as parks and
- 7:49sidewalks, increasing the likelihood that bystanders could witness an overdose event.
- 7:55Dr.
- 7:55Kaiser urges anyone who encounters someone showing signs of an overdose, such as
- 8:00difficulty breathing or unresponsiveness, to call emergency services immediately and
- 8:05administer naloxone if available.
- 8:08The increase in overdose incidents is attributed to several factors, including an
- 8:13increasingly toxic drug supply and prolonged periods of hot weather, which can exacerbate
- 8:19the situation, particularly for those experiencing homelessness.
- 8:23Quebec is currently on track to record consecutive years with over 600 drug overdose
- 8:28deaths, a troubling statistic that calls for urgent action and awareness.
- 8:34In light of these developments, it's crucial for the community to stay informed and prepared
- 8:38to act in the event of an overdose.
- 8:41The presence of potent substances like carfentanil in the drug supply poses a
- 8:45significant risk, necessitating a collective effort to manage and mitigate this public
- 8:50health crisis.
- 8:52In recent news, the government in England has announced that from April, children under the
- 8:57age of sixteen will no longer be able to purchase high-caffeine energy drinks.
- 9:02This decision comes as part of a broader effort to address health concerns related to caffeine
- 9:07consumption among young people.
- 9:10The ban specifically targets drinks with more than one hundred fifty milligrams of caffeine
- 9:15per litre.
- 9:17This means popular brands like Red Bull, Monster, Relentless, and Prime, known for their
- 9:22high caffeine content, will be off-limits to younger teenagers.
- 9:26However, lower-caffeine soft drinks such as Diet Coke, as well as tea and coffee, are not
- 9:32affected by this new regulation.
- 9:35The rationale behind this move is to tackle issues like childhood obesity, sleep
- 9:40disturbances, increased anxiety, and concentration difficulties that have been
- 9:45linked to excessive caffeine intake.
- 9:48Public Health Minister Sharon Hodgson emphasized that this ban reflects a strong
- 9:53commitment to fostering a healthier generation of children.
- 9:57Enforcement of the ban, pending parliamentary approval, will be the responsibility of local
- 10:02authorities.
- 10:03Businesses found violating the law could face fines up to two thousand five hundred pounds.
- 10:08This measure is supported by data indicating that around one hundred thousand children in
- 10:14England consume energy drinks daily, with some brands containing more caffeine than two cups
- 10:20of coffee or four cans of cola.
- 10:24Experts have pointed out that young individuals are particularly sensitive to caffeine, as
- 10:29their bodies and brains are still developing.
- 10:32The consultation leading to this ban received over eleven hundred responses, showing strong
- 10:38public support for the age restriction.
- 10:42The legislation will be introduced through secondary legislation under the Food Safety Act
- 10:47of nineteen ninety.
- 10:49While the British Soft Drinks Association has described the ban as unnecessary, noting that
- 10:54high-caffeine beverages already carry warnings against consumption by children, the policy has
- 11:00been praised by health advocates.
- 11:02Katharine Jenner, Executive Director of the Obesity Health Alliance, called it a
- 11:07common-sense approach to safeguarding children during a critical stage of their development.
- 11:13The move to restrict these sales aligns with a growing awareness of the potential risks
- 11:17associated with high caffeine intake, such as rapid heart rates, abnormal heart rhythms, and
- 11:22even seizures, albeit rare.
- 11:25Additionally, the high sugar content in many energy drinks can contribute to obesity and
- 11:30dental issues.
- 11:32It's worth noting that similar bans are being considered in Northern Ireland, Scotland, and
- 11:36Wales, indicating a broader recognition of the need to protect young people's health across
- 11:41the United Kingdom.
- 11:44Our next story brings us to the realm of food safety, where shredded iceberg lettuce sold at
- 11:49some Taco Bell locations has been linked to a cyclospora outbreak.
- 11:54This outbreak is part of a larger surge of cyclosporiasis cases across the United States,
- 11:59with nearly seven thousand cases confirmed or under investigation since May first, according
- 12:04to the Centers for Disease Control and Prevention.
- 12:07The number of confirmed cases alone is more than six times higher than it was at this time
- 12:12last year, with at least one hundred forty-one hospitalizations reported.
- 12:16The outbreak connected to the lettuce is considered regional, primarily affecting the
- 12:20Midwest.
- 12:22The Centers for Disease Control and Prevention has identified at least four hundred cases
- 12:27associated with this outbreak across Michigan, Ohio, West Virginia, and Kentucky.
- 12:34These are the states where the affected Taco Bell locations are situated, although it's
- 12:38possible that the produce was distributed to other areas as well.
- 12:43Michigan has been particularly impacted, reporting over four thousand three hundred
- 12:48cases during the outbreak investigation.
- 12:51The Michigan state health department has conducted interviews with more than one
- 12:56thousand individuals as part of their investigation, noting that lettuce or salad
- 13:00greens could be a potential source.
- 13:04The concentrated increase in cases strongly suggests that the majority of illnesses are
- 13:09linked to the same outbreak, making it the largest cyclospora outbreak on record in the
- 13:16United States.
- 13:18In response, Taco Bell has taken immediate action, voluntarily removing potentially
- 13:22affected lettuce from their supply chain in select states.
- 13:26According to a statement from the company, the affected ingredient will be replaced within
- 13:30twenty-four hours in the impacted areas.
- 13:34Taylor Farms, the supplier of the lettuce, has not responded to requests for comment.
- 13:40Cyclosporiasis is typically not spread directly from person to person.
- 13:45Instead, infection occurs through the consumption of contaminated food or water.
- 13:51Previous outbreaks have often been linked to fresh produce.
- 13:55Symptoms of cyclosporiasis can include watery diarrhea, cramping, and bloating, which may
- 14:01persist for weeks.
- 14:03Federal health officials are conducting multiple investigations, some tied to the large
- 14:08outbreak in the Midwest, others involving single states or cases not yet linked to any
- 14:13specific cluster.
- 14:15Taylor Farms, known for supplying grocery stores and restaurants nationwide, has been
- 14:20associated with previous illness outbreaks, including E.
- 14:23coli cases linked to slivered onions in two thousand twenty-four and cyclospora cases
- 14:28linked to lettuce in two thousand thirteen.
- 14:32As we conclude today’s episode, it's crucial to remain vigilant regarding food safety,
- 14:38especially with fresh produce.
- 14:40Consumers should thoroughly wash fruits and vegetables and stay informed about potential
- 14:46outbreaks.
- 14:47Thank you for joining me on this episode of Morning Medical with Dr.
- 14:50Chow.
- 14:51Stay healthy and informed, and we'll see you next time.
