Speaker 1: psychiatrist and their staff about Jeffrey Epstein and the events
• 0:15
Speaker 1: leading up to his death. Background information Jeffrey Epstein was

• 0:47
Speaker 1: Health Treatment Coordinator, Central Office, and redacted Correctional Services Administrator

Speaker 1: Northeast Regional Office. The reconstruction was established in accordance with
• 0:58
Speaker 1: Bureau of Prisons Program Statement five three to four zero
• 1:02
Speaker 1: eight Suicide Prevention Program. Formal interviews were not conducted as
• 1:07
Speaker 1: part of this reconstruction at the direction of the Department
• 1:09
Speaker 1: of Justice. A copy of the video is normally made
• 1:13
Speaker 1: following a significant incident, but there was no such video
• 1:16
Speaker 1: in this case, since the original video was confiscated by
• 1:19
Speaker 1: the Federal Bureau of Investigation prior to the beginning of
• 1:23
Speaker 1: this reconstruction. The absence of these two areas of inquiry
• 1:27
Speaker 1: severely limited the ability to establish accurate timelines, confirm subjective reports,
• 1:33
Speaker 1: establish converging and diverging lines of facts, or discover new
• 1:37
Speaker 1: areas of inquiry. As a result, the information typically gathered, reviewed,
• 1:42
Speaker 1: and consolidated during a reconstruction to support actionable findings and
• 1:46
Speaker 1: recommendations is limited. Social history mister Epstein did not have
• 1:53
Speaker 1: a pre sentenced report available at the time of reconstruction. Therefore,
• 1:58
Speaker 1: no official information regarding social history was accessible. The following
• 2:03
Speaker 1: was gathered from publicly available documents. Mister Epstein was born
• 2:07
Speaker 1: in nineteen fifty three and grew up in a middle
• 2:10
Speaker 1: class family in the neighborhood of Seagate on Coney Island
• 2:14
Speaker 1: with one brother. After early promotion in two grades, mister
• 2:18
Speaker 1: Epstein graduated from Lafayette High School in nineteen sixty nine
• 2:22
Speaker 1: at the age of sixteen. He attended Cooper Union and
• 2:25
Speaker 1: New York University, but did not graduate from either. Mister
• 2:29
Speaker 1: Epstein taught at the Dalton School, a private school on
• 2:32
Speaker 1: the upper East side of Manhattan from September nineteen seventy
• 2:35
Speaker 1: four until he was dismissed in June of nineteen seventy
• 2:39
Speaker 1: six for inadequate development as a teacher. Following that, he
• 2:44
Speaker 1: held a number of positions in the financial industry, to
• 2:47
Speaker 1: include a position as a limited partner at Barr Stearns
• 2:51
Speaker 1: until he was dismissed for unknown policy violations in nineteen
• 2:55
Speaker 1: eighty one. He also worked as a financial consultant and
• 2:59
Speaker 1: found it out at least two separate companies. Mister Epstein
• 3:03
Speaker 1: had two significant periods of employment. The rest of these
• 3:06
Speaker 1: was his position as a consultant with Stephen Jude Hoffenberg
• 3:10
Speaker 1: in the late eighties. Mister Hoffenberg was described as his
• 3:14
Speaker 1: first mentor. Mister Hoffenberg was later convicted and incarcerated of
• 3:18
Speaker 1: running a large Ponzi scheme. He implicated mister Epstein in
• 3:23
Speaker 1: fraudently diverting company funds for his own personal use. Years later,
• 3:28
Speaker 1: Leslie Wexner, mister Epstein's sole client at Jay Epstein in Company,
• 3:32
Speaker 1: granted him power of attorney over his affairs. Despite also
• 3:36
Speaker 1: being identified as mister Wexner's mentee. Mister Epstein was again
• 3:40
Speaker 1: accused of misappropriating funds more than forty six million dollars.
• 3:44
Speaker 1: These large sums are believed to be seed money mister
• 3:47
Speaker 1: Epstein used to establish his considerable fortune. These events are
• 3:52
Speaker 1: indicative of mister Epstein's highly regarded intelligence and charismatic personality.
• 3:57
Speaker 1: Legal history, mister Epps seen at a history of adult
• 4:01
Speaker 1: criminal charges and convictions. In June two thousand and eight,
• 4:04
Speaker 1: he entered into a non prosecution agreement and pleaded guilty
• 4:08
Speaker 1: to one count of solicitation of prostitution and one count
• 4:12
Speaker 1: of procuring a person under the age of eighteen for
• 4:15
Speaker 1: prostitution in the state of Florida. He was sentenced to
• 4:19
Speaker 1: thirty months eighteen months of incarceration and twelve months of probation.
• 4:24
Speaker 1: He was also mandated to register as a sex offender
• 4:28
Speaker 1: under the National Sex Offender Registry and Notification Act. Mister
• 4:32
Speaker 1: Epstein served thirteen of his eighteen month incarceration and then
• 4:36
Speaker 1: successfully completed twelve months of probation. It's unclear whether he
• 4:41
Speaker 1: followed the sex offender registration guidelines in each place he
• 4:44
Speaker 1: owned a residence. In regard to the pending charges, mister
• 4:48
Speaker 1: Epstein was formally charged with sex trafficking conspiracy in violation
• 4:52
Speaker 1: of US Code eighteen, Section three seventy one and sex
• 4:56
Speaker 1: trafficking in violation of US Code eighteen, Section fifteen ninety
• 4:59
Speaker 1: one V one A, B, two and two. On July second,
• 5:04
Speaker 1: twenty nineteen. Specifically, he was accused of sexually exploiting and
• 5:09
Speaker 1: abusing minor females over the course of several years. Charging
• 5:13
Speaker 1: documents alleged mister Epstein enticed and recruited minor females to
• 5:18
Speaker 1: engage in sexual activity. The minor females were reportedly compensated
• 5:22
Speaker 1: with cash following the sexual encounters, and some were encouraged
• 5:26
Speaker 1: to find other minor females to accompany them to mister
• 5:30
Speaker 1: Epstein's residences in New York or Florida. He pleaded not
• 5:34
Speaker 1: guilty to the charges and was pre trial at the
• 5:37
Speaker 1: time of his death. In a thirty seven page decision
• 5:40
Speaker 1: an order remanding the defendant signed by Judge Richard M.
• 5:43
Speaker 1: Berman on July eighteenth, twenty nineteen. Eighteen pages were dedicated
• 5:49
Speaker 1: to detailing the danger mister Epstein posed to others and
• 5:53
Speaker 1: the community. The document also alleged that he was a
• 5:56
Speaker 1: flight risk. As a result, mister Epstein's proposed bellpackage was
• 6:00
Speaker 1: determined to be inadequate. He was denied pre trial release
• 6:05
Speaker 1: and held on remand this was likely significant to mister Epstein,
• 6:09
Speaker 1: as he would lose the physical and financial comfort to
• 6:12
Speaker 1: which he had become accustomed. Furthermore, his ability to mount
• 6:17
Speaker 1: the defense would be limited to the parameters of legal
• 6:20
Speaker 1: visits and legal work that could be accomplished in jail.
• 6:24
Speaker 1: These restrictions, and the abundance of witnesses, victims, and negative
• 6:28
Speaker 1: public attention were likely considerable burdens on mister epstein institutional history.
• 6:35
Speaker 1: On July six, twenty nineteen, mister Epstein was arrested at
• 6:38
Speaker 1: Teterborough Airport in New Jersey upon his return from Paris, France.
• 6:43
Speaker 1: It's unknown whether he was anticipating his arrest. He was
• 6:47
Speaker 1: transported to mcc New York and keaden the sentry at
• 6:51
Speaker 1: nine twenty four pm that evening. Mister Epstein was placed
• 6:54
Speaker 1: in general population housing for approximately twenty two hours. On
• 6:59
Speaker 1: July seventh, twenty nineteen, at approximately seven to twenty pm,
• 7:03
Speaker 1: he was moved to special housing unit pending reclassification. Due
• 7:07
Speaker 1: to the significant increase in media coverage and awareness of
• 7:11
Speaker 1: his notoriety among the inmate population. With regard to his adjustment,
• 7:16
Speaker 1: to a correctional setting. Mister Epstein received one incident report
• 7:19
Speaker 1: while in BP custody for self mutilation on July twenty third,
• 7:24
Speaker 1: twenty nineteen. As of August fifteenth, twenty nineteen, the incident
• 7:30
Speaker 1: report had been expunged, but it's unclear why and whether
• 7:33
Speaker 1: mister Epstein knew this. Also, a review of financial transactions
• 7:37
Speaker 1: associated with mister Epstein's prison account revealed one of his
• 7:41
Speaker 1: attorneys was depositing funds into his cellmate, inmate Rayu's commissary
• 7:45
Speaker 1: account for unknown reasons. The plot dickens, Doesn't it notice
• 7:50
Speaker 1: how nobody talks about that? Huh? Why the hell was
• 7:53
Speaker 1: Epstein giving mister rays money? Health Care and Personality description
• 8:01
Speaker 1: BOP electronic medical records indicate Epstein was diagnosed with hyper lipidemia,
• 8:06
Speaker 1: sleep apnea, hypertension, constipation, pre diabetes, neurologia, and neritis unspecified.
• 8:14
Speaker 1: He was prescribed the following medications docusate, sodium, milk of magnesia,
• 8:19
Speaker 1: Omega III, methyl pretozone, and biscodeal whatever that is. Mister
• 8:25
Speaker 1: Epstein was also prescribed insulin and the prescription required him
• 8:29
Speaker 1: to go to the Institution Pharmacy for administration of this medication. However,
• 8:35
Speaker 1: the dates for which it was prescribed have a notation
• 8:38
Speaker 1: indicating does not indicated, Thus it does not appear insulin
• 8:42
Speaker 1: was routinely medically necessary. The rest of the medications prescribed
• 8:47
Speaker 1: were self carry. He also had a continuous positive airway
• 8:50
Speaker 1: pressure machine, which is typically used to treat sleep apnea.
• 8:55
Speaker 1: Mister Epstein was provided with his personal seapap machine on
• 8:58
Speaker 1: July thirtieth to thouhgh per b e MR. In regard
• 9:04
Speaker 1: to mental health history and treatment, there are no known
• 9:07
Speaker 1: available records. Certainly, there were records kept on mister Epstein's
• 9:11
Speaker 1: incarceration in Florida. However, the records were not available for
• 9:15
Speaker 1: review as of the data this report. With regard to
• 9:19
Speaker 1: Psychology Data System records in b MR, doctor Kerry redacted,
• 9:23
Speaker 1: forensic psychologists at MCC's New York completed a routine intake
• 9:28
Speaker 1: screening on July eighth, twenty nineteen. During the screening, mister
• 9:33
Speaker 1: Epstein denied any history of mental health problems, substance abuse
• 9:37
Speaker 1: and treatment. No symptoms of mental illness were observed. He
• 9:41
Speaker 1: was classified as Mental Health Care Level one and was
• 9:45
Speaker 1: not diagnosed with mental illness. Following a consultation with doctor redacted,
• 9:51
Speaker 1: National Suicide Prevention Coordinator. On July eighth, twenty nineteen, Doctor redacted,
• 9:56
Speaker 1: chief psychologist at mcc New York, determine mister Epstein should
• 10:01
Speaker 1: be preemptively evaluated for suicide risk upon his return from court.
• 10:06
Speaker 1: Primary consideration was given to his various risk factors for suicide,
• 10:11
Speaker 1: such as his being a high profile case with media attention,
• 10:14
Speaker 1: his pending sex offense charges, pre trial status, and ongoing
• 10:18
Speaker 1: court proceeding. Mister Epstein returned from court on July eighth,
• 10:23
Speaker 1: twenty nineteen, after normal business hours. He denied suicidal thoughts
• 10:27
Speaker 1: at the time, but due to the potential for other
• 10:30
Speaker 1: risk factors listed above, the on call psychologist placed Epstein
• 10:35
Speaker 1: on psychological observation in one of the suicide watch cells
• 10:39
Speaker 1: until he could be assessed in person by a BOP psychologist.
• 10:43
Speaker 1: On July ninth, twenty nineteen, mister Epstein underwent a formal
• 10:47
Speaker 1: in person suicide risk assessment with doctor Adactid. She determined that,
• 10:52
Speaker 1: while on suicide watch was not warranted at the time,
• 10:55
Speaker 1: mister Epstein should remain on psychological observation status out of
• 10:59
Speaker 1: an abundance of caution. He was removed from psychological observation
• 11:03
Speaker 1: on July tenth, twenty ten. On July twenty third, twenty nineteen,
• 11:08
Speaker 1: Doctor redacted, the on call psychologist, was notified mister Epstein
• 11:13
Speaker 1: had been found in his cell with a piece of
• 11:15
Speaker 1: orange cloth around his neck. Reportedly, he was observed lying
• 11:19
Speaker 1: in the feudal position on the floor with a noose
• 11:22
Speaker 1: around his neck. Medical staff evaluated mister Epstein and found
• 11:26
Speaker 1: friction marks and superficial reddening of the neck and one knee.
• 11:31
Speaker 1: He was placed on suicide watch by the Operations lieutenant
• 11:34
Speaker 1: at approximately one forty am, pending a formal in person
• 11:38
Speaker 1: suicide risk assessment. Doctor adacted staff psychologist at mcc New
• 11:44
Speaker 1: York assessed mister Epstein for risk of suicide. Later in
• 11:47
Speaker 1: the morning of July twenty third, twenty nineteen. He determined
• 11:51
Speaker 1: that he should remain on suicide watch. Mister Epstein denied
• 11:55
Speaker 1: any knowledge of how he received marks on his neck
• 12:00
Speaker 1: and initially informed staff that he thought that his cellmate
• 12:03
Speaker 1: had attempted to kill him. Special Investigative Services staff opened
• 12:08
Speaker 1: an investigation to assess mister Epstein's safety and collect facts
• 12:11
Speaker 1: surrounding the episode. Despite the investigation, staff were unable to
• 12:16
Speaker 1: determine whether he was assaulted or engaged in self directed violence.
• 12:21
Speaker 1: Mister Epstein was removed from suicide Watch on July twenty fourth,
• 12:25
Speaker 1: twenty nineteen, after thirty one hours and five minutes. Thereafter,
• 12:30
Speaker 1: he remained in the suicide Watch cell and was placed
• 12:33
Speaker 1: on psychological observation, where he remained housed until July thirtieth,
• 12:37
Speaker 1: twenty nineteen. According to pds B E MR records, a
• 12:42
Speaker 1: discrepancy exists regarding when he was removed from psychological observation.
• 12:47
Speaker 1: His cell assignment per century indicates that he was transferred
• 12:51
Speaker 1: back to the special housing unit on July twenty ninth,
• 12:53
Speaker 1: twenty nineteen, whereas PDS B E MR indicates that he
• 12:59
Speaker 1: was removed from psychological observation on July thirtieth, twenty nineteen,
• 13:04
Speaker 1: at approximately eight fifteen am. Mister Epstein attended a court
• 13:09
Speaker 1: hearing on July thirty one, twenty nineteen, and upon his return,
• 13:13
Speaker 1: the United States Marshall Services provided paperwork to Receiving and
• 13:17
Speaker 1: Discharge that noted suicidal tendencies. Doctor redacted was notified on
• 13:22
Speaker 1: August first, twenty nineteen about the paperwork. She consulted with
• 13:27
Speaker 1: doctor redacted and then met with Epstein to conduct a
• 13:30
Speaker 1: suicide risk assessment. She determined suicide watch was not warranted
• 13:34
Speaker 1: at the time. Epstein remained classified as a mental health
• 13:38
Speaker 1: care Level one throughout his time at mcc New York.
• 13:41
Speaker 1: During his contact with psychologist, Epstein routinely denied current mental
• 13:45
Speaker 1: health symptoms, including suicidal ideation, and he did not exhibit
• 13:50
Speaker 1: symptoms of serious mental illness. His most frequent complaint was
• 13:54
Speaker 1: difficulty sleeping. He did not have access to his seapat
• 13:57
Speaker 1: machine until it was reportedly provided to him on July thirtieth,
• 14:01
Speaker 1: twenty nineteen. Mister Epstein also reported he was bothered by
• 14:05
Speaker 1: noise in the shoe at times. He noted concerns related
• 14:09
Speaker 1: to his safety in the shoe or in general population
• 14:12
Speaker 1: housing unit. On two occasions, July twenty sixth, twenty nineteen
• 14:17
Speaker 1: and July twenty seventh, twenty nineteen, he described himself as
• 14:21
Speaker 1: a coward and as someone who does not like pain.
• 14:24
Speaker 1: On July twenty eighth, twenty nineteen, he told doctor redacted
• 14:27
Speaker 1: the toilet seat in his cell would not stop flushing
• 14:30
Speaker 1: for an extended period of time, and then he took
• 14:32
Speaker 1: to sitting in the corner with his hands over his ears.
• 14:36
Speaker 1: Mister Epstein indicated he was agitated following this incident and
• 14:40
Speaker 1: was unable to sleep that night. All right, folks, we're
• 14:43
Speaker 1: gonna wrap up right here, and in the next episode
• 14:45
Speaker 1: dealing with the topic, we're gonna pick up where we
• 14:47
Speaker 1: left off. All the information that goes with this episode
• 14:50
Speaker 1: can be found in the description box. What's up, everyone,
• 14:54
Speaker 1: and welcome to another episode of the Epstein Chronicles. In
• 14:58
Speaker 1: this episode, we're going to pick up where we left
• 15:00
Speaker 1: off with the psychological report on Jeffrey Epstein's death at
• 15:06
Speaker 1: the sedents circumstances. Mister Epstein entered BOP custody on July six,
• 15:12
Speaker 1: twenty nineteen, with the history of convictions for sexual offenses
• 15:16
Speaker 1: and allegations comprised of more serious charges. The current indictment
• 15:21
Speaker 1: alleged sexual crimes against minors, and he was facing up
• 15:24
Speaker 1: to forty five years in prison. On July eighteenth, twenty nineteen,
• 15:28
Speaker 1: mister Epstein's request for bell and pre trial release was denied.
• 15:32
Speaker 1: On July twenty three, twenty nineteen, mister Epstein was found
• 15:35
Speaker 1: on a responsive in his cell. The motivation and context
• 15:39
Speaker 1: were never fully determined. After thirty one hours and five
• 15:43
Speaker 1: minutes on suicide watch, He was then placed on psychological observation.
• 15:48
Speaker 1: On July thirtieth, twenty nineteen, mister Epstein was removed from
• 15:52
Speaker 1: psychological observation and needed to be housed with an appropriate
• 15:55
Speaker 1: cell mate. This email was sent to seventy one on
• 16:00
Speaker 1: mcc New York staff, and as of August thirteenth, twenty nineteen,
• 16:05
Speaker 1: only twenty seven staff members had opened the message. On
• 16:09
Speaker 1: August ninth, twenty nineteen, a federal court unsealed approximately two
• 16:13
Speaker 1: thousand pages of documents into the public domain. These included
• 16:18
Speaker 1: graphic allegations against mister Epstein. Included was a book order
• 16:23
Speaker 1: receipt for titles such as SM one oh one, A
• 16:26
Speaker 1: Realistic Introduction slave Craft, road Maps for Erotic Servitude and
• 16:31
Speaker 1: Training with Miss Abernathy, a workbook for erotic slaves and
• 16:36
Speaker 1: their owners. Additional high profile public figures were also named.
• 16:41
Speaker 1: The documents were part of a defamation lawsuit filed by redacted,
• 16:45
Speaker 1: a woman who alleged mister Epstein had victimized her against
• 16:49
Speaker 1: a British socialite Glaion Maxwell, who was mister Epstein's ex girlfriend,
• 16:53
Speaker 1: associate and alleged madam. With the release of the information,
• 16:57
Speaker 1: it became increasingly clear that mister Epstein's chance of avoiding
• 17:01
Speaker 1: a lengthy period of incarceration and a broader public understanding
• 17:05
Speaker 1: of his private life were inevitable. Mister Epstein was afforded
• 17:10
Speaker 1: telephone calls on two different days. No records of the
• 17:13
Speaker 1: call exist, and it's not known with whom he was speaking.
• 17:17
Speaker 1: One occurred on or around July sixteenth, twenty nineteen, and
• 17:20
Speaker 1: the other on August ninth, twenty nineteen. Given the limited
• 17:25
Speaker 1: information known about mister Epstein, knowledge of the content of
• 17:28
Speaker 1: the calls would have been crucial to helping staff work
• 17:31
Speaker 1: with them. Following his final telephone call on the evening
• 17:35
Speaker 1: of August ninth, twenty nineteen, mister Epstein was moved into
• 17:39
Speaker 1: his shoe cell. He was single cells at the time
• 17:42
Speaker 1: because his cell mat, Ephren Reyus, did not return from court.
• 17:46
Speaker 1: The need for a cell mate was communicated between day
• 17:49
Speaker 1: watch and evening Watch ships in the shoe, but no
• 17:52
Speaker 1: cell mat was placed with them by the e W staff.
• 17:55
Speaker 1: BOP staff knew mister Epstein's cellmate had not returned from
• 17:59
Speaker 1: court as as early as one fifty pm that day.
• 18:02
Speaker 1: A review of the thirty minute round forms indicate unit
• 18:05
Speaker 1: rounds were not completed for the entire MW shift on
• 18:08
Speaker 1: August tenth, twenty nineteen. However, a memorandum from Lieutenant Stanley
• 18:13
Speaker 1: Jean indicates officer Toavin Nowell and Material Handler Supervisor Michael
• 18:18
Speaker 1: Thomas made a statement after mister Epstein's death that they
• 18:21
Speaker 1: did not complete proper thirty minute rounds at three AM
• 18:26
Speaker 1: or five am description of the scene. A detailed description
• 18:31
Speaker 1: of the scene was unavailable because the officers who discovered
• 18:34
Speaker 1: mister Epstein did not write memorandums and cannot be interviewed.
• 18:38
Speaker 1: According to the report of incident, on August tenth, twenty nineteen,

Speaker 1: saving measures. Arriving staff alleged they brought an automated external
• 19:01
Speaker 1: defibrillator and stretcher. Cardio pulmonary recitation CPR reportedly continued while
• 19:08
Speaker 1: the AED was placed on Epstein. The AED reportedly indicated
• 19:12
Speaker 1: no shock advised and CPR was continued. Mister Epstein was
• 19:16
Speaker 1: escorted to health services at approximately six thirty nine am
• 19:21
Speaker 1: and emergency medical services arrived at six forty three am.
• 19:25
Speaker 1: He was transported to the local hospital at approximately seven
• 19:28
Speaker 1: ten am. Mister Epstein was pronounced deceased at seven thirty
• 19:32
Speaker 1: six am. It was not possible to confirm this timeline
• 19:36
Speaker 1: without reviewing video footage. Conclusions recommendations. A general appreciation of
• 19:42
Speaker 1: risk factors for suicide with sex offenders is necessary when
• 19:46
Speaker 1: reviewing mister Epstein's death. There are several common factors that
• 19:51
Speaker 1: increase risk for suicide and individuals with a history of
• 19:54
Speaker 1: sexual offense. These include stigma due to the nature of
• 19:58
Speaker 1: sexually based crimes, both within society and the prison system,
• 20:03
Speaker 1: a disruption of the ability to utilize sex as a
• 20:05
Speaker 1: coping mechanism, which can lead to increased levels of distress
• 20:09
Speaker 1: and negative effect, and grief about loss experienced in regards
• 20:14
Speaker 1: to arrest. The grief may be secondary to the loss
• 20:17

Speaker 1: Mister Epstein was a high profile pre trial detainee awaiting

Speaker 1: businessman with a number of high profile acquaintances that he
• 20:52
Speaker 1: accumulated through a combination of charisma, charm, and intelligence. Despite
• 20:57
Speaker 1: his many associates, he had limited significant or deep interpersonal ties.
• 21:03
Speaker 1: Although mister Epstein appeared to cultivate a large social and
• 21:07
Speaker 1: professional network, he was estranged from his only brother. Indeed,
• 21:11
Speaker 1: his identity appeared to be based on his wealth power
• 21:14
Speaker 1: and association with other high profile individuals. Approximately two and
• 21:19
Speaker 1: a half weeks before his death, mister Epstein appeared to
• 21:21
Speaker 1: attempt suicide, but ultimately denied it was a suicide attempt.
• 21:27
Speaker 1: He was convincing in his denial at the time. He
• 21:30
Speaker 1: was saved because his cellmate notified BOP staff. In the
• 21:34
Speaker 1: weeks before his death, he made statements that he was
• 21:36
Speaker 1: a coward and it was having difficulty adapting to his
• 21:39
Speaker 1: diminished circumstances. He also frequently referenced poor sleep and an
• 21:44
Speaker 1: inability to tolerate the noise of prison. On the day
• 21:47

• 21:50

• 22:16

• 22:21
Speaker 1: Priorities should be given to inmates with the history of

• 23:24
Speaker 1: was a high profile inmate, an ex police officer charged
• 23:27
Speaker 1: in multiple murders. However, he and Epstein did not share
• 23:31
Speaker 1: risk associated with being sex offender, and their pairing may
• 23:34
Speaker 1: have aggravated mister Epstein's risk for self directed violence. In
• 23:39
Speaker 1: an effort to treat mister Epstein the same as other inmates,
• 23:41
Speaker 1: a statement repeated by multiple staff, executive staff may have
• 23:45

• 23:50
Speaker 1: associated with individuals who have been charged with, convicted, or
• 23:54
Speaker 1: convicted of a sex offense. On July twenty fifth, twenty nineteen,
• 23:59
Speaker 1: doctor adacted sent an email to redacted Associate Warden explaining
• 24:04
Speaker 1: a consultation between doctor redacted and doctor redacted National suicide
• 24:09
Speaker 1: Prevention Coordinator. In the email, doctor redacted reviewed and consulted
• 24:14
Speaker 1: and recommended from the Psychology Services Branch Central Office that
• 24:18
Speaker 1: mister Epstein be housed with another inmate who had also
• 24:22
Speaker 1: been accused of committing a sex offense. There is no
• 24:25

• 25:18
Speaker 1: In contrast, Officer Wilson Silva, who responded to the emergency,

Speaker 1: wrote in a memorandum dated July twenty third, twenty nineteen.
• 25:26
Speaker 1: In the memorandum, Officer with Silva wrote he saw mister
• 25:29
Speaker 1: Epstein laying down near his bunk with what appeared to
• 25:32
Speaker 1: be a piece of handmade orange cloth around his neck.
• 25:36
Speaker 1: It's critical that all descriptions of the incident accurately reflect
• 25:40
Speaker 1: objective evidence. Officer adacted wrote mister Epstein an incident report
• 25:46
Speaker 1: for self mutilation on July twenty third, twenty nineteen, after
• 25:50
Speaker 1: he was found on responsive in his cell. A Special
• 25:53
Speaker 1: Investigative Services threat assessment was completed on August two, twenty nineteen,
• 25:58
Speaker 1: but results were inconclusive as to whether mister Epstein engaged
• 26:02
Speaker 1: in self directed violence, willingly fought with the cell mate,
• 26:06
Speaker 1: or was assaulted by a cell mate. Although the incident
• 26:09
Speaker 1: report was written in a timely manner, it presumed the
• 26:12
Speaker 1: engaged in self directed violence and neglected to consider the
• 26:16
Speaker 1: other options of fighting an assault. Since it was unclear
• 26:20
Speaker 1: what had taken place in mister Epstein's cell, there should
• 26:23
Speaker 1: have been three incident reports written, one alleging mister Epstein
• 26:26
Speaker 1: engaging in self mutilation, one alleging mister Epstein participated in
• 26:31
Speaker 1: a fight or assault without injury, and a third alleging
• 26:34
Speaker 1: mister Tartaglione participated in a fault or a fight or

• 28:27
Speaker 1: entries from one forty am to six am into the

• 28:35
Speaker 1: this makes it appear as if she was the one
• 28:37
Speaker 1: conducting the watch. This information was discovered and conveyed in
• 28:41
Speaker 1: an email from misredacted associate Warden to doctor redacted with
• 28:46
Speaker 1: a carbon copied a Warden redacted on August twelfth, twenty nineteen.
• 28:50
Speaker 1: Of note, misredacted did not make an entry explaining why
• 28:53
Speaker 1: she was making the logbook changes. Additionally, misredacted then wrote
• 28:57
Speaker 1: entries for six fifteen, six thirty, six forty five and

• 29:05
Speaker 1: not part of the original entries made by Officer redacted,
• 29:08
Speaker 1: nor was misredacted assigned to work suicide watch post. It's
• 29:12
Speaker 1: recommended that if a staff member makes an entry error,
• 29:16
Speaker 1: the staff members should describe the error in the correct logbook,
• 29:20
