- 0:04>> Dr. Terry Simpson: This is part four of our series, Food Isn't a
- 0:06Prescription Pad. And right at the start, let's be
- 0:09clear. Food is not medicine. Yes, food is
- 0:14powerful. Yes, food can lower risk, but food is
- 0:17not a replacement for medicine. Clearly, diets
- 0:20rich in ultra processed food lead to more
- 0:22problems. Heart disease, obesity, diabetes,
- 0:25cancers. And the most empowering thing a person
- 0:28can do is, is eat a better diet. Some people think
- 0:32we never had chronic disease before modern times.
- 0:35They blame food. But then they extend their logic
- 0:39to say that if we just had good food, we wouldn't
- 0:41need medicine at all. So let me give you an
- 0:44example of why that isn't true. One of the most
- 0:47powerful stories in all of medicine. It's the
- 0:52early 1920s. Dr. Frederick Banting and his
- 0:55assistant Charles Best are working in Toronto, and
- 0:58they made a discovery that would change the world.
- 1:02At that time, children with diabetes were doomed.
- 1:06The only treatment was a starvation diet so low in
- 1:10calories that a child might live a few extra
- 1:12months. But death was certain. Parents sat
- 1:16helplessly as children wasted away. Hospital wards
- 1:19were filled with kids in diabetic coma, Families
- 1:23waiting for the end. Then came insulin. Dr.
- 1:28Banding went to one of these hospitals and began
- 1:31to inject these children who were laying down
- 1:33comatose. And this is one of the most famous
- 1:37moments in all of medicine. One by one, the
- 1:41comatose children began to stir. They sat up, they
- 1:46asked for food, and they hugged their parents.
- 1:50That discovery was so profound, that moment so
- 1:53profound that Dr. Banti became the fastest person
- 1:57to ever win the Nobel Prize. After medical
- 1:59discovery, insulin became one of the fastest
- 2:03adopted drugs in the history of that time. And
- 2:07here's the lesson. Those children had the best
- 2:11food their families could provide. But food wasn't
- 2:14enough. Only medicine. Insulin saved their lives.
- 2:24Today, we're going to make sense of the madness of
- 2:26diet and diabetes. I am your Chief Medical
- 2:29Explanationist, Dr. Terri Simpson, and this is
- 2:31Fork U Fork University, where we bust a few myths,
- 2:36make sense of the madness, and teach you a little
- 2:38bit about food and medicine.
- 2:45Let's start with beans. Beans are a legume, and
- 2:49other legumes are some of the best foods that you
- 2:51can eat to help control your blood sugar. They're
- 2:54packed with soluble fiber, which binds glucose and
- 2:57slows absorption in your gut. They're rich in
- 2:59plant protein, which keeps energy steady. And they
- 3:02have a low glycemic index so they don't spike
- 3:05blood sugar like white bread or soda. Studies show
- 3:08that people who eat beans regularly can lower
- 3:11their hemoglobin A1C that long term marker of
- 3:13blood sugar, about 0.3 to 0.5%. That is a real
- 3:18effect from food. And beans aren't just healthy,
- 3:22they're practical. They're inexpensive, available
- 3:25everywhere. And humans have probably eaten beans
- 3:27longer than any other protein source. We're
- 3:31natural hunters. Not really. I mean, I couldn't
- 3:35hit the broadside of a barn. But beans, well,
- 3:37beans were always there. And in the Mediterranean
- 3:40region for many years, including the times of the
- 3:43gladiators, beans were the primary source of
- 3:47protein. Before I compare beans to medicine, let's
- 3:50step back. There are two types of diabetes. Type 1
- 3:53diabetes is where the pancreas loses its ability
- 3:56to make insulin. It's called juvenile diabetes.
- 3:59That was the diabetes that we talked about with
- 4:01Dr. Banting. And best. Usually it is an immune
- 4:04system that destroys the insulin producing cells
- 4:06of the pancreas. And without insulin, glucose
- 4:09cannot enter your cells. Glucose builds up in the
- 4:12bloodstream. Without insulin, life cannot
- 4:15continue, which is why every type 1 diabetic
- 4:18patient needs insulin from the day of diagnosis.
- 4:22Type 2 diabetes, which used to be called adult
- 4:25onset, the pancreas still makes insulin, but the
- 4:28body cells are resistant to it. So the pancreas
- 4:31works harder and harder. And over time, the
- 4:34pancreas can wear out, which is why many type 2
- 4:37diabetics eventually need insulin too, because
- 4:40that's the natural course of the disease. So
- 4:42whether it's type 1 or type 2 food has never been
- 4:46enough. Back to beans. They help, but they are not
- 4:50a drug we call metformin. Metformin is the first
- 4:54line drug for type 2 diabetes. It lowers A1C by
- 4:58about 1 or 2%. That's two to four times more than
- 5:02beans. And metformin also reduces your risk of
- 5:05heart disease, and it has decades of safety data
- 5:08behind it. So, yeah, beans are great, but if your
- 5:11A1C is 9 or 10, no amount of chili or hummus is
- 5:14going to bring you to target. Medicine does that.
- 5:18Food lays the foundation. Medicine does the heavy
- 5:21lifting. And to be clear, the best overall diet
- 5:24for diabetes isn't low carb or keto or carnivore.
- 5:27It isn't some fad diet where you cut out entire
- 5:30food groups. The best diet for diabetes, shown
- 5:33again and again and again in multiple studies
- 5:35across many populations, is the Mediterranean
- 5:38diet. That means fruits, vegetables, uh, olive
- 5:41oils, nuts, legumes, beans, and whole grains. Now,
- 5:46whole grains might sound controversial, especially
- 5:48to those who grew up in the era of low carb
- 5:50madness, but the science is clear. Whole grains
- 5:54are and continue to be a healthy choice for
- 5:57patients with Diabetes. In fact, whole grains are
- 5:59a lot healthier than bacon. But the Mediterranean
- 6:02diet doesn't lower blood sugar, lowers
- 6:04cholesterol, improves blood pressure, reduces
- 6:06inflammation, protects the heart. And unlike crash
- 6:08diets, it's sustainable. It's a way people can and
- 6:11do eat for a lifetime. But even the Mediterranean
- 6:15diet is not medicine. Here's some practical tips
- 6:19for beans. So how do you actually use them to your
- 6:21advantage? Swap ground beef for black beans and
- 6:24chili. Add lentils to soups or stews, which
- 6:26thicken them beautifully. Roast chickpeas for a
- 6:29crunchy snack. Use hummus instead of cheese
- 6:31spread. And toss peas in rice and pasta or grain
- 6:34bowls. Those are simple swaps. They add up to
- 6:36lowering blood sugar, lowering cholesterol, and
- 6:38and keeping patients feeling full. And when
- 6:41they're a part of the Mediterranean diet, the
- 6:43effect is multiplied. All right, scam time. Keto
- 6:48and carnivore diets get hyped as diabetic cures.
- 6:51They can lower glucose in the short term, but at
- 6:53the cost of raising saturated fat. For people with
- 6:57diabetes already at higher risk of heart disease,
- 6:59that's dangerous. It's pouring gasoline on the
- 7:02fire. And the insulin claim? False. Protein spikes
- 7:07insulin. So the idea that no carbs means no
- 7:10insulin is a fantasy. And then there's cinnamon
- 7:14supplements. Cinnamon and oatmeal. Delicious
- 7:17cinnamon capsules as a cure. Worthless. The
- 7:20evidence is minimal. If someone's selling you
- 7:22that, they're selling you snake oil and not
- 7:25science. So here's the truth. Beans are not
- 7:29metformin. Yes, food matters. Yes, food lowers
- 7:33risk. Food empowers you to take control. But food
- 7:36is not medicine. Food is food. Medicine is
- 7:40medicine. Together, they save lives. This has been
- 7:47episode four of, uh, food isn't a prescription
- 7:50pad. Next time, when green tea and broccoli aren't
- 7:53chemotherapy, why Prevention is not the same as
- 7:56cure. I'm Dr. Terry Simpson, your chief medical

- 8:47chickpeas. We have that most of the time, gets
- 8:50made into hummus. Also cowboy, uh, caviar, great
- 8:55beans.
