- 0:34This is Scott Becker with the Becker's Health
- 0:37Care podcast.
- 0:38We're thrilled to be joined by the go
- 0:40to person for getting updates on what's going
- 0:42on in the payer world.
- 0:44We're joined today by Jacob Emerson.
- 0:47Jacob joins us regularly to talk to us
- 0:49about the biggest issues in the payer universe
- 0:52and and that are affecting health care and
- 0:54then involve the payer arena.
- 0:56Jacob, want to tee us up, and you
- 0:58tell us what are the big issues that
- 1:00you're facing currently or watching currently.
- 1:03Yeah. Hey, Scott. It's great to talk with
- 1:04you. So I thought the the first thing
- 1:07we could talk about right off the bat
- 1:09is just everything we're seeing right now with
- 1:11Medicare Advantage enrollment across the country.
- 1:14We just finished up the annual enrollment period
- 1:16this last fall, and and now we're starting
- 1:19to get the data back. Well, we have
- 1:20the data back from CMS
- 1:22on on who won, who didn't,
- 1:24and some wider trends about ultimately where seniors
- 1:27are deciding to go for their for their
- 1:29health coverage,
- 1:30which
- 1:31is looking a lot different than I think
- 1:33it ever has in the past for this
- 1:35program,
- 1:36where the big guys lost a lot somewhat
- 1:39purposefully.
- 1:40And then,
- 1:41the the smaller plans, the start up plans,
- 1:43nonprofits,
- 1:44the hospital system plans,
- 1:46all did very well or at least better
- 1:49than they have in the past. And so
- 1:52just interesting, I I think, reversal,
- 1:55and then also some declines in in some
- 1:57states for the first time ever for Medicare
- 1:59Advantage. So just a lot of different
- 2:01patterns and, I think, messages that came out
- 2:04for us here on, you know, the analysis
- 2:06side
- 2:07of of what's going on with,
- 2:09is a very quickly evolving program.
- 2:12And talk to us a little bit. Is
- 2:14overall enrollment in Medicare Advantage up or even,
- 2:17or where does that stand? And then tell
- 2:19us a little bit. Give us some more
- 2:20details on the winners, the losers.
- 2:23And I and you're right. To call them
- 2:25winners and losers might be wrong because some
- 2:27of the payers, the largest payers, purposely
- 2:30decrease their exposure to Medicare Advantage. But but
- 2:33talk about who's gone up, who's down, and
- 2:34what do you see?
- 2:36Yeah. It is it is growing still. Medicare
- 2:38Advantage overall,
- 2:40but it's slowing, especially compared to years past
- 2:43where
- 2:44you used to see anywhere
- 2:553.6%
- 2:56growth rate last year, and then
- 2:59so far, we've just seen two and a
- 3:00half percent for this last enrollment period,
- 3:03which is just over 35,000,000
- 3:05people. So
- 3:06not slowing down,
- 3:08but
- 3:09slowing down in a way where
- 3:11the the blockbuster
- 3:13growth at all costs that we saw from
- 3:15some of the major insurers,
- 3:17United, Aetna, Ellevance,
- 3:20that that is reversing, and now it's going
- 3:22back towards
- 3:23some of these other plans that in the
- 3:25past were still pretty small. Let's say, SCAN
- 3:28Group over mostly in California,
- 3:30Alignment Healthcare, which is in a few different
- 3:32states,
- 3:33and some of the other system plans,
- 3:36those are all growing. And and now I
- 3:38I talked with SCAN just, this this past
- 3:41week, and they they just shot up. They're
- 3:42now in the top 10,
- 3:44largest Medicare Advantage companies in the country from
- 3:47from number 14. So so so to answer
- 3:49your question, Scott, it is still growing, but
- 3:51it's but it's not growing as fast as
- 3:53it once was.
- 3:54Traditional Medicare enrollment actually grew by 600,000
- 3:57people,
- 3:58which was a reversal after years of decline.
- 4:02And then we're also seeing I think we
- 4:04briefly talked about this previously, but we're just
- 4:06seeing major growth or or a reason for
- 4:08that the MA program is still growing is
- 4:10with the special needs plans. So for people
- 4:13who need more chronic care, for people who
- 4:15are dually eligible for Medicaid and Medicare at
- 4:17the same time,
- 4:19those plans are also driving a ton of
- 4:21growth.
- 4:22And so
- 4:23yeah, I think it's just it's a reversal
- 4:25of of some of the the big publicly
- 4:27traded growth we saw in years past. I
- 4:29will say Asterisk with the exception of Humana,
- 4:33which added over a million members,
- 4:36just this past AEP alone, which is honestly
- 4:38really insane,
- 4:40given just the wider context of what's going
- 4:42on,
- 4:43with with the other guys. And then, you
- 4:45know, we we know from CMS,
- 4:48they are they're as of right now, they're
- 4:50proposing almost flat funding for MA plans going
- 4:54into next year, which, obviously, none of them
- 4:57are are happy about. And they're they're, you
- 4:59know, going pretty crazy with the lobbying right
- 5:01now to try to get that, reversed or
- 5:03increased.
- 5:04So we'll see what happens there, but the
- 5:06federal government is does not seem to be
- 5:08playing ball with giving them more money.
- 5:11And then
- 5:12one last thing, Scott, in terms of updates.
- 5:14We saw earlier this week that the federal
- 5:16government is threatening Elavance, formerly Anthem,
- 5:19with sanctions on its Medicare Advantage program,
- 5:23basically saying that they didn't they didn't follow
- 5:25the right procedures for risk adjustment
- 5:27and how they reported certain things to the
- 5:30federal government,
- 5:31even despite warnings according to to the government.
- 5:34And so
- 5:35if they don't fix basically what CMS has
- 5:37laid out as issues by the end of
- 5:39this month, the federal government is going to
- 5:42stop el Elevance entirely as a company, from
- 5:45enrolling any new, seniors into its Medicare Advantage
- 5:48Plans for the rest of the year, which
- 5:50is which is huge. That's I mean, that's
- 5:52a that is a very bold shot for
- 5:55the government to take at at one of
- 5:57the largest, health care companies overall in the
- 5:59country. And that, of course, comes after the
- 6:01Kaiser settlement
- 6:02from just a few of earlier this year,
- 6:04the largest MA,
- 6:06alleged fraud settlement ever in history. So just
- 6:09so much going on right now in this
- 6:10in this industry. It's hard to even,
- 6:12I at least for me, get get your
- 6:14thoughts around it.
- 6:16Yeah. No. It sure seems like it. And
- 6:18it seems like there's it was a growth.
- 6:19There's traditional growth in Medicare and Medicare Advantage.
- 6:23There's I mean, some of these rules on
- 6:26the big payers, the elevator story that you
- 6:28mentioned, can it really happen where they block
- 6:30them from selling to Medicare Advantage? Is that
- 6:32is that a real can that really happen?
- 6:34Oh, yeah. I mean, at the end of
- 6:36the day, the Medicare Advantage is just the
- 6:38government's,
- 6:40you know, essentially outsourcing Medicare contracts to private
- 6:43insurance companies. They are they're the boss at
- 6:46the end of the day. They decide whether
- 6:48these contracts are live or not, whether these
- 6:50plans can enroll people in them. And if
- 6:53CMS says you are acting fraudulently, you're not
- 6:56following the rules,
- 6:58whatever it is,
- 7:00you you cannot there's nothing you can do.
- 7:02You are not allowed to enroll,
- 7:04people in your health plans even if you
- 7:06are the biggest, one of the biggest guys,
- 7:07even if you have the strongest lobbyists and
- 7:09you throw around the most money,
- 7:12it it the buck stops with CMS. And
- 7:14I think that's why it's just gotten a
- 7:15lot of attention in the industry this past
- 7:17week, because
- 7:19I cannot think of a a plan in
- 7:22recent memory that this large,
- 7:24that has been blocked from potential,
- 7:27new enrollment for this year. So it's gonna
- 7:30be really interesting to see how that plays
- 7:32out. Again, this is just a threat of
- 7:34a sanction if they don't fix the issues
- 7:36within the next month.
- 7:38But, like, I mean, it's it's a huge
- 7:40message that CMS is sending alongside
- 7:43the DOJ settlement with Kaiser for over $500,000,000.
- 7:47Yeah. That's amazing. So that the $500,000,000
- 7:49settlement, what drove that? Talk about that for
- 7:51a sec if you don't mind. Yeah. It's
- 7:53it's the it's the issues we've all read
- 7:55about for over a decade now. It's what
- 7:57has had a lot of plans settling with
- 8:00the DOJ
- 8:01for for tens of millions in the past.
- 8:04And it's the the alleged
- 8:06fraudulent risk adjustment where basically
- 8:09the health plan is,
- 8:11adding on diagnoses to member health records that
- 8:14either don't exist or are exaggerated,
- 8:18or are added on from past medical records
- 8:21that didn't from care that didn't happen at
- 8:23that health plan, but it happened to that
- 8:25patient in the past.
- 8:26And now the that health plan wants to
- 8:29get it wants to bolt it onto their
- 8:31current
- 8:32medical record and then get increased reimbursement from
- 8:35the government. So,
- 8:37all alleged, but Kaiser and, you know, Kaiser
- 8:39has admitted no wrongdoing. The settlement is not
- 8:41in the middle of any wrongdoing,
- 8:44but they are gonna be paying over $500,000,000
- 8:47to the government,
- 8:48to settle allegations that they were basically pressuring
- 8:52their doctors to add on diagnoses that were
- 8:55not supported
- 8:56by medical records from from Kaiser specifically.
- 9:01No. Fantastic.
- 9:03Fantastic.
- 9:04In in other stores you're following currently, any
- 9:06anything else you're following closely currently, Jacob, that's
- 9:08top of mind? Well, Scott, you know, just
- 9:10because we we've gotten all this data back
- 9:12and we've we're we're seeing who grew the
- 9:14most over with Medicare over the last,
- 9:17since since since last October,
- 9:20the health systems, specifically, the ones that own
- 9:22Medicare Advantage Plans. I know we just talked
- 9:24about Kaiser, but the they're they're the big
- 9:26guy. But the a lot of these plans,
- 9:27especially regional ones, you know, they only have,
- 9:30let's say, 80 to a 100,000 members overall.
- 9:33It's a really small business when you think
- 9:35about it just as a health plan, but
- 9:37then also as part of wider health systems.
- 9:39They're not not not all of them. They're
- 9:42not necessarily ditching these plans, but
- 9:44we saw
- 9:45really amazing growth for some of them.
- 9:48Right here actually in the Midwest up in
- 9:50up in Wisconsin,
- 9:52Network Health, which is part of freighter ThetaCare,
- 9:56insane growth, I think, close to 60%,
- 9:59and that was at a growth, and that
- 10:00was after a record year last year. And
- 10:03I heard the same from CoreWell's health plan,
- 10:06Priority Health over in Michigan.
- 10:09I heard this from like I said, I
- 10:12heard it from SCAN, and then I heard
- 10:14it from Alignment Health. We heard it all
- 10:16over the country. And, basically, what and then
- 10:18we heard it from, Henry Ford Health System
- 10:21as well in in Detroit,
- 10:22from their health plan,
- 10:24all of them recording amazing growth. And when
- 10:27I asked, you know, what's what's behind this?
- 10:29And they just said stability.
- 10:31Even when the margins tightened for us, even
- 10:33when we did have losses, and I'm not
- 10:35saying any of them necessarily did, they stayed
- 10:38put. They didn't cut,
- 10:40broker compensation, which most big plans did to
- 10:43save money. They didn't necessarily pull back on
- 10:45a lot of benefits, or they didn't pull
- 10:47back on markets where the big insurers did.
- 10:50And because of that, seniors went to them.
- 10:52Even though they're smaller companies,
- 10:54even though they're not as well known,
- 10:57they all drove Blockbuster growth because they stayed.
- 11:00They didn't,
- 11:01they didn't exit markets when the margins got
- 11:03tough. And so now they're all doing, at
- 11:06least membership wise, they're all doing fantastic. We'll
- 11:08see what happens growing pains wise. And I
- 11:10one company, Alignment, told me, you know, that's
- 11:12great that they got all the, you know,
- 11:14tens of thousands of new members this year.
- 11:16Let's see what happens going into next year
- 11:18because there's gonna be a lot of digestion
- 11:20growth with that, because they're just not infrastructure
- 11:22wise set up for that much. Yeah. And
- 11:24not only not only the, digestion growth, they're
- 11:27also picking up a ton of business that
- 11:29some of the biggest payers have walked away
- 11:31from a little bit. Exactly. So so the
- 11:33great question is not only are they picking
- 11:35up senses and bodies,
- 11:37but will it be profitable, and can they
- 11:39manage it profitably, or is it gonna be
- 11:40an economic disaster? And we've seen that with
- 11:42our systems over the years, jump heavily into
- 11:45insurance and ended up, you know, almost like
- 11:47the dog who got the car.
- 11:49You know? And they they win the business,
- 11:51but then serving the business well is really
- 11:54hard. And and so we'll see how how
- 11:56that goes too. And and for us, it
- 11:58it can be difficult to track that kind
- 12:00of thing just because it is so fragmented
- 12:02fragmented. And at the end of the day,
- 12:04these are private companies that don't have to
- 12:06publicly report their earnings, and especially sometimes they
- 12:09don't even if they do, they don't segment
- 12:10it out into how did Medicare Advantage do
- 12:13specifically financially for them. So
- 12:16a lot of the times, you know, let's
- 12:17say, next year, two years from now,
- 12:20it it it is very difficult to go
- 12:21back and then see, well, how did these
- 12:23x amount of health systems do, when they
- 12:26gained all this growth compared to a United,
- 12:28for example, which
- 12:30generally and I and I put a huge
- 12:32asterisk, but generally lays it all out there
- 12:34on how their Medicare Advantage business is performing.
- 12:38So that's another, you know, piece of all
- 12:40this is we can see the enrollment data
- 12:42overall, but the finances
- 12:44gets a lot tougher to track when it
- 12:46comes to the health system specifically.
- 12:48No. I think that's exactly right. It it'll
- 12:51be fascinating to see how it plays out.
- 12:53It it seems like so much. We
- 12:55we've seen this story before,
- 12:57and some of these huge payers are
- 13:00they're massive, but they're also really smart, and
- 13:03they get out of markets that don't work
- 13:04for them. And then the health systems need
- 13:06to be able to win in those markets,
- 13:08both getting bodies enrolled,
- 13:10which is the easy part. The hard part
- 13:13is then managing that profitably,
- 13:15and maybe the health systems have a little
- 13:16bit of an advantage because they've got their
- 13:18own health systems to help service those patients.
- 13:21But, also, they don't have the technological
- 13:24and and the strengths that some of the
- 13:26massive payers have just in terms of just
- 13:28ultimate computing power and ability to sort of
- 13:31analyze everything
- 13:32in a way that that might leave them
- 13:34a little bit playing mom and pop versus
- 13:35the big payers. And let's hope they could
- 13:37do it profitably.
- 13:39Yeah. So, I mean, like I said, we
- 13:40saw seven states for the first time go
- 13:42down in terms of MA enrollment. They are
- 13:45very small states. Some of I mean, like,
- 13:47Wyoming and Vermont, for example. But overall, Scott,
- 13:50with with surveys of health plan leaders,
- 13:52all over the country that we've seen the
- 13:54results from now, nobody's predicting a negative five
- 13:56year outlook for Medicare Advantage.
- 13:59Medicare Advantage is not going anywhere. We you
- 14:01know, we've we I think we've talked about
- 14:03it too that 10,000 people or more agent
- 14:05to Medicare every day in this country, and
- 14:06that's not stopping.
- 14:08So this is gonna be big business to
- 14:10continue. It's just going through
- 14:13growing pains right now, and it's switching from
- 14:15the big publicly traded guys to now what
- 14:17we're seeing. The the small guys, the nonprofits,
- 14:19the very regional and population specific plans, they
- 14:22had their moment. They took it. They succeeded.
- 14:25We'll see what happens in the years to
- 14:26come, but the the landscape has completely shifted.
- 14:29And, yeah, the small guys,
- 14:32they they they have a chance to to
- 14:34run a lot of the game right now.
- 14:36Fascinating to watch, and let's see how it
- 14:38plays through. But, can I mean, it it's
- 14:40good news for these
- 14:42system plans and some of the smaller guys'
- 14:44plans, and I saw Oscar Health is up
- 14:4610% today? So different people filling these different
- 14:48niches are winning, and then we'll see if
- 14:50they could do it profitably. It's a fascinating,
- 14:52fascinating discussion, Jacob.
- 14:54Yeah. I I agree. We're gonna be tracking
- 14:56all of it. Yeah. No. Thank you for
- 14:58joining us, visiting with us today on on
- 15:00the Becker's Healthcare podcast. I was wondering something.
- 15:02Thank you so much for joining us. Thank
- 15:04you, Scott.
