- 2:56Speaker 1: article by doctor Jimmy Almandam. Day By cancer is one
- 3:00Speaker 1: of the most common cancers affecting the urinary system and
- 3:03Speaker 1: the tenth leading cause of cancer related death in the
- 3:06Speaker 1: United States. What makes it particularly notable is how treatable
- 3:09Speaker 1: it can be when caught early. Many people assume it
- 3:12Speaker 1: appears without warning, but bladder cancer often announces itself through
- 3:17Speaker 1: some very subtle signs. And that's the purpose of this
- 3:21Speaker 1: podcast today. So what happens in your body? Cancer causing
- 3:26Speaker 1: chemicals from smoking industry, in the environment and of the bloodstream.
- 3:32Speaker 1: They're filtered by the kidney and collecting the urine where
- 3:34Speaker 1: they become in contact with the bladder lining. The most
- 3:41Speaker 1: common and often the first symptom of bladder cancer is
- 3:43Speaker 1: blood in the urine a materia. Urine color can range
- 3:47Speaker 1: from a faint pink to smoky, rusty, bright red or
- 3:50Speaker 1: dark red. Blood may appear only once or twice, making
- 3:53Speaker 1: it easy to dismiss or attribute it to other common
- 3:56Speaker 1: conditions such as a urinary track, infection, or postmotopausal women.
- 4:01Speaker 1: This confusion often delays diagnosis, allowing the cancer to advance
- 4:05Speaker 1: to a stage where it's a more difficult to treat.
- 4:09Speaker 1: Other common symptoms include pain, burning, and irritation during urination,
- 4:14Speaker 1: sudden urgency to urinate even when the bladder is not full,
- 4:17Speaker 1: more frequent urination than usual, waking at night with a
- 4:20Speaker 1: strong need to urinate. When bladder cancer becomes large or
- 4:24Speaker 1: sploods beyond the bladder may cause more widespread symptoms, including
- 4:29Speaker 1: flank pain, pain in the side of the back below
- 4:32Speaker 1: the ribs above the waist, abdominal pain, bone pain or tenderness,
- 4:36Speaker 1: loss of appetite, unexplained weight loss, swelling in the feet,
- 4:40Speaker 1: persistent fatigue, and ability to urinate. The bladder is a
- 4:45Speaker 1: balloon shaped organ in the lower abdomen of the stories
- 4:48Speaker 1: you're in until it's ready to be released. It sits
- 4:52Speaker 1: within the urinary system, which also includes the killed kidneys
- 4:55Speaker 1: which filter toxins, the uriners which carry you into the bladder,
- 5:00Speaker 1: and the urethra, where you're an extra to the body.
- 5:03Speaker 1: The urethra is shorter in women and longer in men.
- 5:07Speaker 1: Bladder cancer develops when cells in and around the bladder
- 5:09Speaker 1: begin to grow up. Normally, about ninety percent of cases
- 5:12Speaker 1: are euytheroral carcinoma, which begins in the cells inside the
- 5:17Speaker 1: lining of the bladder. In most cases, the tumor remains
- 5:21Speaker 1: confined to the innerlining because ur ethereal cells also line
- 5:25Speaker 1: other parts of the urinary tract, including the renal, pelvis, urine,
- 5:29Speaker 1: or urethra. People with bladder cancer may also develop tumors
- 5:32Speaker 1: in these areas as well. For this reason, doctors typically
- 5:36Speaker 1: examine the entire urinary track when evaluating and monitoring the disease.
- 5:41Speaker 1: Other types of bladder cancer include squamous cell carcinoma, which
- 5:45Speaker 1: involves flat skin like cells, and adenocarcinoma, which begins in
- 5:49Speaker 1: a gland forming cells. The exact cause of bladder cancer
- 5:54Speaker 1: is not fully understood. There's some genetic issues. Bladder cancer
- 5:57Speaker 1: develops from certain genetic mucations accumulating the genes, allowing the
- 6:01Speaker 1: cells to grow uncontrollably and deformed tumors. A small number
- 6:07Speaker 1: of inherited gene variances can reduce the body's ability to
- 6:10Speaker 1: break down harmful chemicals. Increasing sensitivity to carcinogens such as tobacco, smoke,
- 6:15Speaker 1: and industrial compounds. Inherited symptoms such as lynch syndrome and
- 6:20Speaker 1: caldin syndrome may elevate the risk. Smoking is a big deal,
- 6:25Speaker 1: the biggest smoking biggest single risk factor for bladder cancer.
- 6:30Speaker 1: Chemical exposure, especially aromatic amines such as benzidine and bated naphthaline,
- 6:37Speaker 1: arsenic exposure, medications, diabetes, drugs have been linked to bladder
- 6:42Speaker 1: cancer risk, chronic bladder irritation and infections, radiation exposure, high
- 6:48Speaker 1: intake of red and processed meats, low water intake, decaffeinated coffee.
- 6:54Speaker 1: A twenty twenty five study involving more than one hundred
- 6:57Speaker 1: and twenty thousand participants found that men who drink decafat
- 7:00Speaker 1: to coffee had a significantly higher risk of bladder cancer
- 7:03Speaker 1: than those who did not. Opium use, past cancers, previous chemotherapy,
- 7:11Speaker 1: certain birth effects, organ transplantation, certain demographics too. Age risks
- 7:17Speaker 1: increase with age. Most cases occur in people older than
- 7:20Speaker 1: fifty five. Men are three times more likely than women
- 7:24Speaker 1: to develop bladder cancer. Bladder cancer occurs about twice as
- 7:28Speaker 1: often as white people as it does African American or Hispanics.
- 7:32Speaker 1: Death rates for bladder cancer have declined about one point
- 7:34Speaker 1: four percent annually since twenty sixteen. Some doctors may recommend
- 7:39Speaker 1: screening for people at high risk of bladder cancer using
- 7:42Speaker 1: your analysis. Urine cytology and occasional sistoscopy. Doctors may use
- 7:48Speaker 1: imaging and procedural tests to examine the urinary track. Sistoscopy
- 7:53Speaker 1: uses a thin lighted tube which is used to examine
- 7:56Speaker 1: the bladder and urethra for tumors. Urytoscopy similar to sistoscopy,
- 8:01Speaker 1: but examines the upper urinary tract, including the urinars and
- 8:04Speaker 1: renal pelvis biopsies, Intravenous pylogram uses dye and X raysed image,
- 8:11Speaker 1: urinary tract CT scan, MRI, ultrasound, blood tests, Kiddy function
- 8:19Speaker 1: tests are used, as are liver functioning test, especially alkaline phosphatase.
- 8:26Speaker 1: Other tests may help to evaluate urinary symptoms. To detect
- 8:29Speaker 1: cancer related changes in the urine, you can do a
- 8:31Speaker 1: euroflow test measures so while a person urinates by recording
- 8:35Speaker 1: urine flow in the time it takes empty the bladder.
- 8:37Speaker 1: Urine based tests looking for blood bacteria and adnormal cells.
- 8:43Speaker 1: Genetic testing I recommended just recommended for select patients, particularly
- 8:48Speaker 1: those diagnosed at a younger age. So what are the
- 8:51Speaker 1: gratings and stages of bladder cancer? Once bladder cancer is diagnosed,
- 8:54Speaker 1: it's graded and staged up guide treatment grading describes how
- 8:58Speaker 1: normal cancer cells look compared to healthy cells. In stage zero,
- 9:03Speaker 1: bladder cancer and the cancer is limited to the bladder lining.
- 9:06Speaker 1: Stage two marks a turning point, with the cancer spreading
- 9:09Speaker 1: into the bladder and muscle. In stage three, it extends
- 9:12Speaker 1: beyond the bladder to nearby fat or organs. Stage four
- 9:15Speaker 1: of the cancer spread to distant organs such as lungs, bones,
- 9:18Speaker 1: or liver. The bladder cancer progresses from stage two to
- 9:21Speaker 1: stage four, the five year survival rate drops sharply from
- 9:25Speaker 1: seventy three percent to nine So some of the early
- 9:30Speaker 1: stage treatments. Stage is zero and one surgery or intra
- 9:34Speaker 1: vesicle immunotherapy or chemotherapy. Number two muscle invasive bladder cancer treatment.
- 9:41Speaker 1: Stage two and three surgery bladder reconstruction or stomas, chemotherapy, radiation,
- 9:48Speaker 1: bladder preserving approach targeted therapy and immunotherapy. Then we moved
- 9:54Speaker 1: to metastatic bladder cancer Stage four drug combinations chemotherapy, immunotherapy alone,
- 10:00Speaker 1: radiation combined with chemo surgery, palliative care support the quality
- 10:05Speaker 1: of life Number four recurrent bladder cancer treatments. Treatment depends
- 10:10Speaker 1: on what therapies are used before the cancer returned. Options
- 10:14Speaker 1: include systemic chemo and innotherapy drugs or targeted therapies, acupuncture
- 10:18Speaker 1: and acupressure, cognitive therapy, and then there's emerging in experimental
- 10:23Speaker 1: treatments like gene therapy, antibody drug conjugates ADC's. These targeted
- 10:30Speaker 1: therapies combined to cancer seeking antibody with a potent chemotherapy drug,
- 10:35Speaker 1: natural and lifestyle approaches. Diet well. There isn't a specific
- 10:39Speaker 1: diet for bladder cancer. Focusing on the balance die it
- 10:41Speaker 1: can support overall health during treatment and recovery, meditation and mindfulness,
- 10:46Speaker 1: exercise and music. Dietary recommendations Eat more veggies, include more
- 10:53Speaker 1: whole grains, eat fish and cruciferous vegetables. Eat foods rich
- 10:59Speaker 1: inviteament ead, limit process meats, eat a moderate amount of
- 11:03Speaker 1: red meat, and stay well. Hydrated. Other tips. Quit smoking,
- 11:07Speaker 1: maintain a healthy weight, wear protective clothing and masks, avoid
- 11:10Speaker 1: medicines and herbal remedies containing aristolic acid. Avoid opium use obviously,
- 11:17Speaker 1: treat UTIs properly and seek medical care. Limit unnecessary radiation exposure,
- 11:23Speaker 1: and again, reduce your reoccurrence rates. Some possible combinations of
- 11:27Speaker 1: bladder cancer, anemia, swelling of the urters, nearating of the urethra,
- 11:32Speaker 1: loss of bladder control, difficulty with erections in men, sexual
- 11:35Speaker 1: dysfunction in women. Hopefully you got a tip or two
- 11:41Speaker 1: there which could become very important to make sure that
- 11:44Speaker 1: you don't become a statistic all right, need help with
- 11:48Speaker 1: that by all means, give a call seven seven nine,
- 11:53Speaker 1: God bless you
