- 0:00Good morning, and welcome to Morning Medical with Doctor Wai Chow.
- 0:03I am Doctor Chimming Chiao.
- 0:05And today, we have a series of important updates in the world of health care that you
- 0:09will not want to miss.
- 0:11From rising employer coverage for weight loss drugs to critical discussions on women's health
- 0:15care and cyber security threats targeting the medical field, we have a lot to covered.
- 0:21So let's dive right in.
- 0:25In today's first segment, We will be discussing the significant increase in employer coverage
- 0:30for weight loss drugs as reported by a survey from the International Foundation of Employee
- 0:35Benefit Plans.
- 0:38The survey released on June 13th, 2024, reveals that about 1 third of United States employer
- 0:44health plans are now offering coverage for GLP 1 drugs, which are used for both diabetes
- 0:49management and weight loss.
- 0:52This is a notable rise from last year.
- 0:54Where only about 26% of employers offered these drugs.
- 0:59GLP 1 drugs, such as Novo Nordisk Wiegavi and Eli Lilly's Zep bound have gained popularity
- 1:07due to their effectiveness in promoting weight loss.
- 1:10These drugs work by reducing appetite and causing the stomach to empty more slowly.
- 1:17Initially approved for diabetes treatment, they have been shown to reduce weight by around 20%,
- 1:24making them highly sought after.
- 1:27According to the survey, the portion of employers overall medical claims, spending on
- 1:32GLP 1 drugs for weight loss, has grown to 8.9% in 2024 from 6.9% in 2023.
- 1:41Furthermore, 57% of employers surveyed currently covered these drugs solely for
- 1:47diabetic care with 19% of those employers considering expanding coverage to include
- 1:52weight loss.
- 1:54The international foundation of employee benefit plans represents over 33,000 member
- 1:59companies or public institutions covering more than 25,000,000 employees in the United States
- 2:05and Canada.
- 2:07The survey highlights that the association of obesity with chronic and higher cost
- 2:12conditions, along with consultant recommendations, were the primary factors
- 2:16influencing employers to consider covering GLP 1 drugs.
- 2:22Analyst forecasts suggest that by the early 2030s, global sales for GLP 1 drugs could reach
- 2:28an annual 150,000,000,000 This is a significant increase from a previous forecast in 2023,
- 2:34which had predicted a 100,000,000,000 in sales by the early next decade.
- 2:38However, ensure a resistance to covering these drugs remains a limiting factor for sales
- 2:43growth, as noted in a December report by Morgan Stanley.
- 2:47Currently, out of the 110,000,000 Americans with obesity, 40,000,000 have access to GLP 1
- 2:54drugs for weight loss through their health care plans.
- 2:57This expanding coverage is a critical development in the fight against obesity, a
- 3:02condition associated with numerous health risks and complications.
- 3:06In our next segment, we turn our focus to women's health care, which has been a topic of
- 3:11significant concern in the United States.
- 3:14According to an article by Caitlin Walsh Shelton in the Washington Examiner, the
- 3:18standard for women's health care in the United States is supremely low.
- 3:21The country consistently ranks among the lowest of developed nations for a number of factors,
- 3:26including maternal mortality and overall women's health care.
- 3:30This week, the Senate is considering the so called right to Invitro Fertilization Act.
- 3:37However, this bill is not expected to meaningfully improve the quality of women's
- 3:41health care.
- 3:42Instead, an alternative bill known as the ReStore Act.
- 3:47Proposed by Senator's Cindy Hyde Smith and James Langford could have a more substantial
- 3:52impact.
- 3:54One major issue highlighted in the article is the over prescription of hormonal birth control
- 3:59for girls as young as twelve.
- 4:02These prescriptions are given for a range of issues from acne to severe menstrual cramps.
- 4:07Despite the widespread use, Little research has been conducted on the long term health effects
- 4:13of hormonal contraceptives for young girls and the existing research does not present a
- 4:18favorable outlook.
- 4:20Currently, more than 1 in 3 teenage girls and 65% of women under 50 report using some form
- 4:27contraception.
- 4:29Furthermore, 1 in 6 women experience infertility, which underscores a significant
- 4:35breakdown in women's health care across the board.
- 4:38This spans from research to treatment and affects women from anarchy to menopause.
- 4:44The Restore Act aims to address these issues by promoting restorative women's health care,
- 4:49which currently receives little to no funding, research, or support.
- 4:54Restorative health care methods not only increase the chances of natural conception and
- 4:59carrying to term.
- 5:00But are also significantly cheaper and less invasive than in vitro fertilization.
- 5:06The restore act includes provisions for Title X funding for restorative reproductive medicine
- 5:12and the expansion of research and literature reviews on reproductive health conditions and
- 5:18fertility awareness based methods.
- 5:21Notably, the restore act does not exclude further protections for in vitro fertilization,
- 5:27but offers a pathway for reducing infertility.
- 5:30And consequently, the need for in vitro fertilization in the future.
- 5:35This bill also comes at a time when a growing number of women are choosing not to take
- 5:40hormonal birth control for various reasons.
- 5:42Including concerns over long term effects, personal discomfort, and a desire for more
- 5:47natural treatment options.
- 5:50Responding to this trend, several new companies are investing in cycle tracking and natural
- 5:55methods of fertility control.
- 5:57Companies like natural cycles, which was the first phone application to receive FDA approval
- 6:03as a contraceptive method, Fem, Ava, and Mira are leading the way.
- 6:09These methods focus on the female body's natural hormonal rhythms.
- 6:14Using specific biomarkers to track the cycle and identify fertile windows.
- 6:21These fertility awareness based methods can predict with near perfect accuracy, a woman's
- 6:26fertile window, and identify inconsistencies in her cycle.
- 6:31This data can help diagnose underlying conditions such as endometriosis, polycystic
- 6:37ovarian syndrome, autoimmune disorders, and even certain types of cancer.
- 6:42According to reproductive health practitioner, Lisa Hendricks and Jack, The reproductive
- 6:47system should be monitored closely as it is a woman's 5th vital sign, crucial for her overall
- 6:54health.
- 6:55The current standard of care in the United States often involves prescribing birth control
- 7:00at a young age and referring women to in vitro fertilization later in life.
- 7:05This approach does not address the root causes of reproductive health issues.
- 7:11The Restore Act seeks to change this by enhancing education about the female cycle and
- 7:17supporting methods that track and diagnose underlying conditions.
- 7:22These methods are currently unsupported by federal programs and unrecognized by insurance
- 7:27agencies making them accessible only to those who can afford them.
- 7:31In summary, the Restore Act aims to provide better reproductive health care by focusing on
- 7:37natural and restorative methods.
- 7:40It seeks to improve research, funding, and support for these methods, offering women more
- 7:45options and potentially reducing the need for in vitro fertilization in the future.
- 7:50As the senate debates this bill, It is crucial for policy makers to recognize the importance
- 7:56of comprehensive and supportive women's health care.
- 8:00In our next segment, we turn our focus to a critical issue that has been making headlines
- 8:05cybersecurity threats targeting the health care industry.
- 8:10According to a recent report from the A chip meeting in Las Vegas, hackers are increasingly
- 8:15targeting health care organizations making them the 2nd top target for cyber attacks.
- 8:22Ari Schwartz, Managing Director of cybersecurity services at the law firm of
- 8:26Venable highlighted the vulnerabilities during a session on cybersecurity.
- 8:311 of the most significant incidents this year involved change health care a UnitedHealth
- 8:37company that provides claims processing.
- 8:39On February 21 2024, Change Healthcare was hacked by a group representing itself as ALPHV
- 8:47black cat.
- 8:48This cyber attack caused massive disruptions to pharmacy and medical claim services including
- 8:54electronic prescribing, claim submission, and payment transmission.
- 8:59The federal cyber security and infrastructure security agency or Saisa issued an updated
- 9:05warning about ALPHV black cat shortly after the attack.
- 9:10In the aftermath, change health care paid hackers 22,000,000 1st month, following the
- 9:21ransom payment, there were 44 additional health care related cyber attacks, according to a
- 9:26report by wired.
- 9:28This underscores the increasing frequency and severity of cyber threats facing the health
- 9:32care sector.
- 9:34Clark Harshbarger, director of incident response at crowdstrike, emphasize the value of
- 9:39personal and health information, which makes health care organizations attractive targets.
- 9:45He noted that threat actors are not just shutting down systems, but are learning the
- 9:49intricacies of electronic medical record, EMR systems, and enterprise resource planning ERP
- 9:55systems.
- 9:57This detailed knowledge allows them to navigate within an organization's infrastructure and
- 10:02attack the highest value information points.
- 10:05As health care moves towards cloud and software as a service, SAS based operations, The
- 10:11familiarity of hackers with these infrastructures is also increasing.
- 10:15Health care is particularly vulnerable because it operates as an ecosystem of partner
- 10:20delivering care and sharing information.
- 10:22Application programming interfaces, APIs, which are used for data transfer, are often the
- 10:28weakest link in this ecosystem.
- 10:30Even if the system's holding data are well protected, APIs can be points of vulnerability.
- 10:37Harshbarger pointed out that API systems were initially designed to focus on availability,
- 10:42confidentiality, and data integrity.
- 10:45However, the rapid evolution of cyber threats means that these systems are now prime targets.
- 10:51Ari Schwartz added that while compliance has historically been the primary focus for health
- 10:57care organizations.
- 10:58It is no longer the biggest liability.
- 11:01Compliance remains important but health care organizations must also prioritize preparedness
- 11:07for cyber attacks and have plans in place to mitigate their impact.
- 11:11Steve Roberts, CEO of Eclipse Cybersecurity, emphasized the importance of elevating chief
- 11:18information security officers, CISOs to executive and board levels within
- 11:23organizations.
- 11:25This elevation allows CISOs to provide critical risk reward assessments and help companies
- 11:32recognize that it is not a matter of if, but when they will experience a data breach.
- 11:38Having a seat at the executive table is imperative for effective cybersecurity
- 11:42strategy.
- 11:45Panalysts at the AIP meeting agreed that stopping all cyber attacks is nearly
- 11:49impossible.
- 11:50Therefore, it is crucial for healthcare organizations to have mechanisms in place to
- 11:54prepare for potential attacks.
- 11:56This includes addressing anomalies promptly and having a clear plan for responding to breaches
- 12:02when they occur.
- 12:03Compliance should not be a distraction, but rather a component of a comprehensive risk
- 12:09management program that includes multiple layers of security.
- 12:14In summary, the health care sector is increasingly becoming a prime target for cyber
- 12:19attacks.
- 12:20Organizations must prioritize not only compliance, but also preparedness and response
- 12:26strategies to mitigate the impact of these inevitable breaches.
- 12:31Elevating CISOs to higher decision making levels and focusing on the vulnerabilities in
- 12:37data transfer mechanisms like APIs are critical steps in enhancing cybersecurity in health
- 12:44care.
- 12:45In our final segment today, we delve into a landmark decision by the United States Supreme
- 12:50Court regarding access to the abortion pill, Mifep Perstone.
- 12:55On Thursday morning, the Supreme Court unanimously ruled to uphold the Food And Drug
- 12:59Administration's approval of Mifepristone.
- 13:02A drug that has been at the center of legal battles for nearly 25 years.
- 13:06The case was brought forward by the alliance for Hippocratic medicine.
- 13:10An anti abortion physician group, which challenged the lawfulness of my favorite stones
- 13:15initial approval in 2000.
- 13:17The generic versions approval in 2019 and subsequent modifications in 2016 2021 that made
- 13:24the drug more accessible.
- 13:26The modifications included allowing non physicians such as nurse practitioners to
- 13:31prescribe my pepper stone and permitting its prescription without an in person visit during
- 13:36the COVID 19 pandemic.
- 13:39Justice Brett Kavanaugh delivered the ruling, stating that the plaintiffs did not have legal
- 13:44standing to challenge the food and drug administration's regulation of the drug.
- 13:49Kavanaugh wrote, here, the plaintiffs have failed to demonstrate that the food and drug
- 13:54administration's relaxed regulatory requirements likely would cause them to suffer
- 13:59an injury, in fact.
- 14:00For that reason, the federal courts are the wrong forum for addressing the plaintiff's
- 14:05concerns about the food and drug administration's actions.
- 14:09The American Medical Association, which represents over 271,000 physicians, praised the
- 14:15Supreme Court's decision.
- 14:17Doctor Bobby Mukamala, the association's president-elect, pointed out that restricting
- 14:23access to abortion care without justification leads to negative psychological, physical, and
- 14:29economic consequences.
- 14:31He emphasized that efforts to second guess the food and drug administration's scientific
- 14:35judgment were based on speculative allegations and ideological assertions undermining decades
- 14:41of rigorous scientific review proving the drug's safety and efficacy.
- 14:47Doctor Steve Furr, president of the American Academy of Family Physicians, also lauded the
- 14:52ruling.
- 14:53He stated that maintaining access to Miffy Perstone preserves the patient physician
- 14:57relationship, allowing physicians to make critical decisions based on their medical
- 15:01education, training, experience, and available evidence.
- 15:06Doctor Fur warned that restricting access to certain treatments would set a dangerous
- 15:12precedent, undermining the rigorous drug evaluation process.
- 15:17Monica CPAK, CEO of WIS a provider of sexual and reproductive health care expressed relief
- 15:23at the Supreme Court's decision.
- 15:25She noted that the threat to abortion access and the potential ban of me, Fepristone had
- 15:30been extremely daunting, especially amid increased demand for medical abortion following
- 15:36the overturn of Roe V's Wade.
- 15:39Chase expressed that the court's decision to uphold broad access to the abortion pill is a
- 15:43significant relief for those in the reproductive health space.
- 15:48In conclusion, the Supreme Court's ruling to uphold the food and drug administration's
- 15:52approval of MI, Pepperstone, is a critical victory for reproductive health advocates.
- 15:58It not only preserves access to a vital medication, but also reinforces the importance
- 16:03of scientific integrity and the patient physician relationship in healthcare decision
- 16:08making.
- 16:09That wraps up today's episode of Morning Medical with Doctor Chow.
