- 0:29This is Haley Recker with the Becker's Payer
- 0:31Podcast. And today, we're recording live at the
- 0:33Becker's third annual spring payer issues roundtable.
- 0:36I am thrilled today to be joined by
- 0:38Brad Riley, vice president of analytics and reporting
- 0:40at Longevity Health. Brad, thank you so much
- 0:43for joining me today. Hey. Happy to be
- 0:45here. So before we get started, can I
- 0:47just have you go ahead and introduce yourself
- 0:48and a little bit about your background? Sure.
- 0:50Absolutely. So, appreciate the opportunity to be here.
- 0:53I think this is the second time doing
- 0:54it. So,
- 0:56happy happy to be here and to take
- 0:58part. So, like Haley said, I'm Brad Riley.
- 1:01I'm the VP of analytics and reporting at
- 1:03Longevity Health Plan. Longevity is the fastest growing
- 1:07national I SNP plan, so we're a clinical
- 1:09services organization and an I SNP plan.
- 1:12We just crossed the 10,000 member threshold this
- 1:15year, so we're really excited about that.
- 1:17And we currently partner with,
- 1:19over 300 SNPs
- 1:21in roughly 13 different markets, and we're hoping
- 1:24to expand that a little this year. So
- 1:26excited to be here. Excited to talk about
- 1:29tech, AI,
- 1:30and and ways that, you know, I'm from
- 1:32my perspective, I'm getting to see that impact
- 1:35the industry and and longevity health. Well, that's
- 1:37awesome. Thank you so much for sharing.
- 1:40Now to begin our conversation today, I wanna
- 1:42talk about balancing affordability and quality. So how
- 1:45is your organization innovating to manage cost of
- 1:48care while maintaining or improving member outcomes?
- 1:51That's a good question. I mean, especially with
- 1:53the rates the way the way they are
- 1:54now and everything else. You know, '25 is
- 1:56a tough year. '26 is ramping up to
- 1:58be look a little better. But, with the
- 2:00Medicaid stuff coming in, you know, nobody's ever
- 2:02quite sure how that's gonna look. It's it's
- 2:04you know, changes
- 2:06month by month. So I think a couple
- 2:08things. One,
- 2:10obviously, at the forefront of everything else right
- 2:12now is is both quality based care agreements
- 2:15and quality based payment agreements.
- 2:17I I think that's
- 2:19absolutely where the industry is going. That's how
- 2:21longevity is structured,
- 2:23and how we, you know,
- 2:25partner with with our partners. I think the
- 2:28other piece, obviously, is leveraging technology, specifically,
- 2:32the buzzwords AI, especially, you know, this past
- 2:35at at Becker's, but, you know, everywhere else
- 2:37too. And I think understanding how to
- 2:40how
- 2:41to do that is important. You know? With
- 2:43with Longevity Health, we treat our members
- 2:46in the facilities, and so we we only
- 2:48partner well, our members are only in skilled
- 2:50nursing facilities. And so we have nurse practitioners
- 2:53and registered nurses and LPNs,
- 2:56that dependent upon the market will go in
- 2:58and and they they have a specific facility
- 3:01with a specific panel. And, you know, we
- 3:03found that,
- 3:04that really impacts avoidable admissions, readmissions.
- 3:08It lifts the residents' experience so they're not
- 3:10constantly going back and forth. And so from
- 3:12a model perspective, that absolutely avoids
- 3:15inpatient costs.
- 3:17But at the same time,
- 3:19from an administrative perspective, corporate perspective, understanding how
- 3:22we could
- 3:23leverage different technology tools like AI,
- 3:27to do things. I think it's definitely a
- 3:29little early still on the clinical end of
- 3:32using AI. There's a lot of proof of
- 3:34concepts out there that are really cool,
- 3:37but having
- 3:39the ability to kind of leverage different tools,
- 3:42whether it's,
- 3:44machine learning tools to better stratify your population
- 3:47or to augment or identify
- 3:50fraud waste and abuse, anything like that that
- 3:53we could use to kind of optimize
- 3:55the existing workflows that aren't,
- 3:58particularly clinical in nature, I think, is is
- 4:00a low hanging fruit. And then I think
- 4:02also just administrative
- 4:03tasking,
- 4:05equipping
- 4:06employees with the proper tools that are available
- 4:08in today's market,
- 4:10to help them be more efficient.
- 4:11And and, frankly, you know, everyone's tired of
- 4:14hearing this, but do more with less. I
- 4:15I think,
- 4:17give them a really cool tool. Let them
- 4:19let them use these tools to help them
- 4:20optimize their work. You still have a person
- 4:23involved there, so we're not talking pure Agentics
- 4:25at this point. Right? But,
- 4:27you have human level oversight
- 4:29with a tool that can help them,
- 4:31improve their day to day tasking and keep
- 4:33them more efficient.
- 4:34Absolutely.
- 4:35Well, I'd like to keep that conversation going
- 4:37about that quality of care. Mhmm. So what
- 4:40best practices or tools does your organization rely
- 4:43on to keep quality of care at the
- 4:44forefront?
- 4:46So, I mean, a couple things. I think
- 4:48one,
- 4:49quality of care is
- 4:52there's different couple different angles. Right? You've got,
- 4:54like, the HEDIS angle, but you also have
- 4:55the the actual quality of the care. I
- 4:57already touched on, the quality based agreements. I
- 4:59think that's vital. Right? Moving away from fee
- 5:02for service and and incentivizing people purely based
- 5:04off of the quality of of care that
- 5:06they're delivering to patients, I think, is is
- 5:08key. That's with PCP groups. That's with with
- 5:10skilled nursing facility. You know, every everything across
- 5:12the board. I think,
- 5:14at the same time,
- 5:17understanding
- 5:18broader
- 5:20provider behavior
- 5:21is important. Right? So when we look at
- 5:24and we leverage
- 5:26machine learning or predictive analytics to,
- 5:29create these
- 5:33more network driven
- 5:35oversight tools that allow you to see, you
- 5:37know, of your inpatient stays,
- 5:40which inpatient stay is really you know, what's
- 5:42the readmission rate based off of different inpatient
- 5:44providers?
- 5:45What what type of costs are we seeing
- 5:47coming from them? Is the is it qualitative
- 5:49in nature? And I and I think there's
- 5:51a lot of opportunity there in,
- 5:54optimizing network, understanding and this is across the
- 5:56industry, right, not just at longevity health, but
- 5:58understanding
- 6:00what providers are delivering quality care versus just
- 6:03delivering care. And I think when you optimize
- 6:06your network to focus specifically on the quality
- 6:09care,
- 6:10I think that's a big big next step
- 6:11because you can only control what you can
- 6:13control.
- 6:14And if you're not
- 6:16being proactive in that space and identifying your
- 6:18network
- 6:19and,
- 6:20trying to encourage
- 6:22proactivity
- 6:24amongst
- 6:25the providers that are that are delivering that
- 6:27that high quality.
- 6:29Whatever KPI is gonna reflect that, I think
- 6:31that's probably that's key.
- 6:34Absolutely.
- 6:35We've talked about the imperative to reduce costs,
- 6:37improve quality, and advance health equity, none of
- 6:40which are small tasks.
- 6:42How do you approach aligning these priorities in
- 6:44your organization's strategic vision?
- 6:47So I think the theme of the day
- 6:49is gonna be, you know, technology and tools.
- 6:51And I think the biggest thing,
- 6:53one, is allowing
- 6:56the use of technology. I think over the
- 6:59last three years, right, generative AI has blown
- 7:01up, but
- 7:02artificial intelligence is the big buzzword, but it's
- 7:04really existed since the nineteen fifties.
- 7:07So you have machine learning, predictive analytics, different
- 7:09types of things like that. And I think
- 7:11making sure that we are
- 7:14implementing
- 7:15efficiencies
- 7:16and, by leveraging technology, but at the same
- 7:19time,
- 7:20not reducing
- 7:22the quality and governance of the oversight we
- 7:24have to make sure that we're remaining compliant
- 7:26and to make sure that we're doing it
- 7:27the right way within, you know, CMS standards
- 7:30and whatever other types of standard standards that
- 7:32we operate within. I think that's key. Right?
- 7:35So,
- 7:37driving forward
- 7:39with technology while ensuring that there's governance and
- 7:42oversight,
- 7:43and I think that
- 7:44that also includes ethical oversight as well. I
- 7:47think some of these really cool models that
- 7:49are coming out with
- 7:51are being shown and found to have,
- 7:54some maybe nonethical
- 7:56steps and and and bias that inherently,
- 7:59was inadvertently
- 8:01added to the model because of of human
- 8:04data that that was ingested and and used
- 8:06to train it. Right? And I think
- 8:08so taking those resources that we're getting from
- 8:10creating efficiencies and implementing these type of tools
- 8:13and taking a portion of those to be
- 8:15more efficient and and create cost savings, but
- 8:17also leveraging some of those savings, not just
- 8:19pocketing it all and investing a little bit
- 8:20more in the governance and the oversight into
- 8:22the types of tools that you're using is
- 8:24probably a a more
- 8:26robust and long term strategy
- 8:28to make sure that
- 8:30that through leveraging these new tools, we don't
- 8:32introduce new risk.
- 8:34Well, before we wrap up here, is there
- 8:35anything else that you'd like to share with
- 8:37our listeners today that we haven't already discussed?
- 8:40Yeah. I think my big thing and what
- 8:41I would encourage everybody listening and,
- 8:44especially leaders of teams and executives is
- 8:48if your staff don't become literate
- 8:51with the new tools that are available to
- 8:53them,
- 8:54they're going to get behind and you'll get
- 8:56behind. And,
- 8:57I think that that means
- 8:59it's important for us to find ways,
- 9:01risk free, while keeping risk and and governance
- 9:04in mind
- 9:05of getting our staff,
- 9:08familiar
- 9:09and used to and exposed to tools, not
- 9:12just in their personal lives. Right? Like, I
- 9:13think everyone's like, oh, I've got Chad GPT
- 9:15on my phone or I've got this. And,
- 9:17you know, when I when I press Google,
- 9:18Gemini pops up at the top. But at
- 9:20the end of the day, if if people
- 9:22aren't thinking of how to use these tools
- 9:23in a professional manner,
- 9:25they're gonna miss out. Right? We've got a
- 9:26lot of really smart people, and something I
- 9:28believe,
- 9:29in is, you know, I wanna hire people
- 9:31smarter than me. Right? That's that's how you
- 9:32do do well.
- 9:34And and so
- 9:35empowering that staff
- 9:37to do that, is something that I think
- 9:39is is an opportunity. There's a lot of
- 9:41people that are engaged and excited to do
- 9:43it themselves, but at the same time, there's
- 9:45people that might need a little bit of
- 9:46a push. And so one of the things
- 9:48I actually did for this, I realized on
- 9:51on my team is that I had a
- 9:52lot of really good technical skills, but my
- 9:55staff
- 9:56had an opportunity to learn a little bit
- 9:57more. I like to say how the machine
- 9:58works, but, like, the managed care like, what
- 10:01is managed care? What's a claim? How does
- 10:02a claim process? How does an authorization process?
- 10:05And so I actually created a a customized
- 10:08large language model. And it's just trained. It's
- 10:11a chatbot. Right? But it's trained on stuff.
- 10:12You can actually it's it's open source, so
- 10:14anybody can can go and look at it
- 10:16and use it. You can go to
- 10:18app.managedcare101.ai.
- 10:22But ask it anything about managed care. Right?
- 10:24Get people used to navigating
- 10:26large language models, NLP, and encourage your staff
- 10:28and empower your staff to operate at the
- 10:30top of of what they're capable of doing.
- 10:33And I think that's gonna take us a
- 10:34long way over the next few years as
- 10:36continue to see technology,
- 10:38you know, go down the path it's going.
- 10:40Absolutely. Well, thank you so much for joining
- 10:42me today. Again, this is Haley Rutger with
- 10:44the Becker's Payer podcast
- 10:46recording live at the Becker's third annual spring
- 10:49Payer issues roundtable. Brad, thank you so much.
- 10:51Thank you.
