- 0:00With Parkinson's, this happens all the time.
- 0:02You try to explain to your doctor, I have no motivation,
- 0:06I'm not starting anything. Everything just feels flat.
- 0:10And before you can even finish the sentence, they'll say
- 0:13something like, Yep, that sounds like depression.
- 0:17But what if they're wrong? Because with Parkinson's,
- 0:20there's something else, something totally different that
- 0:24looks like depression. It acts like depression, but it
- 0:28isn't depression at all. It's apathy.
- 0:31And when doctors mix the two up, you end up treating the wrong
- 0:34problem entirely. And that's exactly what we're
- 0:37going to break down today. I'm going to show you how to
- 0:39tell the difference for yourself, the signs you should
- 0:42that you should look for, and the tools that actually help
- 0:45apathy, not make it worse. Plus, I'll give you the exact
- 0:49words to use with your neurologist so you finally get
- 0:53the right treatment and stick around until the very end.
- 0:56Because today's Bryce ISM is the one thing that I now say at
- 1:01every appointment to make sure my doctor never confuses apathy
- 1:05and depression. Welcome to Living with
- 1:08Parkinson's, The Good, the Bad, and the Reality.
- 1:12Real talk, real life, real community with Bryce Perry.
- 1:17Let's begin. Well, welcome back to the
- 1:19channel and thank you to everyone who returns week after
- 1:23week. And if you're new here, my name
- 1:25is Bryce and this is living with Parkinson's, the good, the bad
- 1:28and the reality. And around here, we're all about
- 1:31hope, honesty, and real life tools for navigating
- 1:34Parkinson's. And if that sounds like
- 1:36something you need, hit subscribe to stay connected and
- 1:39join the community. So today we're going to talk
- 1:41about Parkinson's and apathy versus depression and why
- 1:45doctors so often get it wrong and it's so important.
- 1:49So let's just jump right into it.
- 1:51And let's start with the big question.
- 1:55Why do doctors misread apathy as depression in the 1st place?
- 2:00Well, because on the surface, they look almost identical.
- 2:04They both carry low motivation, right?
- 2:06They both have low interest fatigue, that flat effect.
- 2:11And most doctors see those words and automatically think
- 2:14depression. But here's where the path split.
- 2:19You see, depression is emotional.
- 2:23It's heavy, it hurts. There's sadness, hopelessness,
- 2:27emotional pain. But apathy is neurological.
- 2:32It's not sadness. It's more like someone unplugged
- 2:35the spark inside you. So here's how I explain it.
- 2:40If depression feels like sinking in deep water, apathy feels like
- 2:45you're just floating there, not moving, not sinking and not
- 2:49feeling much of anything. So let me tell you a quick story
- 2:52that made the difference for me and really made it clear on what
- 2:56the difference was. So there was a morning where
- 2:59Carmen looked at me and she just said, and she said, are you
- 3:02upset with me? Because you seem very distant.
- 3:05I remember saying like, no, I'm not.
- 3:07I'm not trying to be distant at all.
- 3:11I'm not sad, I'm not angry. I just I don't feel anything.
- 3:15And that's the day I realized that this wasn't depression I
- 3:18was dealing with, it was apathy. So if you've had a moment like
- 3:22that, let me know in the comments makes it will make a
- 3:24huge difference to people watching and everybody's looking
- 3:27at the comments. OK, now that we know the
- 3:31difference between apathy and depression, let's make it even
- 3:34clearer. All right, here are 5 signs that
- 3:38what you're experiencing might be apathy and not depression.
- 3:42And as I go through these, drop a number in the comments for
- 3:46which one fits you. OK, number one, you're not sad,
- 3:51you're just blank. Like, not emotional pain, just
- 3:55complete silence. It's apathy #2 you want to do
- 4:00things, but you just can't get yourself to start.
- 4:03Depression kills desire, but it's apathy that kills
- 4:07initiation. So if you can't get started,
- 4:10that's an apathy problem #3 your joy isn't gone, your energy to
- 4:15begin is 4. It gets worse when your Mens
- 4:19wear off, and that's a dopamine's fingerprint right
- 4:23there. Number 5 other people, other
- 4:26people notice before you do. Apathy is sneaky, and it's
- 4:29sneaky from the inside. So the big question is why does
- 4:35this happen with Parkinson's in the 1st place?
- 4:38Well, let me break that down for you.
- 4:40This is the science made simple, OK?
- 4:45It's like the brain side of this in plain English.
- 4:48But dopamine is your go juice, right?
- 4:51It's your internal go juice. It starts movement, it starts
- 4:54speech, motivation and initiative, OK?
- 4:57And when your dopamine levels dip, your ability to start
- 5:00things dips with it. I like to think of it as like a
- 5:03lamp with a loose cord. Sometimes the light, the light
- 5:06goes bright and it goes on great.
- 5:08Sometimes it flickers, and sometimes it just shuts off
- 5:11completely. Meanwhile, the serotonin system,
- 5:15the one involved with depression, can be totally
- 5:18normal. Which means antidepressants
- 5:22won't fix apathy. They're repairing the wrong
- 5:25wiring. Now if that analogy makes sense
- 5:28to you, tell me about it. If not, I can try it another
- 5:31way. But now let's switch gears and
- 5:34talk about what do we do if you're navigating this alone.
- 5:38So here is a very quick solo spotlight for all of those that
- 5:41are doing this journey without a care person or care partner with
- 5:45them. So number one is the one thing
- 5:48only rule. So don't give yourself a list.
- 5:51Don't give. Just give yourself one tiny
- 5:53thing to start your engine. Walk for three minutes or put on
- 5:57socks. Take meds, that's enough.
- 6:01Second way is visual activation. Let your environment cue the
- 6:05action. Shoes by the bed, water bottle
- 6:08on the counter, meds beside on the coffee table or the side
- 6:12table. You're building yourself a
- 6:13support system that you can see. Also common accountability.
- 6:18So just post here. Just post here.
- 6:20I'm here and I'm doing this solo.
- 6:23Someone will reply and that breaks Apathy's silence and in
- 6:26the comments. Let me know which one you want
- 6:28to try this week. Now here's where things get
- 6:30encouraging. Apathy does improve when you
- 6:33target the right thing. OK, so #1 we're going to target
- 6:38dopamine optimization. So adjust timings, adjust the
- 6:41dose, track your patterns and bring them to a neurologist.
- 6:45The tracker becomes your evidence #2 Another way is micro
- 6:48wins. Apathy hates big goals but loves
- 6:53little ones. One small win leads to another
- 6:57leads to another leads to another #3 external, external
- 7:00structure, timers, routines, reminders.
- 7:04Think of them as like rented motivation.
- 7:07Now, on that note, let me tell you another story.
- 7:09This real moment that made such a difference for me.
- 7:11And I call it like the frozen afternoon, right There was a day
- 7:15when I sat in a chair for almost 2 hours.
- 7:17Not sad, not tired, just kind of stuck.
- 7:22Carmen walked by and she said, hey, try standing up and sitting
- 7:25down once in a while. And that tiny little thing broke
- 7:27the freeze. And it reminded me that apathy
- 7:30doesn't need a big solution. It just needs a spark.
- 7:34So if you've ever had one of those frozen moments, comment
- 7:37frozen so people know that they're not alone.
- 7:40OK, Speaking of Carmen, let's hear from her side of things.
- 7:44Now I want to read to you what Carmen wrote because she sees
- 7:47apathy from a completely different angle, and her insight
- 7:50might help both partners and solo viewers as well.
- 7:53Sometimes apathy looks like withdrawal, and it's easy to
- 7:57take it personally. I used to think Bryce was upset
- 8:00or not interested, but once we understood apathy better, I
- 8:04learned to separate the disease from the person.
- 8:07My approach now is gentle activation.
- 8:10I ask, what's one small thing that you can do right now?
- 8:15And I don't take the I don't take the silence personally
- 8:17because it isn't personal. And if you're a care partner out
- 8:20there, put care partner into this just into the comments.
- 8:24We know that you're here. That was good.
- 8:26Thank you so much, Carmen. It was awesome.
- 8:28So now let's transition and talk about what to say in
- 8:32appointments so this doesn't get misdiagnosed again.
- 8:35And here is the exact, this is the exact sentence that I use
- 8:39now. I like the hey doctor, I'm not
- 8:42experiencing sadness. I'm experiencing lack of of
- 8:46motivation, lack of initiation that changes with my dopamine
- 8:49cycle. This could be apathy.
- 8:52Can we review my medication timings or dosage or dosage?
- 8:57Short, clear and effective. All right, before we wrap up,
- 9:01here comes today's braces with Parkinson's.
- 9:07Depression hurts your heart. Apathy hides your spark, and
- 9:12your spark deserves a chance to shine again.
- 9:16All right, well, there you go. And listen, if this episode
- 9:19helped you, please hit like so more people can find it.

- 9:31you want some deeper support, come join the inner circle.
