- 0:09Hi,
- 0:10I'm Dr. Brenna Hicks,
- 0:11The Kid Counselor.
- 0:12This is the Play Therapy Parenting Podcast where I give you insight,
- 0:15awareness,
- 0:16and enlightenment about your parenting
- 0:18and your relationship with your kids.
- 0:20In today's episode,
- 0:21I'm answering a question from Alison in Texas.
- 0:25And this is about her seven year old
- 0:27currently in play therapy.
- 0:29I'm so thrilled to hear that,
- 0:30Allison,
- 0:32and
- 0:33loves
- 0:34going to therapy,
- 0:36doing really well.
- 0:39Therapist recommended the podcast,
- 0:41so I'm also really thrilled to hear about that.
- 0:44And it says he initially went in because
- 0:47of issues with emotional regulation and anxiety.
- 0:50He is really improving.
- 0:53And the question is,
- 0:54how do I feel about occupational therapy
- 0:57and the combination of doing occupational therapy with play therapy,
- 1:01if that's something that you recommend or if you think
- 1:03they should be separated and done at different times.
- 1:07What a helpful question.
- 1:08I'm so thrilled to be able to tackle this
- 1:11because it's a very common concern,
- 1:14very common consideration,
- 1:15and I don't think I've ever really
- 1:17had a chance to share much about it.
- 1:19So,
- 1:19Alison,
- 1:20thank you.
- 1:22First and foremost,
- 1:22we need to recognize how
- 1:25draining
- 1:26therapy is
- 1:27of any kind.
- 1:28So we have physical therapy,
- 1:29we have occupational therapy,
- 1:31we have speech therapy,
- 1:32we have play therapy,
- 1:33we have
- 1:33all these different kinds of therapies.
- 1:36Any therapy
- 1:37is difficult
- 1:38and it can be in different capacities.
- 1:41So obviously
- 1:42physical therapy is going to be more physically draining.
- 1:45Play therapy is going to be more mentally draining.
- 1:48So obviously it's going to affect
- 1:50children and the body in different ways.
- 1:53But therapy in and of itself,
- 1:55it's tiring
- 1:56and it's a lot to manage,
- 1:58and it can be exhausting.
- 2:00And that's one.
- 2:02So the more we start to add on,
- 2:05and the more therapy sessions a child goes to each week.
- 2:09It compounds
- 2:10because play therapy is draining,
- 2:13and then occupational therapy is draining,
- 2:15and then speech therapy is draining.
- 2:18And especially when a child is struggling and
- 2:21we have recommendations for different treatment options,
- 2:24it can be easy to go,
- 2:26OK,
- 2:26well,
- 2:26we're just gonna do all of them.
- 2:27Like if I can get in and get appointments,
- 2:29we're just gonna do all of them.
- 2:30So we'll do 11 on Monday,
- 2:31one on Tuesday,
- 2:32one on Thursday,
- 2:32one on Friday.
- 2:34We call that over-therapy.
- 2:38And we have to be really careful with that because
- 2:41play therapy in and of itself
- 2:44takes almost everything a child's got.
- 2:47It is so much work.
- 2:49It is so much to process,
- 2:50so much to handle,
- 2:52and it's going to be
- 2:54exhausting for your child.
- 2:56And so then to expect them to be able to
- 2:59pour into a different type of therapy,
- 3:01it's often overwhelming.
- 3:03We don't want kids to be overscheduled.
- 3:05We don't want them to be overwhelmed.
- 3:07We don't want them to be over theapized.
- 3:10And so one of the most helpful strategies that we have
- 3:13is doing one at a time.
- 3:14Here's another
- 3:15factor and consideration that I think is helpful.
- 3:19We also
- 3:20only want to change one thing at a time.
- 3:23Because
- 3:23the second we add in a whole bunch of options
- 3:26and we have multiple variables,
- 3:28causality goes out the window.
- 3:31So let's run a scenario here.
- 3:33Child starts.
- 3:35Play therapy
- 3:38First of April
- 3:39Then they get on ADHD meds,
- 3:42the 10th of April.
- 3:44Then they start
- 3:45occupational therapy the 19th of April.
- 3:49Then in May,
- 3:50they start karate lessons.
- 3:54You can see how by June.
- 3:57If things are changing for better or worse.
- 4:02How in the world do we know
- 4:04what's causing the change?
- 4:07Is it because
- 4:08there's a physical activity outlet with karate?
- 4:11Is it because the meds are helping?
- 4:13Is it because play therapy is
- 4:16helping the child to regulate
- 4:18and self-actualize?
- 4:19Is it that
- 4:21we now have occupational therapy and that is in the mix?
- 4:26We're trying to only change one thing at a time
- 4:29because that gives us insight as to progress.
- 4:32The more variables we have,
- 4:34the more confounded the results.
- 4:36In other words,
- 4:37I can't tell you if it's
- 4:39this or this or this.
- 4:40It could be none.
- 4:41It could be all.
- 4:41It could be one or two.
- 4:44So another consideration is if you're going to start one thing,
- 4:49you need to see that
- 4:50through to
- 4:52The final
- 4:55You need to see that through to completion
- 4:58because
- 4:59then you will know for sure,
- 5:00OK,
- 5:01this was effective or it wasn't,
- 5:02or it helped a little bit or it helped a lot.
- 5:06So not only do we not want a child to be overscheduled and over-therapized,
- 5:09we also don't want to
- 5:11confuse
- 5:12the change process.
- 5:14We're trying to always only change one thing at a time when
- 5:16it comes to children so that we have accuracy in results.
- 5:20I mean,
- 5:21I know how much CCPT works.
- 5:24So,
- 5:24when a child improves,
- 5:26when they become more regulated,
- 5:27when they have a bigger emotional vocabulary,
- 5:30when they self-control,
- 5:32when they have more empathy,
- 5:35I,
- 5:37as a child-centered play therapist says
- 5:39CCPT worked as it always does.
- 5:42But when there are other factors,
- 5:44parents will often say,
- 5:45well,
- 5:45I think it's because they started taekwondo.
- 5:48Or,
- 5:49well,
- 5:50I'm pretty sure it's the meds because we've been on the meds
- 5:52for 2 months now and I think the meds are helping.
- 5:55And
- 5:55I can't argue
- 5:57that theory
- 5:58because we changed more than one thing.
- 6:00I know CCPT works,
- 6:02so I would love to argue and say,
- 6:04no,
- 6:04none of that is relevant except the fact that
- 6:07child-centered play therapy is effective.
- 6:09But when you have different options,
- 6:11it's very hard to determine.
- 6:13So that would be my second consideration.
- 6:15And then my third consideration is
- 6:17often multiple therapies have very different.
- 6:21Approaches and orientations.
- 6:22What I mean by that is,
- 6:23for example,
- 6:24child-centered play therapy,
- 6:26the child
- 6:27is in charge,
- 6:28the child does things in their way at their pace.
- 6:31And when they come,
- 6:33they get to decide what they want to tackle,
- 6:36and we respond accordingly.
- 6:39That would be very confusing for a child to have that kind of freedom with me,
- 6:43for example,
- 6:44on a Tuesday,
- 6:46and then on a Wednesday
- 6:47they go to a different type of therapy
- 6:49that is completely controlled,
- 6:51completely directive,
- 6:52and completely regulated,
- 6:53wherein the therapist says
- 6:55do this
- 6:56like this
- 6:57right now,
- 6:57follow the rules,
- 6:58sit here,
- 6:59listen,
- 7:00and I'm not trying to say that that would be a negative experience.
- 7:03I'm just saying it's so different.
- 7:06Than child centerered play therapy.
- 7:08And so that can be very confusing for a child and it can create friction.
- 7:13Because
- 7:14why do I get to be in charge over here and why do I just have to do what I'm told over here?
- 7:20And if one
- 7:22was tackled first and then the other one was tackled later,
- 7:25it wouldn't seem such a stark contrast.
- 7:28But when they're simultaneous,
- 7:30it can be very difficult for a child to navigate the switch back and forth
- 7:35from having the freedom to not having the freedom.
- 7:38So those are my thoughts on multiple therapies at the same time.
- 7:41Alison,
- 7:41again,
- 7:41thank you for the question.
- 7:42I hope that's helpful for each of you.
- 7:45Especially if a child is diagnosed
- 7:47on the autism spectrum.
- 7:50Many times the recommendations are
- 7:52as many therapies as possible.
- 7:55And just be very thoughtful and careful with that because
- 7:59it's something that we have to tackle with
- 8:01purpose,
- 8:02and there are some reasons to try to limit it to one at a time if that's at all possible.
- 8:09I appreciate y'all.
- 8:10I love you.
- 8:10Hope you have a great week.
