- 0:04Dr Mike Smith (Host): This is Meaningful Medicine, where we dive
- 0:06into medical topics that matter.
- 0:08I'm Dr.
- 0:09Mike, and with me today is Dr.
- 0:10Nicole Belko from Novant Health.
- 0:12And today we're going to be discussing insights and valuable
- 0:16information about pediatric urology straight from the expert.
- 0:21Dr.
- 0:21Belko, welcome to the show.
- 0:23I wanna start off with the age ranges that you see the most.
- 0:28Where does that, fall for you?
- 0:30Dr Nicole Belko: Hi, thank you?
- 0:30so much for having me.
- 0:31I'm so excited to be here today to discuss pediatric urology.
- 0:35so the age range I will see up, you know, zero to 21 years old.
- 0:40see, prenatal consults as well.
- 0:43Dr Mike Smith (Host): So pretty big.
- 0:43You see everybody almost, right?
- 0:45The whole, the whole, the whole, age range thing there.
- 0:49So that's fantastic.
- 0:51how common is it that children experience childhood urinary issues
- 0:58that may prompt further review?
- 1:00Like how, how often are we talking about here?
- 1:03Dr Nicole Belko: Yeah, I think that people don't quite understand
- 1:05or realize how common, urinary or voiding issues are in childhood.
- 1:09You know, up to 40% of children can experience some sort of disruption
- 1:15in the way that they void, and that often prompts urologic evaluation.
- 1:20It's certainly one of the most common things that I
- 1:22see as a pediatric urologist.
- 1:24Dr Mike Smith (Host): And so what are some of the more common, issues that
- 1:28you see out in general pediatrics versus the issues that actually
- 1:33require the specialist like you
- 1:36Dr Nicole Belko: depends on the severity of the issues.
- 1:39You know, so a lot of these issues that general pediatricians see,
- 1:42such as nocturnal enuresis, can often be managed in that setting.
- 1:47if it gets to a point where the parents are feeling like there's no progress being
- 1:50made or the symptoms are so severe that it, they are impacting their lives, and
- 1:56then often that will prompt an evaluation.
- 1:58Another condition, it, that often prompts evaluation is urinary frequency
- 2:02in the pediatric population because that often can be cumbersome to the
- 2:07family if the patient is having to void every several minutes to an hour.
- 2:11And so that is something that I will often commonly see.
- 2:15Dr Mike Smith (Host): You know, with some of the symptoms that present to
- 2:17you, do you ever play kinda let's, let's wait it out, let's see how it
- 2:22goes type of approach before jumping right in to a specific treatment?
- 2:29Dr Nicole Belko: Yes.
- 2:29And so most often and most frequently these urinary disruptions and
- 2:34dysfunctional voiding does resolve with age, and so that is part of the counseling
- 2:39that I will do to, for, with the parents is kind of discuss that although this
- 2:44can be challenging and feel frustrating and sometimes scary, that these issues
- 2:49often do resolve themselves with time.
- 2:52Dr Mike Smith (Host): So, voiding issues, voiding dysfunction, you
- 2:55mentioned nocturnal frequency.
- 2:57These are some of the common things you see.
- 3:00what, are the, in most cases, the causes?
- 3:02You know, if this is something you need to treat, it's not going away with age,
- 3:06what are some of the things driving,
- 3:08the symptoms
- 3:10Dr Nicole Belko: often these symptoms are due to dietary
- 3:14factors and behavioral factors.
- 3:16the largest issue that causes pediatric urinary issues is actually constipation.
- 3:23And so a lot of the work that I do with the parents is educating about the,
- 3:28impact of appropriate stooling habits that have on patient's ability to void.
- 3:35And I also speak about the importance that different food and
- 3:39beverages have on that as well.
- 3:41So dietary modifications and managing constipation are often the standard and
- 3:48the starting point for any treatment with any sort of voiding issues.
- 3:52Dr Mike Smith (Host): but how does that change with-- 'cause you
- 3:53mentioned the age range that you see is pretty much, you know, zero to 21.
- 3:58So how does that change though, with the age ranges?
- 4:03So, you know, so, you know, from a toddler to a teenager.
- 4:06Dr Nicole Belko: the management doesn't often change all that much because
- 4:10the, baseline issue is still the same.
- 4:14And so although, so, but I would say the management shifts a bit to educating
- 4:21the patient themselves in the teenage years because often, you know, teenagers,
- 4:26just as toddlers, are not wanting to drink water or go to the bathroom.
- 4:31And so really educating the patient and kind of imploring them to take
- 4:35control over their dietary habits more so than educating the parent and just
- 4:40pr- trying to provide guidance for them.
- 4:42Dr Mike Smith (Host): Now, do you find if, if you're focused on the, the
- 4:45lifestyle issues, the dietary issues, and, to your knowledge that the patient,
- 4:50the family, they're being compliant with that, does that pretty much resolve
- 4:54the issue or how often do you have to actually go to maybe a pharmaceutical?
- 4:59Dr Nicole Belko: It's very rarely that I have to go to medications,
- 5:02although there are, of course, instances where, after several
- 5:06months of consistent, modifications that that's not quite enough.
- 5:11Typically, these, the medications are used as an adjunct to the modifications.
- 5:18but more often what I will discuss with the parents is the role of
- 5:21actually something called pel- pelvic floor physical therapy and
- 5:24biofeedback with the family, which is far more successful than medications.
- 5:28Because of course, in the pediatric population, we really try to limit the
- 5:32medications we use and try to use them as sort of a bridge while working on the
- 5:37other methods of therapy that we discuss.
- 5:40Dr Mike Smith (Host): Let's move into some of the other urological
- 5:43conditions that you treat.
- 5:46what, are some of the, you know, top two or three that you see often
- 5:50besides what we've already talked about with nighttime urination frequency?
- 5:56Dr Nicole Belko: yeah, I would say that there are several penile and
- 5:58scrotal conditions that are extremely common in the pediatric population.
- 6:03a large amount of pop- patients come in to discuss, something called penile
- 6:07adhesions, and this is what can occur after a newborn circumcision is performed.
- 6:12Over time, the, the foreskin tends to grow a bit, bit quicker than the penis, and so
- 6:17there is often a little bit of an excess.
- 6:19And if that is not cared for during the period, then it can get stuck
- 6:25to the head of the penis, and then I discuss treatment for that.
- 6:28also some other, a scrotal condition I see often is something called an undescended
- 6:33testicle, where the testicle is not within the scrotum, and so I talk to them
- 6:37about the management for that as well.
- 6:40Dr Mike Smith (Host): obviously, in those cases, there's probably a multi,
- 6:43modal approach to treatment with other, other, I would assume other specialties?
- 6:48Dr Nicole Belko: well, for undescended testicle, that is
- 6:51typically just a urologic issue.
- 6:55it's very common.
- 6:57I would s- there are patients who do require multidisciplinary care
- 7:00for other issues where, who commonly have an undescended testicle.
- 7:05But an undescended cu- testicle in itself is a very common isolated
- 7:08pediatric urology condition.
- 7:10Dr Mike Smith (Host): How often do you see
- 7:11that?
- 7:12Dr Nicole Belko: I would say multiple times a week, for sure.
- 7:15it's very common because when the baby is a fetus in the prenatal
- 7:20period, the testicles are actually formed within the abdomen.
- 7:23And while it grows within the abdomen, the testicles are supposed
- 7:26to descend into the scrotum.
- 7:28And so it commonly, the testicle can commonly not make it all the way.
- 7:32and then if it has not descended in the first six months of life, then we talk
- 7:36about surgical approaches to manage that.
- 7:39Dr Mike Smith (Host): Let's talk a little bit about drinking enough water.
- 7:42That seems to be a problem for a lot of kids.
- 7:47how can parents and other adults support kids, e- especially like the
- 7:52athletes, right?
- 7:53In making sure they're getting enough
- 7:55water?
- 7:56Dr Nicole Belko: Yeah, I mean, that's a great question and it's obviously, you
- 7:58know, very-- can be very challenging, to encourage the child to drink enough
- 8:03water, especially when there are a lot of other beverages out there.
- 8:06I really just try to voice the importance to parents and, have them
- 8:10try to lead by example and p-providing water as the mainstay in the household.
- 8:14And now, you know, with how the popularization of water bottles these
- 8:19days, I really, you know, emphasize and try to encourage the parents to g-go and
- 8:24just purchase a water bottle to-- for their child to carry around with them.
- 8:29And, you know, a lot of schools, a lot of schools allow them to do that.
- 8:32the, same thing with the athletes, you know, just really trying to encourage
- 8:36the family to switch over the majority of the fluids in the house to water-based
- 8:40water products and treat other beverages more as like a, a treat or a, a special
- 8:44beverage, and really encourage the use of a water bottle throughout the day.
- 8:50Dr Mike Smith (Host): seems to have been an explosion of, flavored
- 8:54waters, you know, little drops
- 8:57that you can put into water.
- 8:58Are you-- What do you think about
- 9:00those?
- 9:00Dr Nicole Belko: Yeah, I do tell patients that I am okay with the drops being added
- 9:04to the water to provide with some flavor.
- 9:06I do caution parents, and teenagers about sparkling water, as there is
- 9:11still carbonation in that water, which can be irritating to the bladder,
- 9:15which can perpetuate their symptoms.
- 9:19and I try to caution them about, you know, flavored waters because often they
- 9:22have sugars, like a lot of sugar in them.
- 9:25But the drops I find to be a suitable alternative.
- 9:30Dr Mike Smith (Host): Okay.
- 9:30Yeah.
- 9:31If it gets the kids to drink water, it, it, it's, it's a good solution, right?
- 9:34At the end of the day, if it's safe.
- 9:36it's funny you mentioned the w- water bottles.
- 9:39I mean, it-- There was a time, Dr.
- 9:40Belko, where they were unheard of.
- 9:43And now, and now you find it every…
- 9:45Now, now apparently, if you don't have one, you're not in or something
- 9:48Dr Nicole Belko: E-exactly.
- 9:49And so I really just try to ride that, that, you know,
- 9:51ride that as much as I can because, having water
- 9:55bottles is hip these days.
- 9:57And so, and with that, they're very affordable at,
- 10:01the local stores around town.
- 10:03And so I pr- often provide some examples of stores that have, you
- 10:06know, high-quality water bottles for a very reasonable price
- 10:10Dr Mike Smith (Host): Now, when it comes to water, do you like to give
- 10:13a specific amount that kids should be drinking, or are you just more
- 10:18about more hydration in general?
- 10:20H- how do you talk to that?
- 10:22Dr Nicole Belko: Well, with children since, you know, their weight is often
- 10:24changing, I don't like to give a, hard stop amount as I would in adults.
- 10:29But I do try to encourage them to substitute all the fluids that they
- 10:33were be, would be drinking with all of their meals with water and make sure
- 10:37that they're at least having, you know, several large cups of water a day.
- 10:42Dr Mike Smith (Host): Now, what about, other conditions that you treat?
- 10:44Now, we've touched on few things.
- 10:46Is there anything else you'd like the listening audience to know about,
- 10:49when it comes to your specialty and, and treatment of disease?
- 10:53Dr Nicole Belko: pediatric urology at the end of the day is
- 10:55actually a, a very vast field.
- 10:58some other conditions that I can touch on, you know, that people may
- 11:02not know about is, you know, there's conditions of the kidneys as well
- 11:06that I see, such as hydronephrosis.
- 11:08Many babies are born with congenital swelling of the kidney, and so
- 11:12that is something that I follow.
- 11:14there are children who get something, recurrent urinary tract infections, and
- 11:17so those often are, can be due to several anatomical diseases, one being where when
- 11:23you pee, instead of the pee going out, the pee, some of the pee can go backwards.
- 11:27That's called vesicoureteral reflux.
- 11:30And then, you know, another really common penile condition I see that, you know,
- 11:33people are not super educated about is something called hypospadias, where
- 11:38the urethra or where the tip of the pee comes out of does not, is not on the
- 11:43lo- correct location of, on the penis.
- 11:45And so it, it can be, anywhere else alongside the penis.
- 11:48And so that's something that I often treat as well.
- 11:51Dr Mike Smith (Host): Dr.
- 11:51Belko, this has been great information.
- 11:54As we kinda wrap things up here, I'd like to ask you got into urology.
- 11:59How did you know this was your passion?
- 12:01Dr Nicole Belko: Yeah, you know, great question.
- 12:02You know, I think that often urologists do get asked this question for sure.
- 12:05for me, I really wanted to do a surgical field that would impact the quality of
- 12:09life for people, and I wanted to have a close, you know, working relationship with
- 12:14patients with good continuity of care.
- 12:16And in urology, you know, voiding complaints can be very intimate
- 12:21and be very stressful for people to go through and have to talk about.
- 12:26and wanting to support people through some uncomfortable times in their lives.
- 12:31Dr Mike Smith (Host): That was fantastic.
- 12:32Dr.
