- 0:31This is Madeline Ashley with the Becker CFO
- 0:34and Revenue Cycle podcast, and I'm excited to
- 0:36be joined today by Nick Olson, executive vice
- 0:39president and CFO of Sanford Health. Nick, it's
- 0:42great to have you back on with me.
- 0:44Yeah. Thanks for having me. Appreciate it.
- 0:46Yeah. I'm really excited to dive into the
- 0:48convo. But before we do, do you mind
- 0:49sharing just a little bit about yourself and
- 0:51Sanford Health with our listeners who might not
- 0:54have caught one of our previous discussions?
- 0:56I have the privilege of serving as the
- 0:58executive vice president and chief financial officer at
- 1:01Sanford Health, the largest rural health system in
- 1:04The United States, headquartered in Sioux Falls, South
- 1:06Dakota.
- 1:07My background includes leadership roles both within Sanford
- 1:11Health and also outside the organization,
- 1:13both of which have shaped my perspective and
- 1:15the lens through which I look through.
- 1:18In my current role, I'm really focused on
- 1:20financial stewardship of our organization
- 1:22to ensure both near term, but more importantly,
- 1:25long term
- 1:26sustainability while also
- 1:28enabling reinvestment into the communities that we are
- 1:31so privileged to serve.
- 1:33Sanford Health serves more than 2,000,000 patients and
- 1:36approximately 430,000
- 1:38health plan members across
- 1:40320,000
- 1:41square miles, a region that is 99%
- 1:45rural.
- 1:46With over 12,000,000,000 in annual revenue, our integrated
- 1:50nonprofit health system includes
- 1:5257 hospitals,
- 1:53approximately
- 1:55290 clinic locations,
- 1:57a 150
- 1:58senior living locations,
- 2:00around 4,000 physicians and advanced practice providers,
- 2:04more than 1,100
- 2:05active clinical trials and studies, and over fifty
- 2:08five fifty five thousand employees.
- 2:11Nearly two thirds of our patients come from
- 2:14those rural communities that I touched on across
- 2:16the Upper Midwest.
- 2:1870% of our residents through, served through Good
- 2:21Samaritan,
- 2:22and approximately
- 2:24two thirds of our Sanford Health Plan members
- 2:26also live in rural communities. Further, many of
- 2:28these counties that we serve are federally designated
- 2:30provider shortage areas,
- 2:32which makes access to preventative
- 2:34and specialty care even more challenging.
- 2:37But all that said, these realities really shape
- 2:39everything that we do. They drive our commitment
- 2:41to eliminate barriers, improve,
- 2:44outcomes, and deliver high quality care no matter
- 2:47where our patients live.
- 2:50I really appreciate you sharing that. And, you
- 2:51know, it sounds like Sanford Health definitely stays
- 2:53busy. You guys are a a wonderful organization,
- 2:56so I truly appreciate you sharing.
- 2:58Now, Nick, I know, again, we've we've talked
- 3:00many times, and I feel like there's always
- 3:01something going on in health care. It is
- 3:03never a a dull industry.
- 3:06So I would love to hear, you know,
- 3:07as we're entering 2026,
- 3:09some of the financial trends or shifts that
- 3:11you're keeping a close eye on, particularly maybe
- 3:13even around the rural health care side of
- 3:16things right now and and why you're you're
- 3:18watching these trends.
- 3:19Yeah. As we enter 2026,
- 3:21certainly, there's a lot that's top of mind
- 3:23for me. But, really, the the one thing
- 3:25that I am most focused on heading into
- 3:28this new year
- 3:29is really this continued acceleration
- 3:31towards
- 3:32value based care and the opportunity that it
- 3:34creates to improve outcomes,
- 3:37to lower the total cost of care, and
- 3:39really to build healthier communities for our patients,
- 3:42for our members, and for our residents.
- 3:45There's certainly growing
- 3:47recognition across the industry that aligning
- 3:49care delivery, aligning coverage,
- 3:52and the financing with which that coverage is
- 3:54provided,
- 3:55all of which is essential to delivering long
- 3:57term value.
- 3:59And here at Sanford Health, value based care
- 4:01is a strategic priority for us.
- 4:03We're establishing a value based care committee to
- 4:06strengthen
- 4:07the system wide alignments,
- 4:09and, really, this committee is gonna be focused
- 4:11on four key areas.
- 4:13The first of which is gonna be aligning
- 4:15incentives between providers,
- 4:17between payers, and between the patients through contracting.
- 4:21The second of which will be through enabling
- 4:24technology and data analytic tools.
- 4:27The third of which is gonna be supporting
- 4:28care model transitions,
- 4:30and the fourth and final area is gonna
- 4:32be strengthening core health plan operations.
- 4:35As a fully integrated system
- 4:37with a health plan, we believe that Sanford
- 4:40Health is really uniquely positioned to lead this
- 4:43transformation.
- 4:45And integration really allows us to align incentives
- 4:47across,
- 4:48care delivery and coverage, improve that care coordination,
- 4:52and the efficiency and focus on that total
- 4:54cost of care that I mentioned.
- 4:56And at the end of the day,
- 4:57if if done right, which we believe we
- 4:59will do this correctly,
- 5:01lowering the total cost of care can be
- 5:03a win win. Health care can become more
- 5:05affordable for our patients, which we know are
- 5:07top of mind,
- 5:08for each of our patients in our communities,
- 5:10but we can also remove cost from the
- 5:12system, making us more sustainable
- 5:14long term.
- 5:15Really, for those reasons,
- 5:16the shift towards
- 5:18that lowering of the total cost of care
- 5:20via value based care arrangements is,
- 5:22one of the things that's certainly top of
- 5:24mind for me as we begin 2026.
- 5:26Yeah. That's such an important thing to to,
- 5:28you know, keep an eye on as well.
- 5:30I would also be curious to hear,
- 5:32you know, here at Becker's, we've done a
- 5:34lot of coverage just in terms of the
- 5:36rural health funding.
- 5:37I was curious if you had any thoughts
- 5:39there right now. I I do believe all
- 5:4150 states, of course, got the the funds.
- 5:43You know, how that's being distributed is there's
- 5:45some question marks. So how are you following
- 5:47this right now?
- 5:48Yeah. I know it's, as we look at,
- 5:512026, you know, one of the other things
- 5:52that's,
- 5:54certainly top of mind for me is just,
- 5:56you know, the areas that we think about
- 5:57as we continue to evolve and and to
- 5:59grow. And one of those areas,
- 6:01amongst other things is the rural health transformation
- 6:04program that you touched on.
- 6:06It's, it's really a significant opportunity,
- 6:09really in the history of health care to
- 6:11accelerate progress in in rural health care, which
- 6:14is really what we're all about. As I
- 6:16mentioned,
- 6:17our significant Yeah. Health rural presence here in
- 6:20the Upper Midwest. And,
- 6:22you know, I think, our our 300,000
- 6:24square mile rural footprint,
- 6:27fully integrated model and and proven record of
- 6:29innovation
- 6:31really uniquely positions us to partner with CMS
- 6:34to to transform what the future of rural
- 6:37health care can look like.
- 6:39And,
- 6:40Yeah. You know, we've built and scaled certain
- 6:43capabilities
- 6:44across
- 6:46virtual care, across value based care.
- 6:49We've invested a lot into workforce development, into
- 6:52digital
- 6:53infrastructure,
- 6:54and delivering, you know, improvements in access outcomes
- 6:57and affordability
- 6:59for,
- 7:00rural
- 7:01rural communities.
- 7:02And, you know, maybe just by way of
- 7:04a couple of examples,
- 7:06you know, and and, the way of virtual
- 7:08care infrastructure, we've delivered more than 1,000,000
- 7:11virtual visits
- 7:12across nearly 80 specialties
- 7:15supported, certainly by our new virtual care center
- 7:18and hub and spoke model.
- 7:19You know, this approach, certainly,
- 7:22helps extend that specialty care gap that I
- 7:24touched on. It allows us to extend our
- 7:27clinical trials.
- 7:30And, you know, from a workforce development perspective,
- 7:32we have one of the most extensive
- 7:34rural workforce programs in the country,
- 7:36inclusive of graduate
- 7:38medical
- 7:39education, mobile simulation, and academic and community partnerships
- 7:42that strengthen
- 7:43pipelines for,
- 7:45the critical clinical roles that we're looking to
- 7:47fill. And so, really,
- 7:49as we think about that infrastructure that we
- 7:50have,
- 7:52kind of in light of this new rural
- 7:54health transformation program, we see a big opportunity
- 7:57to accelerate
- 7:58what is already working. Right? These are not
- 8:00net new things that you're already working.
- 8:02But, certainly, you know, we look to expand
- 8:04those programs to address the the barriers,
- 8:08that that we've touched on around workforce shortages,
- 8:11interoperability
- 8:12fragmentation.
- 8:14And so, really, ultimately, this program aligns directly
- 8:16with our mission, and we're certainly
- 8:18excited about the opportunity that it presents for
- 8:20us.
- 8:22Something that can, you know, even further set
- 8:23you up for success. So, yeah, that's great
- 8:25to hear. I'd like to shift gears a
- 8:27little bit and talk about some maybe some
- 8:28of the biggest opportunities
- 8:30for smart growth that you're seeing, in the
- 8:32year ahead. Clearly, you know, as we've also
- 8:35covered here at Becker's, the Sanford Health Marshall
- 8:37Clinic merger occurred, you know, just a little
- 8:39bit over a year ago. So, how are
- 8:41things looking there? What what does smart growth
- 8:43look like to you this year?
- 8:46Yeah. You know, in addition to the the
- 8:48rural health transformation program, which we just touched
- 8:50on and, we're we're putting a lot of
- 8:52emphasis into,
- 8:54There's a there's a couple of other areas
- 8:56that are top of mind,
- 8:58for us.
- 8:59The first of which is really the focus
- 9:02on continued integration,
- 9:04and expanding care in and around the communities
- 9:07that we already provide care in. Right? We
- 9:09are fortunate, super fortunate to be the provider
- 9:12of choice in many of the communities that
- 9:14we have a presence,
- 9:15and we need to maintain that trust by
- 9:17continuing to expand access,
- 9:19ensuring there is capacity in our facilities,
- 9:22investing in new technology,
- 9:24and listening to where our patients need even
- 9:26more from us.
- 9:27You mentioned the, integration
- 9:30with, Marshall Clinic Health System and others. Integration
- 9:33remains central to our strategy. We are not
- 9:35a holding company. We never will be a
- 9:36holding company.
- 9:38Following our merger with Marshall Clinic Health System
- 9:41in 2025,
- 9:42in addition to our expansion,
- 9:45in the Rapid City in the Western Part
- 9:46of South Dakota,
- 9:48our combination with Prairie Lakes Health System in
- 9:50Watertown, South Dakota, and the acquisition of Lewis
- 9:53drug, all of which occurred in 2025.
- 9:56We are continuing the strong focus of integrating
- 9:59them into our organization and fully leveraging
- 10:01the strengths of an integrated system to deliver
- 10:04more patient centered care, ensuring we can invest
- 10:07into the communities,
- 10:09in the years to come. We're also focusing
- 10:11our health plan efforts, in geographic markets where
- 10:14we can provide both that care and coverage
- 10:17because we found that where we can do
- 10:19both of those things is where we can,
- 10:21deliver the greatest value for those patients and
- 10:23and members.
- 10:25And, you know, continuing to invest into our
- 10:27legacy communities remains central to our mission and
- 10:29our long term strategy.
- 10:31Yeah. The other area I see opportunity for
- 10:33smart growth
- 10:34is through,
- 10:36strategic partnerships.
- 10:38Our growth strategy is fundamentally about strengthening access,
- 10:42improving quality, and doing so in a a
- 10:44sustainable way.
- 10:46And we are focused on not only inorganic
- 10:48growth, but looking to, you know, partner with,
- 10:52strategic mission driven organizations.
- 10:55We're not focused on growth just for growth's
- 10:57sake, but about building a strong foundation for
- 11:00the future,
- 11:02which we think can enable us to make
- 11:04the the necessary investments and whether whatever headwinds
- 11:07that, may come at us in the future.
- 11:09And so our our strategy is rooted in
- 11:11in meeting community needs. Every combination in our
- 11:15history,
- 11:16has been focused on expanding specialty care, enhancing
- 11:19quality,
- 11:21and bolstering provider recruitments,
- 11:23to ensure that long term sustainability.
- 11:25And so, you know, for us, the future
- 11:27of health care
- 11:28really belongs to organizations
- 11:30willing to combine both that purpose and vision,
- 11:34recognizing that opportunities and challenges ahead are really
- 11:37too big for any one system,
- 11:39to tackle alone. So,
- 11:41those are a couple of other areas that,
- 11:43we're focused on as we look at and
- 11:45think about growth in 2026.
- 11:48So fair to say that, you know, last
- 11:50year was a big year for acquisition and
- 11:52partnership, and you're not ruling anything out for
- 11:54this year as well in terms of any
- 11:56additional partnerships
- 11:57or acquisition.
- 11:59Yeah. I mean, again, we're gonna focus where
- 12:01we're currently at. We've got a lot of
- 12:03needs that our patients, and our existing communities
- 12:05need for us, so that's gonna be a
- 12:07focus area. We've got a lot of intentionality
- 12:09around continuing to integrate
- 12:11those organizations we merged with in 2025.
- 12:14We're gonna focus on the rural health transformation
- 12:16program. We're also gonna keep, kind of an
- 12:19ear open for other,
- 12:21kind of geographically aligned systems that,
- 12:24may make sense, and that have cultural alignment
- 12:27to Sanford Health.
- 12:29And, you know, with with all of the
- 12:30great activity that happened with Sanford last year,
- 12:33could you share maybe one lesson that you
- 12:35took away from 2025
- 12:37that you're carrying now into 2020?
- 12:40Yeah.
- 12:422025 was a year of of growth for
- 12:44Sanford Health, and certainly
- 12:46at the top of the list of important
- 12:48lessons,
- 12:49for 2025 for me,
- 12:52again, during the year when we welcome several
- 12:54new organizations
- 12:55and teams into Sanford Health is the power
- 12:58of cultural alignments,
- 13:00particularly during these periods of integration and change.
- 13:03Alignment around
- 13:05mission, around vision and values and expectations is
- 13:09extremely critical
- 13:11as operational and financial integration begins, to take
- 13:14place.
- 13:15We've found that when teams are culturally aligned,
- 13:18this integration can move faster.
- 13:21Buy into change can become easier, and decisions
- 13:24become
- 13:25more clear.
- 13:26And that change that is certainly always present
- 13:29becomes embraced and not a challenge.
- 13:31And so that that alignment,
- 13:33begins to build trust, begins to
- 13:36build accountability and momentum across a very complex
- 13:39organization.
- 13:42And so, you know, another key lesson is
- 13:44the importance of viewing challenges as opportunities. You
- 13:47know, workforce constraints,
- 13:49access gaps,
- 13:51evolving economic conditions have created opportunities,
- 13:55right, to innovate, to strengthen partnerships, and rethink
- 13:57how care is delivered. And so as we
- 13:59move into 2026, we're focused on really deepening
- 14:03that cultural alignment across our integrated integrated system
- 14:07and continuing to approach challenges and complexities
- 14:10not as negatives, but as optimism
- 14:12and purpose. And so that mindset positions us
- 14:15well,
- 14:16again, around our focus areas of how can
- 14:18we continue to advance access,
- 14:20improve quality, and do so in a financially
- 14:22sustainable way for both,
- 14:24our communities,
- 14:26today and into the future.
- 14:28Yeah. Some strong, lessons learned there. Thank you
- 14:31so much, Nick. And, again, it is always
- 14:34such a pleasure having you on the podcast
- 14:36and getting to chat with you. Really eager
- 14:38to see all the exciting things that Sanford
- 14:40Health does this year. So, hopefully, we'll get
- 14:42to connect again soon.
- 14:44Sounds great. Thanks for having me. I appreciate
- 14:45it.
