- 0:26This is Haley Redker with the Becker's Healthcare
- 0:28podcast. And today, we are recording live at
- 0:30the Becker's twenty twenty six annual meeting.
- 0:33Right now, I am joined by Teresa McDonnell,
- 0:35senior vice president and chief nursing officer at
- 0:38Duke University Health. Teresa, thank you so much
- 0:41for joining for me today. It is my
- 0:43absolute pleasure to be here with you all
- 0:44this morning.
- 0:46Now to start, could you briefly introduce yourself,
- 0:48your role, and the scope of your organization,
- 0:50and what's currently commanding most of your attention
- 0:52as a leader?
- 0:53Sure.
- 0:54So I serve,
- 0:56Duke Health as the senior vice president
- 0:59of patient care services, chief nurse executive,
- 1:02and vice dean of clinical affairs for the
- 1:05Duke University School of Nursing.
- 1:08Duke Health is about a $9,100,000,000
- 1:11enterprise
- 1:12that has four acute care hospitals
- 1:14in North Carolina, 250
- 1:17plus ambulatory
- 1:19sites throughout the state,
- 1:21as well as Ducomb Health and Hospice, which,
- 1:24spans a tristate
- 1:26area.
- 1:28The thing that's commanding
- 1:30most of my intent my attention as a
- 1:32leader these days is clinical care transformation.
- 1:36We are at a critical juncture in health
- 1:39care, and we have to transform the way
- 1:41we do things.
- 1:43Well, thank you so much for that introduction.
- 1:45Now I'd like to get started here. So
- 1:48in a year marked by rapid change, what
- 1:50recent decision or pivot required the most leadership
- 1:52conviction from you, particularly when it came to
- 1:55aligning people, resources, and capabilities?
- 1:58So I I would say that,
- 2:00the decision that is is most consuming of
- 2:03my time and resources
- 2:05is the decision to
- 2:07truly integrate virtual care into our acute care
- 2:10and ambulatory
- 2:11settings.
- 2:12And what I mean by that is we
- 2:14are transforming
- 2:15the traditional model of care.
- 2:18Our inpatient nurses are being cross trained to
- 2:20deliver virtual care. We do believe this is
- 2:23going to be the model of the future,
- 2:25and we're
- 2:27now transitioning
- 2:28to team based models
- 2:31where the same team of nurses will work
- 2:33in a in a mixed methodology,
- 2:35virtual and on-site, caring for the patient population.
- 2:40This has taken,
- 2:41some technology
- 2:43resource as well as a lot of human
- 2:45capital in training and change management.
- 2:48How did that moment challenge your assumptions, and
- 2:50what did it change about how you approach
- 2:52decision making, talent, or organizational readiness today?
- 2:56So I I think, you know, traditionally,
- 2:59we have relied on the legacy model of
- 3:03a nurse in the acute setting caring for
- 3:07a ratio of patients, one to four, one
- 3:09to five, one to three based on acuity.
- 3:13And
- 3:13what we're changing
- 3:15is that whole paradigm. We're really trying to
- 3:18lean in and look at acuity
- 3:20and needs of a patient population
- 3:22so that we can meet the needs and
- 3:25do things like decreasing length of stay, but
- 3:27more importantly, decreasing the burden on our staff.
- 3:30You know, there's been a big narrative around
- 3:33burnout and around turnover, and by transitioning the
- 3:37care model and easing the burden
- 3:39on our talent,
- 3:41that hopefully is going to d lead to
- 3:43decreased turnover and increased satisfaction for patients as
- 3:46well as staff.
- 3:48When you think about the forces reshaping health
- 3:50care right now, where do you believe leaders
- 3:52need to be more decisive,
- 3:54especially when balancing speed, risk, and having the
- 3:56right teams in place?
- 3:58Well, I I think we need
- 4:00to decide as health care leaders that the
- 4:03way we've done things is not going to
- 4:04be the way we're going to do them
- 4:06moving forward.
- 4:07We need to truly transform the way we're
- 4:10delivering care.
- 4:11It needs to be less resource intense,
- 4:14more patient and staff centered.
- 4:16And, you know, in order to do that,
- 4:19we really need to think about
- 4:23how we integrate technology
- 4:25into the care setting and because there's tremendous
- 4:28risk. If staff don't feel like they're engaged
- 4:30in the decision making,
- 4:32they're not going to be willing to come
- 4:34with us on this journey of transforming care.
- 4:37And as uncertainty has become the norm, what
- 4:40leadership habit or mindset has been most critical
- 4:42in keeping your organization and your people moving
- 4:44forward?
- 4:46Well, at at Duke Health, we have something
- 4:48called, our cultural commitments. And the first one
- 4:50of those cultural commitments is putting people first.
- 4:53And what we mean by that is putting
- 4:55our staff and our patients first.
- 4:57So when we think about keeping our people
- 5:02engaged and moving forward,
- 5:04it's really taking those frontline staff that are
- 5:06going to be most impacted by technology
- 5:09and having
- 5:11them join us in co designing and
- 5:14figuring out how to effectively integrate these technologies
- 5:18into care. You know, we all know that
- 5:20the people closest to the work are the
- 5:22ones who can easily identify the the problems,
- 5:24and they are the ones who are going
- 5:26to solve the problems with us.
- 5:28And for my final question today, as technology
- 5:31becomes more embedded in daily operations, how are
- 5:33you ensuring digital investment support rather than straying
- 5:36your workforce?
- 5:37So we've actually made some pretty significant investments,
- 5:41in integrating technology
- 5:43for staffing.
- 5:44So we're about to go live with an
- 5:46AI enabled staffing tool where staff can self
- 5:50schedule,
- 5:51and we've integrated it with CareHog so that
- 5:54we have now a closed loop
- 5:56of demand and capacity when it comes to
- 5:58our staffing. So our goal in all of
- 6:00this is to be more efficient with our
- 6:02staffing, less less flexing down, less floating,
- 6:06and really enabling
- 6:08to make sure that we've got the right
- 6:10staff staffing complement for the right group of
- 6:13patients in the right setting, but also ensuring
- 6:16that our staff have more control in how
- 6:19they're scheduled and how their time is spent.
- 6:21Absolutely. Well, that's it for today's conversation. Theresa,
- 6:25thank you so much for joining me today.
- 6:27Again, we are recording live at the Becker's
- 6:29twenty twenty six annual meeting. Thank you. Thanks
- 6:32so much.
