- 0:33Hello, everyone. This is Elizabeth Cassello with the
- 0:36Becker's Payer Issues podcast.
- 0:38I'm thrilled today to be joined by Bethany
- 0:41Stein, segment president of pharmacy at Humana.
- 0:44Bethany, thanks so much for chatting with us
- 0:46today. Yeah. Happy to be here. Thanks for
- 0:48having us, Liz.
- 0:49Yeah. So before we jump into the conversation,
- 0:52can you tell us a little bit more
- 0:54about yourself, your health care career background,
- 0:57and what your role is at Humana?
- 0:59Yeah. Happy to do that.
- 1:01Bethany Stein, as Liz said, president of Humana's
- 1:04pharmacy segment, CenterWell Pharmacy. I am a pharmacist
- 1:08by trade,
- 1:10and started at Humana right out of pharmacy
- 1:12school as a resident.
- 1:14So today, I oversee the strategic growth operations
- 1:17and p and l of Humana's three different
- 1:20pharmacy businesses, which include
- 1:22CenterWell Pharmacy, our home delivery pharmacy,
- 1:24CenterWell Specialty Pharmacy, and then Humana Pharmacy Solutions.
- 1:29And I would just add that my team's
- 1:31focus is incredibly simple. Right? We wanna make
- 1:34pharmacy more transparent, more consumer friendly,
- 1:37and more clinically effective at scale.
- 1:41That's great to hear, Bethany, and thanks again
- 1:43for joining us.
- 1:44So the big news I wanna talk about
- 1:46today is how Humana Centerwell Pharmacy is going
- 1:49to dispense Eli Lilly obesity management drugs
- 1:52through its employer plan sponsored carve out programs.
- 1:56What are employers asking for right now when
- 1:58it comes to GLP one coverage, and how
- 2:00does this partnership
- 2:02enable Humana to respond to that demand?
- 2:05Yeah. Thanks, Liz. I love this question. I
- 2:07think employers are in a very different place
- 2:09with GLP one coverage today than even a
- 2:12year ago.
- 2:13The conversation has essentially shifted from should we
- 2:16cover them to how we cover them responsibly.
- 2:20They're looking for predictability,
- 2:21control, and transparency,
- 2:24but without
- 2:25restricting access for the people who truly need
- 2:27these medications.
- 2:29So what we're hearing consistently from employers is
- 2:31that not all GLP one use is the
- 2:34same.
- 2:35Right? So different employee populations
- 2:37have very different clinical needs, whether that's dose
- 2:40escalation,
- 2:41duration of therapy,
- 2:42or just like the type of behavioral support
- 2:44required to be successful.
- 2:47So employers
- 2:48are increasingly asking for benefit designs that stratify
- 2:52populations
- 2:53thoughtfully,
- 2:54rather than treating GLP ones as one large
- 2:57undefined cost category.
- 2:59So for example,
- 3:01some programs that distinguish between
- 3:03high risk metabolic patients that may see the
- 3:05greatest downstream medical savings. Right? A weight only
- 3:09population
- 3:10where employers may want more guardrails and accountability.
- 3:14And then finally, maintenance versus initiation phases where
- 3:18intensity and cost can differ significantly.
- 3:21So that kind of segmentation
- 3:23helps employers move from this broad utilization
- 3:26concern
- 3:27to an intentional strategy where they can support
- 3:30the right patients without runaway spends.
- 3:34And, honestly, Liz, like, the cost pressures are
- 3:36real. Right? A recent Gallagher study showed that
- 3:39GLP one drug class spend increased 35%
- 3:42in 2024
- 3:44and another 25%
- 3:45in 2025.
- 3:46So even with good clinical outcomes, that cost
- 3:50trajectory is truly outpacing
- 3:52the near term medical savings.
- 3:54So employers are rethinking their models and leaning
- 3:57into approaches that give them clear line of
- 3:59sight to both utilization
- 4:01and value.
- 4:03And so, again, going back to
- 4:05integrating segmentation,
- 4:07clinical oversight,
- 4:08and a more transparent pharmacy model, we at
- 4:11CenterWell are really helping employer structure GLP one
- 4:14benefits
- 4:16in a way that meets the moment,
- 4:18sustainable,
- 4:18intentional,
- 4:20and still high quality for their members. So,
- 4:22ultimately, it's about giving employers the confidence that
- 4:25they can offer these important therapies without sacrificing
- 4:29affordability
- 4:30and giving employees
- 4:31a support model that actually helps them succeed.
- 4:34So I would just say that is in
- 4:35general why we are so excited
- 4:37with where we're going in our partnership with
- 4:40Lilly and other various partners within this ecosystem.
- 4:44Yeah. So I was actually going to ask
- 4:45about those other partners just to give context
- 4:48to our listeners.
- 4:49Humana is also working with Novo Nordisk on
- 4:52direct to patient, we go via access.
- 4:54And there's also just beyond that, some recent
- 4:57research points to slowed medical cost growth for
- 4:59g l p GLP one users in the
- 5:01long term.
- 5:03So how do those collaborations
- 5:05with manufacturers
- 5:06so Eli Lilly, like what we're talking about
- 5:08today, Novo Nordisk,
- 5:10work together to bend that cost curve without
- 5:12restricting access? Just elaborating a little bit more
- 5:14on those stratification points that you mentioned earlier.
- 5:18Yeah. I would just say, I think GLP
- 5:20ones are both clinically transformational
- 5:23and financially challenging at scale. Right? And so
- 5:26that's exactly why we're talking about such a
- 5:29deliberate approach.
- 5:30So Humana
- 5:32is building a consumer focused health care company,
- 5:34and pharmacy is a strong expression of just
- 5:37that strategy.
- 5:39Whereas CenterWell Pharmacy, just as as background information
- 5:42for you all, is was built originally for
- 5:45the health plan by the health plan. So
- 5:48from day one,
- 5:50it has been designed around outcomes,
- 5:53transparency, and affordability,
- 5:54not spread pricing, rebate fees, or intermediaries.
- 5:58And so we have results that really speak
- 6:00for themselves in the form of better outcomes.
- 6:03Right? CenterWell patients have
- 6:0515% fewer ER visits and 13%
- 6:09lower hospitalizations
- 6:10than patients using any other pharmacy in our
- 6:12network, and we validate that annually.
- 6:15We see better experience.
- 6:16Right? Our net promoter score is consistently 15
- 6:20points higher than other pharmacies.
- 6:22We we deliver that true transparency. Right? So
- 6:25for thirty years, we've operated with no hidden
- 6:28fees, no gains, and complete visibility for the
- 6:30health plan.
- 6:32And then lastly, affordability
- 6:33is just built into our model. Right?
- 6:36At at the highest level, 90% of our
- 6:38generics cost less than $2 a month, many
- 6:40with no co pay at all.
- 6:42And we truly proactively
- 6:44help members find lower cost alternatives
- 6:46and financial assistance for high cost brands and
- 6:49specialty drugs.
- 6:50So when you combine all that
- 6:53with home delivery
- 6:54scale and a consumer first mindset,
- 6:57you really end up with a pharmacy model
- 6:59that is uniquely aligned to what the GLP
- 7:02one marketplace requires,
- 7:04predictable pricing and clinical integrity.
- 7:07And so that's why we're expanding this model
- 7:10beyond Humana members to a direct to patient
- 7:13pharmacy solution model for GLP ones. And so
- 7:16to get to your question,
- 7:18our collaborations with manufacturers like Novo Nordisk and
- 7:21Eli Lilly are built upon those same principles.
- 7:24We just simplified access at a lower out
- 7:27of pocket cost. And so this truly does
- 7:30bend the cost curve and expand access,
- 7:33and we're doing it
- 7:34exactly how a consumer health care company should
- 7:37do it. Right? Simplifying the ecosystem,
- 7:40removing friction and waste,
- 7:42delivering predictable transparent pricing,
- 7:45improving outcomes and adherence,
- 7:46and scaling safely and responsibly.
- 7:49So we're not restricting access. We're expanding safe,
- 7:53affordable, high quality access
- 7:55while building a cost structure that is sustainable
- 7:58for payers, employers, and consumers over the long
- 8:01term.
- 8:02And I believe that this is really the
- 8:05future of pharmacy care. Right? A consumer centric,
- 8:08clinically guided, transparent,
- 8:11and and it's built to support the next
- 8:13generation of therapies at scale.
- 8:16So I actually wanted to ask a little
- 8:18bit about that adherence point because that is
- 8:20such a concern here.
- 8:22What
- 8:23steps is Humana taking while they're working
- 8:26with employers
- 8:28on promoting adherence?
- 8:30And what what else do you have to
- 8:32say on adherence?
- 8:33Because that is just such a big concern
- 8:35in terms of the cost here.
- 8:37Yeah. I totally agree. And if you if
- 8:39you taking a step back, I would say
- 8:41the strongest support around medication adherence
- 8:44is a lot of the clinical programs and
- 8:46support elements that we have in place today.
- 8:49So just another metric for you. At CenterWell
- 8:52Pharmacy, we see about 260 basis points better
- 8:56medication adherence than we see in other pharmacies
- 8:58in our network. And so our pharmacy is
- 9:01truly incentivized
- 9:02to to focus on adherence because we know
- 9:06there is a ton of downstream clinical impacts
- 9:10related to that. The hospitalizations
- 9:12I mentioned, the ER visits,
- 9:14those are all
- 9:15lowered, and this is well studied in the
- 9:17industry, when patients take their medicines.
- 9:21Right? And so I would say at CenterWell
- 9:23Pharmacy, we have
- 9:24robust medication adherence programs
- 9:27that are in all forms, whether it be
- 9:30reminder calls or having conversations with pharmacists around
- 9:34barriers to adherence.
- 9:36We segment the population
- 9:38on those that are high risk of falling
- 9:41out of medication adherence
- 9:43versus ones who we know may just need
- 9:45a little bit of a nudge or a
- 9:46lower cost intervention.
- 9:48So we've done a ton of research on
- 9:49this. I would say we have a ton
- 9:51of clinical interventions, and I'm really proud of
- 9:54the investments that we have made to support
- 9:56medication adherence
- 9:58because it is a win win solution when
- 10:00members take their drugs. We just see the
- 10:02better better medical outcomes
- 10:04that that result.
- 10:07Yeah. Absolutely. And you mentioned this earlier, but
- 10:10you think that employees are
- 10:12shifting their perspectives on GLP ones at least
- 10:14in the past year.
- 10:16Could you talk a bit more about why
- 10:18now is the time to deepen relationships with
- 10:20manufacturers
- 10:21offering weight loss drugs?
- 10:23Yeah. It's it's a great question.
- 10:25The health care system is under pressure to
- 10:28lower drug costs and expand access. Right? Creating
- 10:31a truly a favorable environment for an alternative
- 10:34benefit model. The system does need a new
- 10:37model,
- 10:38and both manufacturers
- 10:39and employers know that they cannot solve this
- 10:42alone.
- 10:43So the environment,
- 10:44I would say, today makes partnerships like ours
- 10:47especially valuable
- 10:49because they deliver on what the market is
- 10:52demanding right now. But just going back, I
- 10:54mean, GLP ones were definitely the catalyst, and
- 10:57here's why. I mean, one, affordability
- 11:00was and still is a top concern for
- 11:02regulators and legislators,
- 11:04and access to GLP ones
- 11:06were declining.
- 11:07Two, drug shortages at the time exploded. Right?
- 11:11And the market was flooded with compounds and
- 11:13non FDA approved alternatives,
- 11:15creating real safety risks.
- 11:18Three,
- 11:19manufacturers were looking for a way to disrupt,
- 11:22and we became that partner.
- 11:24Starting with our partnership last year with Novo
- 11:26Nordisk for Wegovy,
- 11:28and now with Eli Lilly.
- 11:30Together, we've lowered costs, helped patients avoid unsafe
- 11:33alternatives, and addressed barriers to access and affordability.
- 11:38And I wanted to hear some final thoughts
- 11:40and advice as we wrap up for other
- 11:42industry leaders. And I know
- 11:44you mentioned a lot about what CenterWell does
- 11:47really well, but what advice do you have
- 11:48also for payers that might not have a
- 11:51pharmacy component
- 11:52and what they could be thinking about right
- 11:54now?
- 11:56Yeah. If there were
- 11:57if there's one takeaway from our experience, it's
- 12:00this. The GLP one moment isn't just about
- 12:03GLP ones. It's about rethinking the entire ecosystem
- 12:08around access, access, affordability, and consumer experience.
- 12:11My advice is simple. Right? Start designing
- 12:15for the consumer first. When you build models
- 12:18that prioritize
- 12:19transparency,
- 12:20clinical quality, and predictable cost, you don't just
- 12:24improve access. You naturally bend the cost curve
- 12:27because the system becomes simpler.
- 12:31Second, leaning into partnerships. None of this none
- 12:34of us can solve the drug affordability and
- 12:36access challenge alone.
- 12:38Manufacturers,
- 12:39pharmacies,
- 12:40payers, and employers
- 12:42each control a different lever. So when those
- 12:45work together, like our collaborations with Eli Lilly
- 12:48and Novo Nordisk, you can really create access
- 12:50channels that are clinically responsible,
- 12:53financially sustainable, and scalable.
- 12:56And then just finally,
- 12:58embracing transparency and data.
- 13:00The organizations
- 13:01that win in this space will be the
- 13:03ones who can demonstrate real outcomes,
- 13:06drive medication adherence,
- 13:08and give payers and employers true line of
- 13:11sight into utilization and value.
- 13:13So that obviously requires openness,
- 13:16shared data, and a willingness to rethink long
- 13:19standing structures in the pharmacy supply chain.
- 13:22We're at a pivotal moment in health care,
- 13:24and if we design with the consumer in
- 13:26mind, commit to responsible collaboration,
- 13:29and build models that are transparent by default,
- 13:31we can expand access to transformational therapies like
- 13:35GLP ones
- 13:36while keeping the system sustainable for everyone.
- 13:40Well, thank you, Bethany, so much for joining
- 13:43us today. It was great to get a
- 13:44bit more context on your collaboration with Eli
- 13:47Lilly. And to our listeners, if you'd like
- 13:50to listen to more podcasts from Becker's HealthCare,

- 13:55Bethany, thanks again, and have a nice day.
- 13:58Thanks, Liz. You too.
