- 0:08Today's episode, Human Oversight in Agent Workflows, what it actually means to stay
- 0:12in control.
- 0:13An AI agent booked a $50,000 ad spend last month at a midsize retailer, and nobody signed
- 0:19off until the invoice landed.
- 0:21That's the story that's got everyone talking about oversight this week.
- 0:25So tell me, is human in the loop just a comforting phrase we say to sleep at night?
- 0:29Actually yes, the way people use it.
- 0:32Human in the loop, meaning a person approves each step before the agent proceeds, sounds
- 0:37reassuring until you realize a human clicking approve 40 times an hour approves everything.
- 0:43That's not oversight, that's a rubber stamp with a pulse.
- 0:47So the retailer had a human in the loop and still lost 50 grand?
- 0:50They had what I'd call a human near the loop.
- 0:53The agent could take actions and the person reviewed them after the fact, asynchronously.
- 0:58The article's point is sharp here.
- 1:01Oversight is a design decision, not a checkbox.
- 1:04Where you put the human determines whether they can actually stop anything.
- 1:08Break that down.
- 1:09Where can you put them?
- 1:11Three real spots.
- 1:12In the loop, the agent pauses and waits for a yes before every consequential action.
- 1:18On the loop, the agent runs, but a human monitors and can interrupt.
- 1:23And out of the loop.
- 1:25It's fully autonomous, you only see reports.
- 1:29Most companies think they're in the loop, and they're actually on it, or worse, out
- 1:33of it without admitting it.
- 1:35That sounds exhausting.
- 1:36If I make my agent stop and ask permission constantly, why did I automate anything?
- 1:42Exactly the tension.
- 1:43Full stop and ask kills the value.
- 1:46Full autonomy kills your risk tolerance.
- 1:48The article's answer is you tighten and loosen based on two things, one to shoot,
- 1:53reversibility and blast radius.
- 1:56Reversibility, can you undo it?
- 1:58Blast radius, how much damage if it's wrong?
- 2:01Give me the concrete version.
- 2:02An agent drafting an email.
- 2:04Reversible small blast radius.
- 2:07Let it run out of the loop.
- 2:08An agent issuing refunds.
- 2:10Reversible per case.
- 2:12But the blast radius scales fast if it loops.
- 2:15So you cap it.
- 2:16Autonomous under $100, human approval above.
- 2:20An agent deleting production data or wiring money.
- 2:23The reversible, huge blast radius.
- 2:26In the loop every time, no exceptions.
- 2:29That refund cap sounds nice on a slide.
- 2:32Does anyone actually build it that way?
- 2:34Anthropic and OpenAI both ship what they call tool use with approval gates now.
- 2:39The agent proposes an action, and the system holds it for a human on high-risk tools
- 2:44while auto-executing low-risk ones.
- 2:47Sierra, the customer service agent company, publishes that their agents escalate to humans
- 2:52on roughly a fixed set of trigger conditions.
- 2:55Refunds over a threshold.
- 2:57Angry sentiment.
- 2:59Policy edge cases.
- 3:00That's vendor framing, so take the polish with salt.
- 3:03But the mechanic is real.
- 3:05Here's my uncomfortable one.
- 3:06When the human does get pulled in, do they even have enough context to make a good call?
- 3:11Or are they just guessing?
- 3:13That's the failure everyone ignores.
- 3:15If the agent hands you approve this with no trace of why it decided that,
- 3:20you're worse than useless.
- 3:22You're laundering the machine's decision with your signature.
- 3:25Good oversight means the agent shows its reasoning and the evidence before you rule.
- 3:30Without that, the human is Liability Theater.
- 3:33Liability Theater.
- 3:34That's grim.
- 3:35It's meant to be.
- 3:37The other trap is alert fatigue.
- 3:39Flood someone with approvals, and their judgment degrades to reflex.
- 3:43The article's data point I liked.
- 3:45Teams that escalated everything saw approval quality collapse,
- 3:49while teams escalating only the genuinely ambiguous cases kept humans sharp.
- 3:54Scarcity of interruptions is a feature.
- 3:57So control isn't about watching more.
- 3:59It's about watching less, but the right things.
- 4:02Precisely.
- 4:03You're not supervising the agent.
- 4:05You're designing which decisions are worth your attention and making the rest safe to lose.
- 4:10Alright, one thing my listeners can do Monday morning, what is it?
- 4:14Take your agent workflow and list every action it can take.
- 4:17For each one, ask two questions.
- 4:20Can I undo this?
- 4:21And what's the worst case cost if it's wrong?
- 4:24Anything irreversible with real cost gets a hard human gait.
- 4:28Everything else runs free.
- 4:30That single table is the whole oversight strategy, and most teams have never written
- 4:34it down.
- 4:35That's a wrap.
