- 0:00SPEAKER_01: If you ever told anybody that you want a tummy tuck and they didn't look at you like you were absolutely out of your freaking mind, you may be a mini tummy tuck candidate.
- 0:10SPEAKER_01: Only about 1% of patients are good candidates for the mini tummy tuck.
- 0:14SPEAKER_01: Yeah, yeah, we'll do a mini on you, and they're not a mini candidate, and I hate that.
- 0:18SPEAKER_01: And I fix those a lot.
- 0:20SPEAKER_00: You do seem to fix a lot of bad tummy tucks.
- 0:23SPEAKER_00: Abdominal contouring, is it worth it?
- 0:38SPEAKER_01: Hi, everybody.
- 0:39SPEAKER_01: Dr.
- 0:40SPEAKER_01: Labraska, board certified plastic surgeon north of Pittsburgh, Pennsylvania, and New York City.
- 0:44SPEAKER_01: I'm here with my baby sister, Mystica Lebraska.
- 0:48SPEAKER_01: She is a dermatology PA of 16 years.
- 0:51SPEAKER_01: She is the master of everything.
- 0:52SPEAKER_01: Skin.
- 0:53SPEAKER_01: I'm the master of the scalpel.
- 0:54SPEAKER_01: Together we're a scalpel and skin.
- 0:56SPEAKER_01: We're siblings.
- 0:57SPEAKER_01: We're business partners, and somehow we still get along.
- 1:00SPEAKER_01: This is scalpel and skin.
- 1:02SPEAKER_01: Let's go.
- 1:03SPEAKER_01: Today we are talking about my favorite thing.
- 1:07SPEAKER_01: We're talking about abdominal contouring, specifically tummy tuck and other related procedures.
- 1:13SPEAKER_01: As you can tell, I'm a little bit hyped.
- 1:15SPEAKER_01: Let's go.
- 1:18SPEAKER_00: You could be on caffeine right now, but you're not.
- 1:22SPEAKER_00: You're you're out there on your own.
- 1:23SPEAKER_00: I'm not because you're talking about your favorite thing.
- 1:27SPEAKER_00: Tummy tuck.
- 1:29SPEAKER_01: Yeah.
- 1:29SPEAKER_01: So, you know, it is so fun.
- 1:33SPEAKER_01: Tummy Tuck is the thing that folks come from more than anything else, come from all across the country and Canada.
- 1:39SPEAKER_01: They come to little old me here in BFE, Pennsylvania, north of Pittsburgh, also now in New York City, because of my passion for the tummy.
- 1:50SPEAKER_00: What so what I want you to describe in the plainest detail ever is what is a tummy tuck?
- 2:00SPEAKER_00: Exactly.
- 2:01SPEAKER_00: What do you do?
- 2:02SPEAKER_01: Yeah, so a tummy tuck is an abdominal contouring procedure.
- 2:07SPEAKER_01: The basis of it, what I tell every single patient is so what I do is a little different than others.
- 2:13SPEAKER_01: I'm going to talk about a tummy tuck now in general, not specifically what I do.
- 2:17SPEAKER_01: You make an incision down as low as you can, typically below where a C-section would go.
- 2:23SPEAKER_01: So for those of you that have a C-section or have seen a C-section, the incision is under there, but longer.
- 2:30SPEAKER_01: And you lift the tissue up, you lift the skin and the it's called subcutaneous fat up, and you are essentially staring at the muscle, the abdominal wall muscles, and you lift everything up, and then you tighten the six-pack muscles that's called the rectus abdominis muscles, and you tighten those up because inevitably after weight loss, weight gain, and so what do you do?
- 2:54SPEAKER_00: You you you stitch them back together?
- 2:57SPEAKER_01: Tighten them up with sutures, yep.
- 2:58SPEAKER_01: Yep.
- 2:59SPEAKER_01: Tighten them up with sutures and then cut off the extra skin, and it can be a dramatic amount sometimes, uh, and then bring the belly button back out through a new hole.
- 3:11SPEAKER_01: So it pulls the removes the extra skin and is a dramatic reshaping of the abdomen.
- 3:19SPEAKER_00: So what do you, as someone who's never seen a tummy tuck, nor do I ever want to see a tummy tuck because I'll probably pass out.
- 3:28SPEAKER_01: Such a wussy.
- 3:30SPEAKER_00: Just keep me out of the OR.
- 3:32SPEAKER_00: There's a reason I do the non-invasive stuff.
- 3:35SPEAKER_00: I I don't I I don't want to see abdominal muscles being stitched together.
- 3:41SPEAKER_00: I don't want to be see the entire abdominal wall being cut off.
- 3:47SPEAKER_00: But I would like you to describe exactly how you do that because I have a hard time picturing exactly what happens when you say you're cutting off the excess skin.
- 3:58SPEAKER_00: So is there just you cut it off and there's a big bloody open abdomen?
- 4:04SPEAKER_01: Yeah, but I'm not looking like I don't see your intestines, right?
- 4:07SPEAKER_01: I don't see your liver, I don't see your stomach, I don't see any of that.
- 4:11SPEAKER_01: It's yeah, I just I I see the abdominal wall.
- 4:15SPEAKER_01: So that like the six-pack muscles, the muscles that you see in people who are in really good shape, it, but it it doesn't look like that in most of my tummy tuck patients.
- 4:25SPEAKER_01: So it's a skin excision and abdominal contouring by tightening muscle repair.
- 4:34SPEAKER_01: Now, in all of my tummy tucks, I do liposuction of the like the love handles and around the back and the upper abdomen just for shape and contour.
- 4:44SPEAKER_01: So I create curves on the side, curves in the back, and shape and definition in the front because people come all this distance uh because mine look a little different than most of the ones they see online.
- 4:58SPEAKER_01: And really, it's the lipo suction and it's for me, it's the belly buttons uh that make that set mine apart that make people come to me.
- 5:06SPEAKER_01: And that's not me bragging.
- 5:08SPEAKER_01: That's what that's why they come.
- 5:09SPEAKER_01: It's true.
- 5:10SPEAKER_00: I do your I'll do some of your post ops, you know, and and I'll say, so I'm pulling out their dreams that I can handle.
- 5:18SPEAKER_00: And they'll say I appreciate it, thank you.
- 5:20SPEAKER_00: And and I'll say, you know, how did you you you're all the way from California, you're all the way from Nevada.
- 5:26SPEAKER_00: How did you find him?
- 5:27SPEAKER_00: Did you find him on TikTok?
- 5:28SPEAKER_00: And they'll say, Yeah, he had the best belly buttons.
- 5:30SPEAKER_00: That's why I picked him.
- 5:31SPEAKER_00: They all say it.
- 5:33SPEAKER_01: Yeah, I, you know, whenever I trained in plastic surgery, uh a lot of the people I trained with, they all said, Well, you know, the the contour looks really good, but the belly buttons, you know, they just never look natural.
- 5:46SPEAKER_01: And so just whatever.
- 5:47SPEAKER_01: And and I I love a challenge, and I never accepted that as a reasonable answer.
- 5:55SPEAKER_01: So I I set about learning how to create incredible belly buttons.
- 6:02SPEAKER_01: And it's a it's been a long process, and it's not perfect every time, but I hit home runs a lot.
- 6:09SPEAKER_01: I I hit a lot of home runs and the the even the ones that aren't my favorite are would be really good for you know for for a lot of people.
- 6:19SPEAKER_01: So I'm very proud of them.
- 6:21SPEAKER_01: I work really hard at it, and you know, I probably do I haven't done one since today.
- 6:25SPEAKER_01: I'm doing one tomorrow.
- 6:27SPEAKER_01: You do them all.
- 6:29SPEAKER_00: Do you have a number on how many tummy tucks you've done?
- 6:32SPEAKER_01: I do somewhere between 150 to 200 a year, probably now.
- 6:38SPEAKER_01: Yeah, it's a lot.
- 6:38SPEAKER_01: I mean, it's it's it's three to three to five a week, you know, and I take a couple of vacations and you don't do any, you know, that week or whatever.
- 6:45SPEAKER_01: So, you know, it's a lot.
- 6:46SPEAKER_01: But I love it and it's great, and uh it's so fun, and I I I just love doing that.
- 6:52SPEAKER_01: So, you know, there are a couple of different tummy tucks that people are gonna want to talk about, and there's the everybody wants the mini tummy tuck.
- 7:00SPEAKER_01: Now, who is a patient for a mini tummy tuck?
- 7:03SPEAKER_01: If anybody has, if you ever told anybody that you want a tummy tuck and they didn't look at you like you were absolutely out of your freaking mind, you may be a mini tummy tuck candidate.
- 7:17SPEAKER_01: If anybody has ever agreed with you that you need a tummy tuck, you you're you don't need a mini, you need a full tummy tuck, at least.
- 7:24SPEAKER_01: And this is where sometimes folks get into trouble.
- 7:28SPEAKER_01: Only about 1% of patients are good candidates for the mini tummy tuck.
- 7:33SPEAKER_01: But there are plastic surgeons who don't get you know nearly endless amounts of tummy tuck patients to them.
- 7:39SPEAKER_01: So they'll be like, Yeah, yeah, we'll do a mini on you, and they're not a mini candidate.
- 7:43SPEAKER_01: So a a piss pour result was guaranteed before the surgery even started.
- 7:48SPEAKER_01: And and I hate that.
- 7:49SPEAKER_01: And I fix those a lot.
- 7:51SPEAKER_00: You do seem to fix a lot of bad tummy tucks and labocations.
- 7:56SPEAKER_00: That does seem to happen a very scary amount, right?
- 8:01SPEAKER_01: I mean, well, I've seen I talk about it a lot.
- 8:04SPEAKER_01: Yeah, it's I listen.
- 8:05SPEAKER_01: So I I just saw two weeks or so ago a woman who lives in uh another state.
- 8:12SPEAKER_01: I won't tell you which one, but she lives in another state, and I saw her like right here for a virtual consult.
- 8:18SPEAKER_01: And she said, I had a Lipo 360 three years ago, and I was like, Oh, okay, well, uh, why did why did you have a Lipo 360?
- 8:29SPEAKER_01: Well, they told me that the skin would tighten and I hadn't seen her tummy yet.
- 8:34SPEAKER_01: And I said, Wait, you have loose skin?
- 8:36SPEAKER_01: She's like, Yeah, they told me that the skin would tighten and I was fully awake for it.
- 8:41SPEAKER_01: And I went, You were fully awake for a lipo 360.
- 8:44SPEAKER_01: She said, uh, I before I went further, I said, Was your surgeon a plastic surgeon or a cosmetic surgeon?
- 8:51SPEAKER_01: And she said, Well, I thought it was a plastic surgeon, but it turns out it was a cosmetic surgeon.
- 8:58SPEAKER_01: I went, Okay, this is starting to make sense.
- 9:01SPEAKER_01: And and she said, It was so painful, I had to quit part way through.
- 9:05SPEAKER_00: Why would anyone I mean that sounds so horrible?
- 9:09SPEAKER_00: Yeah, absolutely horrible.
- 9:12SPEAKER_01: The poor woman.
- 9:13SPEAKER_01: Yes.
- 9:14SPEAKER_01: Yep.
- 9:15SPEAKER_01: This woman paid this guy to torture her.
- 9:20SPEAKER_00: And and Libo 360 is when you do the back, the ab, you do the legs too.
- 9:26SPEAKER_01: Yep.
- 9:27SPEAKER_01: Uh no, it typically circumferential trunk.
- 9:30SPEAKER_01: So flanks, up maybe to like the um the scapula, which is like the the bone in the back that's connected to your shoulder there.
- 9:40SPEAKER_01: Uh, and then the and the abdomen in the front, all the way down to the mons pubis area, which would be like the kind of like the fupa area.
- 9:47SPEAKER_01: Um, so all of that.
- 9:48SPEAKER_01: That's your Lipo 360.
- 9:50SPEAKER_01: And so this woman, she got as far as the lower abdomen in the front, and that was it.
- 9:55SPEAKER_01: She had to quit.
- 9:56SPEAKER_00: Oh my God.
- 9:57SPEAKER_00: The poor woman.
- 9:58SPEAKER_01: Yeah, right.
- 9:59SPEAKER_01: And so I saw it and I was absolutely horrified.
- 10:02SPEAKER_01: Uh it was exactly what I expected.
- 10:04SPEAKER_01: Uh, well, she first of all, she had hundreds of stretch marks.
- 10:08SPEAKER_01: And so when you see stretch marks, you think, no, this person needs a tummy tuck.
- 10:12SPEAKER_01: And so that person, the the cosmetic surgeon knew he was doing the wrong surgery on the wrong patient, but did it anyway.
- 10:20SPEAKER_01: And and that happens so much more than than I care to admit.
- 10:24SPEAKER_01: And it drives me up an absolute wall.
- 10:27SPEAKER_00: He she was she was an ATM machine to that, to that.
- 10:32SPEAKER_01: I mean, that's exactly right.
- 10:33SPEAKER_01: So unethical.
- 10:34SPEAKER_01: That person, that yeah, that person just didn't care.
- 10:38SPEAKER_01: Uh, just was gonna say yes to whatever and really screwed her over.
- 10:42SPEAKER_00: So when you, if they're a tummy tuck candidate, but you do something like that guided and do lipo instead and just keep your fingers crossed and hope for the best.
- 10:53SPEAKER_00: Can you actually make the loose skin worse because you're removing the fat and the support?
- 10:58SPEAKER_01: Oh, yeah, it's it's way worse.
- 11:00SPEAKER_01: Oh, it's absolutely worse.
- 11:01SPEAKER_01: Because uh, the if you have stretch mark skin, that means that it has gone beyond its capacity to tighten again.
- 11:10SPEAKER_01: Right.
- 11:10SPEAKER_01: And and so it's it's it's not gonna bounce back.
- 11:13SPEAKER_01: It's it's never gonna bounce back.
- 11:15SPEAKER_01: And it's it's it's a poor result, is is guaranteed 100% of the time in that particular patient.
- 11:23SPEAKER_01: And and it sucks.
- 11:24SPEAKER_01: It's it's terrible.
- 11:25SPEAKER_01: I hope she comes to me because I can't wait.
- 11:28SPEAKER_01: I love a challenge.
- 11:29SPEAKER_01: Uh I love a challenge and I'm excited.
- 11:31SPEAKER_01: I hope she gives me the opportunity to fix her.
- 11:33SPEAKER_01: You know, I had another one from another, like the DC area.
- 11:38SPEAKER_01: There's a very well-known, very good plastic surgeon there who, in this particular patient, had a little bit of a decent amount of what we call necrosis, meaning some of the skin died uh under the belly button and um after a tummy tuck and do uh like a skin graft.
- 11:57SPEAKER_01: So that happens if you do enough tummy tucks.
- 12:00SPEAKER_01: It's happened to everybody.
- 12:01SPEAKER_01: I've been blessed that I haven't had anything quite like that happen before.
- 12:05SPEAKER_01: I thought this patient was gonna come to me and she may be on my books, but uh, but I'm really, I really hope to fix her because uh I think I can give her a really, really nice result.
- 12:16SPEAKER_00: And uh so this is this is the one that had the necrosis?
- 12:20SPEAKER_00: Is that what you mean?
- 12:21SPEAKER_01: Yeah, the one that had the necrosis.
- 12:23SPEAKER_00: She already had a tummy tuck.
- 12:24SPEAKER_00: So what will you do?
- 12:25SPEAKER_00: She has some scarring, I assume.
- 12:27SPEAKER_01: Um I'm gonna rotate some skin in kind of the the way if if you were to kind of imagine it, imagine like a pizza slice where the crust is like where her pubic hair is, and the point of the pizza is at her belly button.
- 12:45SPEAKER_01: So that's where the necrosis is kind of like that.
- 12:48SPEAKER_01: And I think I can cut that out and bring it in from the sides, and so she'll have kind of like a like a T incision, but there, but there won't be you know any necrotic skin left.
- 13:00SPEAKER_01: And I I think part of her belly button is there, and I think I can make her a decent belly button from it.
- 13:05SPEAKER_01: I'd love to do that stuff like that.
- 13:06SPEAKER_01: I hate that I have to do it, but I love that I can and I I love a challenge.
- 13:11SPEAKER_00: Is is the is she she's healed though, right?
- 13:15SPEAKER_01: She's healed.
- 13:15SPEAKER_01: It was a few years ago, yeah.
- 13:17SPEAKER_01: So we talked about mini tummy tuck and then full tummy tuck is the one that most people get, either a full tummy tuck or an extended tummy tuck.
- 13:25SPEAKER_01: If the kind of extra skin kind of hangs off to the side when they're just in their underwear, we do an extended tummy tuck, and the incision can go like back uh almost to like if you're laying down, uh almost to where the the bed or whatever touches your skin that far out.
- 13:43SPEAKER_01: So it's it's pretty long.
- 13:44SPEAKER_01: Uh, but beautiful, beautiful results, especially with the liposuction that I do, uh, really gives curves and shape and contour.
- 13:52SPEAKER_01: And I just did one of those today.
- 13:53SPEAKER_01: I think I have another extended tomorrow.
- 13:56SPEAKER_01: Yeah, love it.
- 13:57SPEAKER_00: Can the fat come back after you've done a tummy tuck?
- 14:00SPEAKER_01: Sure.
- 14:00SPEAKER_01: So we have the same amount of fat uh we had when we were born, the same amount of fat cells, but they get bigger and smaller as we gain and lose weight.
- 14:10SPEAKER_01: So fat that's gone is gone.
- 14:13SPEAKER_01: Uh fat that's been lipoed out is gone.
- 14:16SPEAKER_01: But you know, can you out-eat a tummy tuck?
- 14:18SPEAKER_01: Yeah, you absolutely can.
- 14:20SPEAKER_01: So please don't do that.
- 14:21SPEAKER_00: Do that.
- 14:22SPEAKER_00: Yeah, that would really be a that would be a bummer.
- 14:25SPEAKER_00: Because tummy tucks look amazing.
- 14:27SPEAKER_01: The another tummy, there's a couple other tummy tucks we want to touch on.
- 14:30SPEAKER_01: Uh, one is called a fleur-de-lis or an FDL tummy tuck.
- 14:34SPEAKER_01: So that's for patients who have lost massive amounts of weight and they have lots of extra skin kind of east-west or side to side in addition to north-south, which is up and down.
- 14:46SPEAKER_01: And so they have the typical tummy tuck incision, but they also have an incision kind of in the front, in the middle.
- 14:52SPEAKER_01: Most people don't want that, but if you've lost 150, 200, 300 pounds, you need it.
- 14:58SPEAKER_01: And it is what it is.
- 14:59SPEAKER_01: And I do a ton of those.
- 15:01SPEAKER_01: I did one on Friday, it came out spectacular.
- 15:03SPEAKER_01: Today's Tuesday, and uh, and I love it.
- 15:06SPEAKER_01: It came out great.
- 15:07SPEAKER_01: And and with the muscle tightening and the liposuction, she has a waist for the first time in her adult life.
- 15:13SPEAKER_00: Aw, she's gonna be so happy.
- 15:15SPEAKER_00: I mean, those are life-changing surgeries.
- 15:20SPEAKER_01: When it's something you might see her this week.
- 15:21SPEAKER_01: I'm not sure where she's if she's coming to you or me for the post operation.
- 15:25SPEAKER_00: Probably you, because you'll be in the office.
- 15:27SPEAKER_01: Oh, that's true.
- 15:27SPEAKER_01: Yeah, yeah, she'd come to me.
- 15:29SPEAKER_00: Yeah, I mean, when it's when it's weight loss related, excess skin like that, hundreds of pounds.
- 15:34SPEAKER_00: I mean, that's that's life-changing for her.
- 15:37SPEAKER_01: Yep, it sure is.
- 15:38SPEAKER_01: And then the other one would be a circumferential tummy tuck, which goes all the way around.
- 15:42SPEAKER_01: I don't love to do that in one stage because you have to cut corners, you can't do as much lipa suction.
- 15:50SPEAKER_01: And the there are wound complications that come with that a fair amount, at least in my hands, that maybe there are some people who are able to do it without any any more negative effects than a regular tummy tuck, but I uh I just feel like the it's better to do it in two stages than it is to do like a butt lift on somebody three or six months down the road after a tummy tuck to get the best overall result.
- 16:14SPEAKER_00: What's the what's the recovery like?
- 16:16SPEAKER_00: I mean, it's gotta be pretty.
- 16:18SPEAKER_01: Yeah, the f what I typically tell folks is the first week sucks.
- 16:22SPEAKER_01: The second week sucks less.
- 16:24SPEAKER_01: Uh, and after that, it's okay.
- 16:26SPEAKER_01: After about two weeks, most of my patients are feeling human.
- 16:30SPEAKER_01: They're not ready for primetime yet.
- 16:32SPEAKER_01: But like teachers can go back to work and uh, you know, they probably watch movies with their kids, and and uh there's some quiet time that first week, and they're not probably their best teaching ever, but they can go back to work.
- 16:45SPEAKER_01: Folks who do more aggressive work, they may be out up to six weeks.
- 16:49SPEAKER_01: Nurses are somewhere in the two to four week range, depending on what kind of nursing they do.
- 16:54SPEAKER_00: Um they're just kind of sore after those two weeks, right?
- 16:57SPEAKER_00: Just after the first couple weeks.
- 16:59SPEAKER_01: Yeah, that that first week, it it's pretty rough.
- 17:02SPEAKER_01: There, you know, there are people who say they do a painless tummy tuck, and I can tell you that they that they don't do the same tummy tuck that I do.
- 17:09SPEAKER_00: They do less to not have any pain.
- 17:13SPEAKER_00: You do the pain pumps, right?
- 17:15SPEAKER_01: You Yep, yep.
- 17:16SPEAKER_01: Yeah, I do all the I do pain pump and everything.
- 17:18SPEAKER_01: So, and I uh so all of my tummy tucks include some type of muscle repair, some type of Mons or upper pubic lift, uh, some type of three-day non-narcotic pain release, either expirel shot or the on cue pain ball.
- 17:36SPEAKER_01: They both work excellently.
- 17:38SPEAKER_01: And everybody gets liposuction.
- 17:40SPEAKER_01: And that's just because I don't upcharge my patients.
- 17:43SPEAKER_01: Uh, I the full gamut on everybody because I want everybody to get the best result.
- 17:48SPEAKER_01: I I don't upcharge people.
- 17:50SPEAKER_01: I think it's stupid.
- 17:51SPEAKER_01: There are people that upcharge for a muscle repair.
- 17:53SPEAKER_01: It's absolute bullshit.
- 17:54SPEAKER_01: It's part of a tummy tuck.
- 17:55SPEAKER_01: Don't charge extra for it.
- 17:57SPEAKER_01: Just do it.
- 17:58SPEAKER_00: Yeah, like I don't get why people do stuff like that.
- 18:00SPEAKER_00: I mean, you you you people because patients will not, they will choose to not do the muscle repair if they have to pay more for it.
- 18:09SPEAKER_00: And then their result isn't as good.
- 18:11SPEAKER_00: So just make it part of the the treatment package.
- 18:16SPEAKER_00: You want your patients to be thrilled.
- 18:18SPEAKER_00: You don't want them to be less than thrilled.
- 18:20SPEAKER_01: So let's talk a little bit about muscle repair.
- 18:23SPEAKER_01: So I do a muscle repair in all of my patients.
- 18:25SPEAKER_01: It's called diastasis recti or separation of the rectus muscles.
- 18:30SPEAKER_01: It's not a hernia and insurance doesn't cover it, but it is a separation of the abdominal uh muscles.
- 18:36SPEAKER_01: Um, it comes with weight loss and weight gain and pregnancy and and everything.
- 18:43SPEAKER_01: And the muscles, they just don't bounce back the way that they once prior to kids and and weight loss and weight gain and everything as well.
- 18:53SPEAKER_01: Um, and so I do what's called a two-layer muscle repair.
- 18:56SPEAKER_01: I used to do a one-layer muscle repair, and then I found out I read a a uh journal article that said that 40% of the time it comes back if you don't do absorb, if you don't do permanent sutures.
- 19:08SPEAKER_01: So I do both absorbable and not absorbable and permanent sutures.
- 19:13SPEAKER_01: And my patients, their tummies are freaking flat.
- 19:15SPEAKER_01: So they're adjustable.
- 19:16SPEAKER_00: The sutures really stay in there for life?
- 19:19SPEAKER_01: Yeah.
- 19:20SPEAKER_00: Wow, what's it made out of?
- 19:21SPEAKER_01: It's called a Tychron or Ethabond.
- 19:24SPEAKER_01: It is, it's just a it's a permanent suture.
- 19:26SPEAKER_01: It's a permanent braided Morgan sell that?
- 19:29SPEAKER_00: Morgan?
- 19:29SPEAKER_00: Does she sell is that her product?
- 19:31SPEAKER_01: She sells ethabond, yes.
- 19:33SPEAKER_00: Hi Morgan.
- 19:34SPEAKER_00: Shout out to Morgan.
- 19:37SPEAKER_00: We love her.
- 19:38SPEAKER_01: She's engaged.
- 19:39SPEAKER_00: Yes, she is.
- 19:40SPEAKER_00: Yes, she is.
- 19:40SPEAKER_00: She's engaged.
- 19:41SPEAKER_00: She's engaged.
- 19:42SPEAKER_00: Her dog, her adorable dog, Hank, is going to have a a dad, and we're very happy for Morgan.
- 19:49SPEAKER_00: Morgan, for everyone listening, all eight of you, hopefully, Morgan's one of them.
- 19:55SPEAKER_00: Morgan is his suture rep, and and I've gotten to know her over the years, and she's wonderful.
- 20:01SPEAKER_01: She brings Hank into the office sometimes.
- 20:04SPEAKER_01: So diastasis erecti is not a hernia.
- 20:06SPEAKER_01: Sometimes we see hernias while we're in a tummy tuck.
- 20:10SPEAKER_01: I just fix them.
- 20:11SPEAKER_01: I don't make a big deal out of it.
- 20:13SPEAKER_01: Um, if I found a very large hernia, I might get a general surgeon to come help.
- 20:18SPEAKER_01: But I was a general surgeon prior to being a plastic surgeon, so I typically handle it.
- 20:22SPEAKER_00: And a hernia, for those listening, is when part of the bowel comes through the muscle.
- 20:29SPEAKER_01: Well, it's not always bowel, it's more likely to be the fat.
- 20:34SPEAKER_00: Wait, peritoneum.
- 20:35SPEAKER_00: Wait, come on.
- 20:37SPEAKER_00: I am a PA, I have a medical degree, I know anatomy.
- 20:41SPEAKER_00: Is it momentum?
- 20:44SPEAKER_01: It is momentum.
- 20:46SPEAKER_00: But there's also peritoneum.
- 20:49SPEAKER_01: Sometimes bowel is true.
- 20:51SPEAKER_01: It's super rare.
- 20:52SPEAKER_01: Peritoneum is it's a it's a medical student layer.
- 20:57SPEAKER_01: Okay.
- 20:58SPEAKER_01: So I mean, it exists.
- 21:01SPEAKER_01: And you know, some people even try to say they repair it uh uh during a uh a bowel surgery or whatever.
- 21:08SPEAKER_01: Most people don't even do that, they just point to it and then they repair the more important layers.
- 21:13SPEAKER_01: Yeah, so that we talked a little bit about lipo 360, what that is.
- 21:17SPEAKER_01: And so I don't call what I do a lipo360, because typically a lipo360 includes removal, uh liposuction of the fat in the lower abdomen as well.
- 21:30SPEAKER_01: But I cut that tissue out.
- 21:32SPEAKER_01: Yeah, I cut it out.
- 21:33SPEAKER_01: So why would I liposuction something that I'm gonna then remove five minutes later?
- 21:37SPEAKER_01: So let's see here.
- 21:38SPEAKER_01: We talked about belly buttons a little bit, and we I did talk about why people come to me and and what it is is I use a get a little nerdy here.
- 21:48SPEAKER_01: Um, most plastic surgeons use one specific shape of the belly button.
- 21:54SPEAKER_01: And I use one that's completely different than everybody else.
- 21:58SPEAKER_01: And um, And I think that's why mine's a little bit more reproducible for me.
- 22:04SPEAKER_00: What do you mean by what do you mean by shape?
- 22:06SPEAKER_01: What we do is when we're lifting the belly, the skin off, we cut around the belly button, uh, and the belly button is kind of left in place temporarily while we lift all the skin away around it.
- 22:21SPEAKER_01: Then we do the muscle repair and all the other um things inside, and then we cut off the extra skin.
- 22:28SPEAKER_01: And the kind of the last thing is we bring the belly button out through a new hole.
- 22:33SPEAKER_01: So that kind of new hole is I make a different shape than most everybody else.
- 22:39SPEAKER_01: And uh it's worked out well for me.
- 22:41SPEAKER_00: Yeah, they look like real belly buttons.
- 22:44SPEAKER_01: They don't have that Well, they are real belly buttons.
- 22:46SPEAKER_00: They're just they don't have that that tummy tuck belly button look like you see on social media.
- 22:52SPEAKER_00: They look like Thank you.
- 22:53SPEAKER_01: You're very kind.
- 22:54SPEAKER_01: I appreciate it.
- 22:55SPEAKER_00: They look like they were they look like they were there since birth.
- 22:59unknown: Oh, good.
- 22:59SPEAKER_00: Thanks.
- 23:00SPEAKER_00: They were they were sculpted by Dr.
- 23:03SPEAKER_00: Algie Labrasca, board certified plastic surgeon.
- 23:06SPEAKER_01: You're my favorite real sister that I have.
- 23:09SPEAKER_01: I uh I I do have a we we do have a uh a stepsister, Meg, who is super pregnant at the time of this recording.
- 23:19SPEAKER_00: She's about she's do she's having three weeks.
- 23:23SPEAKER_00: She's having a C-section on September 12th, I think.
- 23:27SPEAKER_00: So we're September 13th.
- 23:29SPEAKER_01: By the time this comes out, yeah, yeah, the 13th.
- 23:31SPEAKER_01: By the time this comes out, she may, we we may have a new niece.
- 23:35SPEAKER_00: Oh my god, a baby.
- 23:37SPEAKER_01: My first niece.
- 23:38SPEAKER_00: Yes, yes, it is.
- 23:39SPEAKER_01: She's just and you have an you have a niece, my daughter.
- 23:43SPEAKER_01: Um, so that'll be my first niece, and I think that's great.
- 23:47SPEAKER_00: I I'm gonna steal that baby and the and just smell her head for like three hours.
- 23:56SPEAKER_01: You're so weird.
- 23:57SPEAKER_01: No, I don't know.
- 23:58SPEAKER_01: But you're right.
- 23:58SPEAKER_01: Yeah, right.
- 23:59SPEAKER_01: New baby smell is the coolest thing.
- 24:01SPEAKER_00: Every, every mom out there knows what I'm talking about.
- 24:06SPEAKER_00: I know what you're talking about.
- 24:07SPEAKER_00: Like, I wanna just smell the baby's head like a total creep.
- 24:12SPEAKER_00: I'm gonna be a total creep and smell her head for hours.
- 24:15SPEAKER_00: Yes, I am.
- 24:16SPEAKER_00: I'm probably doing it right now while I listen to this episode.
- 24:21SPEAKER_01: Great.
- 24:22SPEAKER_01: So, yeah, so we have uh we're gonna have a new um new member to the family here shortly, uh, probably by the time this comes out.
- 24:30SPEAKER_01: And uh, and that's great.
- 24:32SPEAKER_01: So we've talked about what's good.
- 24:34SPEAKER_01: Let's talk about what's bad.
- 24:36SPEAKER_01: Cool sculpting.
- 24:37SPEAKER_00: Oh, we're going, we're talking about not oh, so we're the topic is abdominal contouring.
- 24:43SPEAKER_00: I gotcha.
- 24:44SPEAKER_01: Yeah.
- 24:44SPEAKER_01: So okay.
- 24:46SPEAKER_01: Cool sculpting.
- 24:47SPEAKER_00: Unpopular opinion.
- 24:49SPEAKER_00: I happen to really like my cool sculpting results.
- 24:53SPEAKER_00: I got it a couple of years ago.
- 24:55SPEAKER_00: I I knew the reps, they were I love them.
- 24:59SPEAKER_00: They're they're not with the company anymore, but they were very kind to come into Skinlab and just let me try it out.
- 25:06SPEAKER_00: And I really got great results, but I was also a very good candidate for for cold sculpting.
- 25:15SPEAKER_00: So, you know, you can't, it's it's not a good option for people who just have a lot of fat and and and they need and they have more loose skin.
- 25:27SPEAKER_00: I just kind of had this little bit of a little bit of abdominal fat, just a little bit from from having a couple kids, and I freaking love my results.
- 25:39SPEAKER_01: The problem with folks, uh, most folks who do cool sculpting is they can't do surgery.
- 25:47SPEAKER_01: So, you know, when when all you have is a hammer, everything is a nail.
- 25:52SPEAKER_01: So they throw cool sculpting at folks who are not good candidates because they paid, you know, 150 or 200,000 for a cool sculpting machine that has limited use, and you get about 20 to 30 percent improvement if you're super lucky.
- 26:11SPEAKER_01: And in general, it leaves most folks unhappy.
- 26:15SPEAKER_01: It's one of the main things that I would never do uh on a patient in the, you know, for their uh abdomen.
- 26:23SPEAKER_00: There's just there's just such a small subset of people who could actually benefit from cool sculpting.
- 26:32SPEAKER_00: I I fully recognize that, and I happen to be one of them.
- 26:34SPEAKER_00: So I got really good results.
- 26:36SPEAKER_00: It it did what it promised to do for me.
- 26:39SPEAKER_00: But you know, there are what do you say to some plastic surgeons do have cool sculpting to devices.
- 26:46SPEAKER_00: I one of Skin Lab's employees used to work for a plastic surgeon, and that surgeon had two.
- 26:53SPEAKER_00: I mean, what what do you think the benefit of her having cool sculpting available to her patients is?
- 26:59SPEAKER_00: I mean, why does she have it?
- 27:00SPEAKER_01: Sure.
- 27:01SPEAKER_01: So, you know, very early in my career, I talked to a guy who had one, and I said, you know, what do you think about it?
- 27:08SPEAKER_01: Because I I was kind of unsure at the time.
- 27:10SPEAKER_01: He said, Well, it's great because people call the office and ask for it, and then they come in for a consult and I tell them they're not a candidate, they're a tummy-tuck candidate instead.
- 27:19SPEAKER_00: I've heard this.
- 27:20SPEAKER_01: I mean, to get to get more tummy tuck consults would probably be the the main reason to do it.
- 27:26SPEAKER_01: Why somebody would get to, I I can't answer that.
- 27:29SPEAKER_01: That uh, I mean, that probably means that that that surgeon uses it, you know, or it's I I don't know.
- 27:36SPEAKER_01: I just it's it's not really something that's in my wheelhouse.
- 27:39SPEAKER_01: It's not something uh I always have a solution for it that for something like that that's gonna give you a better result.
- 27:47SPEAKER_00: I I have heard that before.
- 27:49SPEAKER_00: That I I can't remember where I heard this, but I I may it might have been from a rep from some company, but plastic surgeons will buy one cool sculpting and put it in their closet because it will it will bring in new consults because people Google it, who has cool sculpting in my area.
- 28:07SPEAKER_00: The they make a consult appointment and then it ends up being Lipo or a Tummy Chuck.
- 28:12SPEAKER_01: Absolutely.
- 28:12SPEAKER_01: You know, there's some other ones that came out uh over the last few years as well, the EM sculpt or M sculpt.
- 28:20SPEAKER_01: That was that's a Lutronic.
- 28:21SPEAKER_00: Is that a loot?
- 28:22SPEAKER_00: Who makes that?
- 28:22SPEAKER_00: Lutronic?
- 28:24SPEAKER_01: No, uh no, uh-uh, somebody else.
- 28:26SPEAKER_01: I can't remember.
- 28:27SPEAKER_01: I tried it once.
- 28:28SPEAKER_01: So it was one that said that you know it's like 10,000 sit-ups in 30 minutes.
- 28:32SPEAKER_01: I mean, I it was fine for muscle, you know, I felt it in my abs for a couple days afterwards.
- 28:39SPEAKER_01: It it did that, but it didn't really remove any fat.
- 28:44SPEAKER_01: Right.
- 28:44SPEAKER_01: It didn't remove any fat.
- 28:46SPEAKER_01: And, you know, if you don't do it every month or a couple of weeks or whatever, any muscle that you gain is gonna go away as well.
- 28:54SPEAKER_00: So those muscle stimulating devices are stupid.
- 28:59SPEAKER_00: I mean, I'm not gonna say it.
- 29:00SPEAKER_00: That's stupid.
- 29:01SPEAKER_01: Someone's gonna charge you thousands of dollars to you need one in your house and you need to do it every week.
- 29:08SPEAKER_01: Right.
- 29:08SPEAKER_01: Like you need it.
- 29:09SPEAKER_01: Please don't spend your money on that.
- 29:11SPEAKER_01: Uh agree.
- 29:12SPEAKER_01: Right.
- 29:12SPEAKER_01: And that's so one of the things when they realized that it was too expensive and the results didn't last long, they they started recommending you do like a like a membership, like a monthly membership to go in and get it every month or whatever.
- 29:25SPEAKER_01: Come on, people.
- 29:26SPEAKER_00: I mean, that's the that's the stupid shit in the industry.
- 29:29SPEAKER_00: I just can't stand.
- 29:30SPEAKER_00: Right.
- 29:31SPEAKER_00: Yeah, it's the stuff that we hate.
- 29:32SPEAKER_00: You we hate it.
- 29:34SPEAKER_00: You bought a stupid device and now you're trying to make patients pay for it.
- 29:38SPEAKER_00: It's that's that's dumb.
- 29:40SPEAKER_01: Right.
- 29:41SPEAKER_01: You know, that's it.
- 29:42SPEAKER_01: That's probably the main reason I'm a late adopter of a lot of things because a lot of things turn out to be shit.
- 29:49SPEAKER_01: They turn out to be absolute butt.
- 29:52SPEAKER_01: And uh, and I hate doing that.
- 29:54SPEAKER_01: I I've I've made a few mistakes in my life and I and I've eaten them.
- 29:58SPEAKER_01: And I try not to make any more, you know, hundred thousand dollar mistakes for something that you know promises to be something, and we see it every year.
- 30:08SPEAKER_01: There's two or three devices that the next greatest thing ever.
- 30:12SPEAKER_01: And then a year down the road they're gone.
- 30:14SPEAKER_00: Right.
- 30:14SPEAKER_00: There's there's a couple I can think of right now.
- 30:17SPEAKER_00: I I'm not, I yes, no comment.
- 30:21SPEAKER_00: No, no comment because I do like that company.
- 30:24SPEAKER_00: We'll see what happens.
- 30:25SPEAKER_00: We'll see what happens.
- 30:26SPEAKER_01: Yeah, yeah.
- 30:27SPEAKER_01: A couple other uh do you do any non-surgical skin tightening?
- 30:31SPEAKER_00: I mean, I why so I do okay, I got a story.
- 30:35SPEAKER_00: So I really believe in RF microneedling.
- 30:41SPEAKER_00: We have the sinus or electronic genius for the face.
- 30:45SPEAKER_00: I love my results, my genius results.
- 30:48SPEAKER_00: People get beautiful results, they get a little bit of tightening, and I'm saying tightening, not lifting.
- 30:55SPEAKER_00: The skin does tighten, doesn't lift, we're not lifting, we're tightening.
- 30:59SPEAKER_00: But I had a patient come in.
- 31:02SPEAKER_00: This was not even two weeks ago.
- 31:05SPEAKER_00: And she came in because she wanted genius, she wanted RF microneedling for her for her stomach.
- 31:14SPEAKER_00: She just had she had some loose skin kind of around her belly button.
- 31:18SPEAKER_00: She was in her 40s, she's had multiple kids.
- 31:21SPEAKER_00: And she came in specifically for that.
- 31:24SPEAKER_00: And I was like, look, I don't want your money.
- 31:28SPEAKER_00: I I have this device, or I could do this device, I don't think that you would be happy.
- 31:35SPEAKER_00: You're gonna, you know, you're you're gonna spend thousands of dollars for a very modest benefit, if any.
- 31:43SPEAKER_00: And if you really want to do it and you really want to try it, you really feel that strongly that you're not gonna do anything else.
- 31:51SPEAKER_00: Cause she was kind of like, I'll never do surgery.
- 31:54SPEAKER_00: So this is the only thing I'm gonna do.
- 31:56SPEAKER_00: And I was like, I'll do it, but I'm telling you right now, I don't think you should do it.
- 32:03SPEAKER_00: And she was like, Okay, you know what?
- 32:06SPEAKER_00: Thanks.
- 32:07SPEAKER_00: Thanks for your honesty.
- 32:08SPEAKER_00: I really appreciate that.
- 32:11SPEAKER_00: And I guess I'll make a consult with your brother.
- 32:14SPEAKER_01: Oh, good, thanks.
- 32:15SPEAKER_00: So she I said it it's at least worth a conversation.
- 32:19SPEAKER_00: You've never met with a plastic surgeon, at least hear what he has to say.
- 32:24SPEAKER_00: You know, you it it might not be surgery, it might not be what you're envisioning in your head.
- 32:29SPEAKER_00: You should have that conversation.
- 32:32SPEAKER_00: But if you're telling me there's zero chance you'll ever do it, and and if you do nothing else, it's just gonna be mic RF microneedling.
- 32:42SPEAKER_00: But we'll try it.
- 32:43SPEAKER_00: I I mean, I I don't think you're gonna be happy though.
- 32:46SPEAKER_00: Just I I was basically telling her, like, please don't do it.
- 32:49SPEAKER_00: Because if someone isn't going to be thrilled with their results, I would prefer they don't do it.
- 32:55SPEAKER_00: It like you always say, it has to be high yield.
- 32:58SPEAKER_00: We I I want I don't I say to patients if you aren't if you feel like your results are less than perfect, I want a phone call.
- 33:07SPEAKER_00: I don't want you walking around feeling like you wasted money or feeling like you made a mistake.
- 33:13SPEAKER_00: Call me and and let's get you to as close to perfect as we can.
- 33:19SPEAKER_00: And and it wasn't gonna be perfect.
- 33:21SPEAKER_00: I there's it just wasn't going to be.
- 33:23SPEAKER_00: So I I think she might end up seeing you.
- 33:25SPEAKER_00: I don't know when that's going to happen, but she she's she sounded like she might at least make an appointment for a consult.
- 33:32SPEAKER_00: So I she decided against genius, which I'm glad she made the right decision.
- 33:38SPEAKER_01: Good.
- 33:39SPEAKER_01: Good.
- 33:39SPEAKER_01: She did make the right decision.
- 33:40SPEAKER_00: So to answer your question, no, not for body, right?
- 33:46SPEAKER_01: Very good.
- 33:46SPEAKER_01: Okay, so these are questions folks have asked me.
- 33:49SPEAKER_01: We we're gonna call these the social media rapid fire questions.
- 33:52SPEAKER_01: Mini tummy tuck gives full tummy tuck results.
- 33:55SPEAKER_01: Absolutely never.
- 33:57SPEAKER_01: Drainless tummy tuck, automatically better.
- 34:00SPEAKER_01: No.
- 34:01SPEAKER_00: If you don't do any shit, do you ever not put a drain in?
- 34:04SPEAKER_00: You always put a drain in.
- 34:05SPEAKER_01: No, and why?
- 34:06SPEAKER_01: Because I do a lot of liposuction on people.
- 34:09SPEAKER_01: And in my opinion, with the amount of liposuction that I do, you need drains because a lot of fluid goes in.
- 34:14SPEAKER_01: That fluid has to come out.
- 34:16SPEAKER_01: So I love drains.
- 34:17SPEAKER_01: I think they're great for tummy tucks.
- 34:19SPEAKER_01: Lipo 360 creates an hourglass figure on everybody.
- 34:24SPEAKER_01: No, it doesn't.
- 34:25SPEAKER_01: Um, and but a lipo360 and a tummy tuck is gonna give you the best result in the right, if you're the right candidate for it, it's gonna give you the best overall hourglass shape.
- 34:37SPEAKER_01: Cool sculpting is non-surgical liposuction.
- 34:40SPEAKER_01: No, no, it's not.
- 34:42SPEAKER_01: Muscle sculpting gives everybody abs.
- 34:45SPEAKER_01: No, it absolutely doesn't.
- 34:46SPEAKER_01: There is no absolute everybody.
- 34:48SPEAKER_00: Wait, can we just cancel it?
- 34:50SPEAKER_00: Cancel, cancel the muscle sculpting devices, please, guys.
- 34:54SPEAKER_01: Here's one tummy tuck is a weight loss surgery.
- 34:58SPEAKER_01: No, it is absolutely not a weight loss surgery.
- 35:01SPEAKER_01: It is abdominal contouring surgery.
- 35:03SPEAKER_01: Lipo treats visceral fat.
- 35:05SPEAKER_01: So that's fat on the inside of the abdominal wall.
- 35:08SPEAKER_01: No, it absolutely doesn't.
- 35:10SPEAKER_01: It absolutely doesn't.
- 35:11SPEAKER_00: Abdominal contouring, is it worth it?
- 35:14SPEAKER_01: It is absolutely worth it.
- 35:16SPEAKER_01: Only if you treat it, if you treat the right thing.
- 35:20SPEAKER_01: If what I always say is treat the right patient at the right time with the right procedure.
- 35:26SPEAKER_01: And if you do that, you will get the best possible results for each patient.
- 35:31SPEAKER_00: And the best belly buttons if you come to Dr.
- 35:35SPEAKER_00: Lebraska.
- 35:36SPEAKER_00: Dr.
- 35:36SPEAKER_00: Lebraska, look at you hyping me.
- 35:38SPEAKER_00: Oh, there you go.
- 35:39SPEAKER_00: Yeah.
- 35:40SPEAKER_00: Go ahead and say it again.
- 35:41SPEAKER_00: www.drlabrasca.com.
- 35:45SPEAKER_01: That's my that's my favorite sister right there.
- 35:49SPEAKER_00: We love you too, Meg.
- 35:51SPEAKER_01: Well, yes.
- 35:52SPEAKER_01: Right.
- 35:52SPEAKER_01: Well, we talked about Meg.
- 35:53SPEAKER_01: Oh, you know what?
- 35:54SPEAKER_01: I had, I forgot to mention, I had a really nice weekend.
- 35:58SPEAKER_01: So um, dad and Pam.
- 36:00SPEAKER_01: Uh Pam is uh dad's wife, Meg's mom.
- 36:05SPEAKER_01: We have uh Amy and I, my wife, uh, we have a uh a ski house in Ellicottville, New York.
- 36:11SPEAKER_01: And they wanted to go up there this past weekend.
- 36:14SPEAKER_01: So we met them up there.
- 36:15SPEAKER_01: We I never go up there during the summer, but it is awesome.
- 36:20SPEAKER_00: I bet it's so beautiful.
- 36:21SPEAKER_01: So Ellicottville, New York.
- 36:24SPEAKER_01: If you're listening, like you are amazing in the summer.
- 36:27SPEAKER_01: I'm so sorry.
- 36:28SPEAKER_01: I've had a house there for years and never been there in the summer.
- 36:31SPEAKER_01: It's not a mistake that I'm gonna make ever again.
- 36:33SPEAKER_01: So it was great.
- 36:34SPEAKER_01: I got to see dad, I got to see Pam, Amy and I got to go and have fun.
- 36:38SPEAKER_01: And um, you know, we had a little, you know, couple of cocktails.

- 36:44SPEAKER_00: I'm coming next time.
- 36:45SPEAKER_01: Okay, sounds good.
- 36:47SPEAKER_01: So I wanted to tell that story um because we want to make sure that we talk about uh family uh sometimes on here.
- 36:53SPEAKER_00: Yes, we've at the point of recording this, we've aired two episodes, and each time our dad has told us that we haven't talked about him nearly enough.
- 37:05SPEAKER_00: So right.
- 37:06SPEAKER_01: He told me this morning.
- 37:07SPEAKER_00: Yeah, yeah.
- 37:08SPEAKER_00: I hear it multiple times a week.
- 37:09SPEAKER_00: Like you guys barely talked about me.
- 37:12SPEAKER_00: You didn't barely mention me.
- 37:14SPEAKER_00: So we'll just have to make it a point to very casually work dad into every episode somehow.
- 37:22SPEAKER_01: I'm Dr.
- 37:22SPEAKER_01: Labrasca, board-certified plastic surgeon north of Pittsburgh and New York City.
- 37:27SPEAKER_01: I'm here with my sister, Mystica Labrasca.
- 37:29SPEAKER_01: She's a board-certified cosmetic dermatology PA of 16 years.
- 37:34SPEAKER_01: She is the master of skin.
- 37:35SPEAKER_01: I'm the master of scalpel.
- 37:37SPEAKER_01: Together, we are scalpel and skin.
- 37:40SPEAKER_01: We're siblings, we're business partners, and somehow we still get along.
