- 0:00when people come into us on a GLP-1 is, yes, they've lost weight, but what kind of weight have they lost?
- 0:06In fact, we see people that have lost almost no fat and all muscle.
- 0:11He went from saving hearts as a cardiologist to rewriting the entire aging playbook.
- 0:17Dr.
- 0:17Jeffrey Gladden believes 100 is the new 30 and he's got the science to back it up.
- 0:22This is the Gladden Longevity Podcast.
- 0:25Welcome to the Galadon Longevity Podcast.
- 0:27Autumn, it's great to see you.
- 0:28Great to see you too, in person.
- 0:30person, right?
- 0:31I'm happy to be back.
- 0:33No, we have not.
- 0:34time.
- 0:35Yeah.
- 0:35And you've been with us for the last couple days getting some stuff done.
- 0:38Yeah.
- 0:38Yeah.
- 0:39Doing all my checkups, running all the tests.
- 0:41Very excited.
- 0:42Got some already got some really helpful information back.
- 0:45So that's good.
- 0:46I love it.
- 0:46Yeah.
- 0:48I know I was like, Oh, I'm a little relaxed.
- 0:54Great.
- 0:55Right.
- 0:55good.
- 0:56Yeah.
- 0:56We find that brain tap is really great.
- 0:58It's just kind of allowing you to slip into that more relaxed, calm brain state, right?
- 1:04I was like at the beach.
- 1:05Right.
- 1:06Looking at the sky with the sun.
- 1:07That's right.
- 1:08It was great.
- 1:08that's awesome.
- 1:09Well, cool.
- 1:10One of the things we wanted to talk about, you actually brought this up to me, that there's a lot of conversation around GLP-1s, GLP-2s, GLP-3s, and there's a lot of, I
- 1:19think, controversy around those medications.
- 1:21There's also...
- 1:23some good that they can do, but also they come with some potential side effects.
- 1:27And we see a lot of people, you know, regaining the weight that they lost.
- 1:30And I thought it would be helpful to kind of walk through and get your questions on some of the GLP-1.
- 1:35And I printed off a bunch of stuff, looking at some things and integrating some of the ideas that I have around how actually a person can safely taper off a GLP-1 and not regain
- 1:44weight.
- 1:45Right?
- 1:45So I've got some stuff to share there as well.
- 1:48But in your world, what are you seeing with the GLP-1s?
- 1:52So, fitness and nutrition expert in my world, obviously, it's talked about nonstop right now.
- 1:58And it's almost like this buzzword that everybody wants to jump on.
- 2:02It's like turned into the trend.
- 2:04Yeah.
- 2:04Okay.
- 2:05But I think so many people don't even really understand what it is.
- 2:09It kind of goes along with peptides, right?
- 2:11Like, what are peptides?
- 2:13What's the GLP-1?
- 2:14What is all of this?
- 2:15Yeah.
- 2:15So, is GLP-1 a peptide?
- 2:18It is a prep test.
- 2:18Right.
- 2:19So maybe what we could do is, in the most basic terms, explain what a peptide is and explain what the GLP-1 is.
- 2:27exactly.
- 2:29essentially your body has many peptides.
- 2:31A peptide is simply any signaling molecule that's made up of amino acids.
- 2:36so insulin, for example, is uh a hormone, but it's also a peptide.
- 2:40Glucagon, which is the sort of the opposite of insulin, is also a signaling peptide.
- 2:46So the body makes lots of signaling peptides.
- 2:48The peptides that are out there...
- 2:50that are commonly talked about BPC157, GHKCU, TB500, CJC1295, and some of the mitochondrial peptides like Humalin and MOTC and SS31.
- 3:03Those are all essentially just strings of amino acids that when they are put into the body, signal the body to do something.
- 3:13So the beauty of a peptide is that unlike in the pharma world where
- 3:17You look at a biological biochemical pathway and you say, well, if we had less of that coming through, they'd have less pain and have less inflammation.
- 3:25So we'll develop a molecule that blocks the development of that molecule.
- 3:30Right.
- 3:31So that's the pharma model.
- 3:32The problem with the pharma model is that yes, it will decrease that, but the precursors have to go somewhere.
- 3:38So they go down other pathways and then you end up with side effects and some other things happening.
- 3:43Right.
- 3:43So.
- 3:43when peptides are used wisely, actually sets off an entire cascade of sort of healing benefits.
- 3:49For example, BPC157, which is probably the grandfather of all peptides, probably the one most people are familiar with.
- 3:56When you put it into the body, it doesn't do anything except basically stand there like the head domino, which then knocks all the other dominoes down to engage the body in the
- 4:09healing process.
- 4:10So BPC157, I've used that for healing.
- 4:13Okay.
- 4:14Do we have BPC157 naturally in- So let me ask that question first.
- 4:20Everybody's jumping on peptides.
- 4:22Are we all just deficient in peptides?
- 4:25Is this- Yeah, good question.
- 4:26we all need it?
- 4:27these are peptides that we make, some of them we don't make.
- 4:30uh But the point is that with BPC157, we make it in our gut, right?
- 4:35What's been found, however, is that when you increase the concentration of it, you can facilitate healing in other parts of the body as well.
- 4:43Whether it's even the brain, the shoulder, the knee, the hip, the gut, you can take it orally.
- 4:48And when you take it orally,
- 4:49Typically, unless it's uh modified in its delivery mechanism, if you just take it orally, it can heal the gut, but it's not going to do anything for any other part of the body.
- 4:57Okay.
- 4:58It doesn't work systemically.
- 4:59But if you inject it just below the skin, a sub-Q injection, then it will travel systemically to wherever it needs to go.
- 5:06You don't have to put it into the area that's actually hurt.
- 5:09Okay.
- 5:09But you can do that if you choose to, but you don't have to.
- 5:13Right.
- 5:13So we are, in an essence, not deficient.
- 5:18physiologically, but we benefit from an extra boost of it is the best way to think about it.
- 5:23So what I'm seeing, and this is gonna lead into the GLP-1, is a lot of people like, peptide this, peptide that, lose weight, duh, get a little shredded, everything's just,
- 5:32use the peptide to do it.
- 5:35So.
- 5:36uh
- 5:38right?
- 5:38Let the peptide do the work.
- 5:40oh
- 5:41kind of let the peptide do the work, which is we know that some peptides, we'll talk about GLP-1, can help us lose the weight, but the peptides aren't going to do the work without
- 5:49us doing the work as well, correct?
- 5:51They're great partners, but they're not surrogate actors, right?
- 5:56They can't do everything.
- 5:57They need us to do our part too, right?
- 5:59Like you can take BPC for your gut, but if you're continuing to drink alcohol and eat the wrong foods and whatever else and you're destroying your gut biome, the BPC can't overcome
- 6:11that, right?
- 6:12So physiology cannot overcome bad habits, so to speak.
- 6:15You're not going to out-peptide the bad habits.
- 6:18You're not going to peptide your way into actual great physical fitness.
- 6:22Right.
- 6:22If you're on a track for physical fitness, you can actually accelerate and augment and enhance the outcome of the work you put in with the peptide.
- 6:31But the peptide is there as a partner, not as the thing that's doing it for you.
- 6:36Right.
- 6:37So you can't hire the peptide to mow the lawn.
- 6:39You've got to mow the lawn, but it'll make the lawn look much better and you'll do it much faster.
- 6:43It's like being on the riding mower instead of the push mower.
- 6:45Might make it little easier, but it's not gonna do it for you.
- 6:49So when we get into GLP one, let's just really, I'll just for lack of a better word, let's dumb that down.
- 6:56What is it exactly?
- 6:58Because we're seeing it in injectables, obviously.
- 7:02We're seeing it in pill form.
- 7:03We're seeing it in sort of like, take this powder in a drink form.
- 7:07It starts to go, what's the one that's really doing the work?
- 7:10Are all these is everybody just slapping a label on the AGLP one to market off of it?
- 7:14What are we?
- 7:15What are we talking?
- 7:16make our own, so to speak.
- 7:18And so we have GLP-1 receptors.
- 7:20So when we take a GLP-1, it actually does a number of different things.
- 7:26I'm talking about the inject.
- 7:27Okay.
- 7:28One.
- 7:29Right, we inject GLP-1s.
- 7:30And I've got biochemistry here that we could go through, but the interesting thing is that in the neurological system, it actually increases the excitatory...
- 7:39neurotransmitter glutamate, but it also increases the inhibitory neurotransmitter GABA, right?
- 7:46And keeping that balance is important.
- 7:48So if you're using a GLP-1 and for some reason you notice that you're not sleeping as well or whatever, it may be that you're getting too much of the glutamate effect and not enough
- 7:58of the GABA effect and you might need to work on the GABA effect, right?
- 8:01Like things like taurine and...
- 8:04things that will actually boost GABA, staying away from foods that are high in glutamate, that kind of thing.
- 8:08So that's one thing to keep thinking about.
- 8:10The other thing to know is that the GLP-1s, like any peptide, they go systemically and they go to different receptors.
- 8:16So what happens is they go to the hypothalamus in the brain and they decrease a hunger peptide called ghrelin.
- 8:24So they decrease the production of ghrelin.
- 8:26So all of a sudden you don't have the same hunger.
- 8:30And the other thing that happens is they go into the gut and they actually slow gastric emptying.
- 8:35So your stomach is, you know, it stays full longer.
- 8:38Right.
- 8:39So you have more of a sense of satiation just from the food you do eat.
- 8:42And then the other thing is they will actually increase insulin release from the pancreas to control blood sugar.
- 8:48So those are good things that it does, but it also decreases glucagon.
- 8:52Glucagon is the hormone that's anti-insulin.
- 8:54Insulin putting blood sugar into your cells.
- 8:56Glucagon is actually causing it to release blood sugar from the liver.
- 9:00to support blood sugar levels.
- 9:02So when you combine less glucagon with more insulin effect, it's gonna lower the blood sugar, which is why these drugs were actually developed for diabetics, right?
- 9:10People with metabolic syndrome, pre-diabetes, they're wonderful drugs to help control the blood sugar.
- 9:16And in that context, they have wonderful effects also that have been shown to improve blood vessel function and decrease heart attacks and improve kidney function.
- 9:26So there are...
- 9:28organs that actually benefit from the impact of the GLP-1s.
- 9:31This is through the medical lens, right?
- 9:33The situation comes where they have other impacts and we'll talk about that in a second, but that's a GLP-1.
- 9:41Okay, let me tell you about a GLP-2.
- 9:42Okay.
- 9:43And then a GLP-3.
- 9:44And then we'll take your questions.
- 9:45Is that okay?
- 9:46Yes.
- 9:46Okay.
- 9:47So, terezepatite is supposed to GLP-1 and a GIP agonist, right?
- 9:53So, what the GIP does is it increases insulin.
- 9:56production even further, and it also improves fat handling in the fat cells.
- 10:02Okay.
- 10:03So it improves that.
- 10:04It may be a little bit better tolerated.
- 10:05Now the last one, the GLP-3, let's call it, is not FDA approved.
- 10:10It's Reta-Trutide.
- 10:11Now Reta-Trutide has been available, but the FDA is now kind of clamping down on certain pharmacies that have been selling it.
- 10:18Okay.
- 10:19So it is available inside research protocols.
- 10:22but Eli Lilly kind of has those locked up at the moment.
- 10:25Where it stands with FDA approval is it's now in phase three trials.
- 10:28So it's passed a safety study, it's proven efficacy.
- 10:31Now they're doing it in larger populations to find out what the side effect profiles are before they approve it, before they release it.
- 10:37So that's where it's probably gonna come out in 26 or 27.
- 10:42Just to put that in perspective.
- 10:43The beautiful thing about Ritter-Trutride is it actually adds this glucagon receptor agonist, right?
- 10:49And what happens with that, when you do that, is an increased energy expenditure, right?
- 10:54Whereas a GOP1 actually decreases energy expenditure.
- 10:58Okay.
- 11:01And then it also increases fat lipid utilization.
- 11:05So in other words, it's utilizing fat as a fuel source.
- 11:08It's increasing energy production and it's using fat to fuel that.
- 11:12Okay.
- 11:12Which is a good thing.
- 11:13Right.
- 11:14So it's increasing fatty oxidation.
- 11:16So those are the differences right there.
- 11:18And so...
- 11:19When you compare them for weight loss, semi-glutide, GLP-1 has an effect.
- 11:24Teresepatide has a greater effect and red or trutide has an even greater effect.
- 11:29Specifically.
- 11:31weight loss.
- 11:31Right.
- 11:32For weight loss specifically.
- 11:33This is why a lot of people are really fascinated by red or trutide at the moment.
- 11:36Right.
- 11:37And then in terms of insulin sensitivity, GLP-1's increase that insulin sensitivity, right?
- 11:43So blood sugar, better control.
- 11:44GLP-2's do it even more.
- 11:46GLP-3's also do it more.
- 11:48Okay.
- 11:49So that's how that works.
- 11:51And then when you look at the cardio metabolic effects and risk, semi-glutite decreases it by a factor of let's call it one, the GLP2 by a factor of two and erythritide by a factor
- 12:01of two.
- 12:01Okay.
- 12:02Okay.
- 12:02So you're getting, as you add in these other components to a GLP1, you're getting more benefit and more effect, if you will.
- 12:11And in terms of apokite control, they're all strong.
- 12:13They all work on the girl.
- 12:14And in terms of energy expenditure,
- 12:16The GLP-1 decreases energy expenditure.
- 12:20The GLP-2 is neutral and the GLP-3 increases energy expenditure.
- 12:25That's why people are so taken with credit true type, right?
- 12:28Okay.
- 12:29And I think rightly so.
- 12:30When it comes to uh fat mass loss, it can happen with the GLP-1, although you have to be careful in how you do this.
- 12:37We'll talk about that.
- 12:37Okay.
- 12:38It can happen with a GLP-2, but the highest fat loss is with the GLP-3.
- 12:44And the reason for that
- 12:45is because you're increasing energy expenditure.
- 12:48So with the GLP-3, one of the big side effects that we see in the practice when people come into us on a GLP-1 is, yes, they've lost weight, but what kind of weight have they
- 13:00lost?
- 13:00Right.
- 13:01And what we see is that muscle mass is going down proportionally more than fat mass.
- 13:08In fact, we see people that have lost almost no fat and all muscle.
- 13:15because they've suppressed their appetite.
- 13:17They've decreased energy production, okay, but they're eating less, they're exercising less, and they're losing muscle because the body can burn fat as a fuel, but it actually
- 13:28needs more than just fat to burn.
- 13:29And so it gets other minerals and things from the muscle, so it has to go break that down.
- 13:34We had a guy come into us who was doing a water fast, and he did a water fast for 14 days.
- 13:39He was gonna do it for 28 days.
- 13:41We had done an in-body on him.
- 13:43before he started this fast.
- 13:45Upon being honest to he decided to do this.
- 13:46He'd it previously in his life.
- 13:48Okay.
- 13:49He came back in and said, I'm doing a 14 day water fast.
- 13:52And I said, that's interesting.
- 13:54How's that going?
- 13:55What's that?
- 13:55I'm losing weight.
- 13:56I was like, great.
- 13:57Let's have him do an in-body for you.
- 13:59He had lost weight.
- 14:00I don't, maybe 15, 20 pounds.
- 14:02A hundred percent of it was his muscle.
- 14:05He had lost no percent body fat.
- 14:08It's brutal.
- 14:08Right?
- 14:09Really brutal.
- 14:09Maybe a little bit of fat.
- 14:10And the bone density was also down.
- 14:13So when people go into starvation mode, right, this is not the way to quote unquote lose weight.
- 14:18Right.
- 14:19And so we're interested in the strategies of how do we actually take advantage of the benefits of either a GOP 1, 2 or 3 and yet maintain muscle mass, right?
- 14:30So that comes down to your neck of the woods, which is nutrition and fitness.
- 14:35So you've got to basically be doing resistance training, exercise that's stimulating the muscle to grow and maintain its mass and you've got to be feeding it appropriately.
- 14:44One of the best ways that we found to do that is a lot of people out there saying, well, just eat a lot of protein.
- 14:49It's like, okay, well, let's think about that for a minute because only even the best protein, only 48 % of that protein goes into making protein in your body.
- 14:57The rest of it turns into carbohydrates.
- 14:59It's my pet peeve right now.
- 15:00Right?
- 15:00And so how much protein do you want to eat?
- 15:02How much methionine do you want in?
- 15:04Because we know that people on high protein diets actually age faster, activate more mTOR, they don't live as long and they end up with more cancer and kidney disease.
- 15:12Right?
- 15:12So this is not exactly what we want.
- 15:14What you want is to build protein.
- 15:16And so the logic is, well, if I'm going to build protein, I need to eat a lot of protein.
- 15:20Well, that's the wrong logic.
- 15:22What you need to do is build a lot of protein.
- 15:24How do I do that without having to eat a lot of protein?
- 15:27Well, you can do that really nicely with
- 15:29Perfect Aminos.
- 15:30Perfect Aminos is a great product that basically has all the essential amino acids, including a lot of leucine, which stimulates muscle growth, right?
- 15:38And you don't have to take in all the protein to get the benefit of the protein.
- 15:42And when you combine it with creatine, that becomes really pretty magical in terms of maintaining muscle mass because now you can work out harder.
- 15:50And creatine basically enables your body to make ATP.
- 15:54bypassing one of the main ways you make it, it's kind of a shortcut to making ATP for energy.
- 16:01So if you take creatine before you work out, you can take your perfect aminos then or after, and you do your workout, now you're actually teeing yourself up for success in
- 16:10maintaining muscle mass, right?
- 16:11And if you're taking a bone density supplement also with vitamin K2 and K4, not MK7, but MK4, you can maintain your bone density as well as long as you're doing, you know,
- 16:23training where you're basically loading your bones.
- 16:27I my EAAs, my perfect aminos, and my creatine every day.
- 16:32my weight training.
- 16:34I love it.
- 16:35I feel great.
- 16:36Like that has definitely been something I started probably Six or seven months ago, and it feels really good, but I want to back it up I have yeah, I have a lot of questions and I
- 16:45think people watching or listening are gonna have a lot of these questions So GOP ones twos threes does everybody need to be on it?
- 16:54No, right exactly But why but everybody's getting on it or everybody's trying to get on it
- 16:59Yeah.
- 16:59Here's the thing.
- 17:00If you have the ability to modulate your diet, let's say, let's say GOP once didn't exist for a minute.
- 17:06How would we go about optimizing somebody's weight?
- 17:10Yes.
- 17:11The first thing we would do here, what we've learned is to do a five day fast mimicking diet.
- 17:17Because with a five day fast mimicking diet, what happens is there's enough carbohydrate in there and enough protein that you actually don't really lose muscle mass during that.
- 17:28five days.
- 17:29What happens with a five day fast food making diet that is so critical is it resets your taste buds so that you no longer are craving the same things you were before.
- 17:39It also resets the hypothalamus.
- 17:41So it lowers the garland.
- 17:44It also increases leptin resets.
- 17:46sensitivity.
- 17:47So leptin is basically the hormone that gets released from the small intestine and fat cells to tell you to stop eating.
- 17:52People that are obese have leptin resistance.
- 17:55simply, they have a lot of leptin, but they're not getting the signal because they have resistance at the receptor.
- 18:00They don't get the signal to stop eating.
- 18:02Right.
- 18:03Well, right now you're hearing this term food noise.
- 18:05Have you seen this?
- 18:06Like, it's like a buzz phrase.
- 18:09I'm taking this peptide.
- 18:10I'm taking GLP-1.
- 18:11It's gotten rid of my food.
- 18:13Okay, so food noise is basically...
- 18:15Like I'm thinking about food or I feel like I'm thinking about food all the time.
- 18:20It's consuming me Yeah, and these are not necessarily obese people These are people who might have 10 or 15 pounds to lose sure So does the person who has even five ten or 15
- 18:31pounds to lose is that does that should that person be?
- 18:34Using a GOP one two or three or does that person need to get off the overprocessed Highly stimulating your taste buds foods and put in the word
- 18:43So here's the trick.
- 18:44That person can take the GLP-123.
- 18:47The problem is when they get off of it.
- 18:51Because when they get off of it, what happens is basically they still end up with, especially with the GLP-1s, with decreased energy consumption or production, right?
- 19:01So they're slowed their metabolism essentially.
- 19:04yet the ghrelin, which has been suppressed by the GLP-1 working in the hypothalamus, suppressing ghrelin, suppressing appetite,
- 19:12That rebounds.
- 19:13So now it comes back stronger.
- 19:16So now you come back with a stronger appetite and a lower metabolism.
- 19:20What's that going to do?
- 19:22You're right there.
- 19:23You're right back where you started, if not worse.
- 19:25Right.
- 19:26So there's really two questions here.
- 19:28One is if we were to use a GLP one, two or three.
- 19:31Now the GLP threes are a little better because you've raised your metabolic rate, but you still have the girl in rebound to deal with.
- 19:37Okay.
- 19:37But if you.
- 19:38If you were to say, okay, I'm not going to go down a GLP pathway.
- 19:42What would I do?
- 19:43Okay.
- 19:44Well, I would do a five day fast-mimicking diet because it resets garland.
- 19:48It resets your taste buds.
- 19:50It resets your gut biome.
- 19:52People don't realize that a lot of their appetite is fueled by the bacteria in the gut.
- 19:57Yes.
- 19:58If you're eating sugar all the time, you are basically growing a garden of sugar loving bacteria.
- 20:04And when they don't get sugar, they start moaning about it.
- 20:07And they're sending signals to the brain.
- 20:09Hey, hey, hey, what about it?
- 20:11It's like little birds in a nest.
- 20:13Feed us.
- 20:14Right.
- 20:14So you can actually reset the gut biome by doing the five day fast women can diet.
- 20:20can quiet that noise.
- 20:21So when you think about food noise, some of it is psychological.
- 20:25Some of it is gut biome creating it.
- 20:28And some of it is the level of girl in, your brain.
- 20:31So when you do the five day fast women can diet as a starter, then
- 20:36You're resetting all of that.
- 20:38And that's what you want to do.
- 20:40We'll come back to that as a tapering technique also.
- 20:43The other thing that we like is we like sugar to fiber.
- 20:47Sugar to fiber is a really interesting product because it actually takes carbohydrates and has some enzymes in it and breaks them down.
- 20:54And instead of you absorbing and raising your blood sugar, it actually turns it into fiber, which feeds your gut biome in a healthy way.
- 21:00Right?
- 21:01So now all of a sudden I'm not getting that sugar spike from what I am eating.
- 21:05I reset my taste buds when I am eating some carbs.
- 21:08I'm not getting into sugar spike that's fueling the whole problem.
- 21:11And I'm getting fiber from my gut.
- 21:13The other thing we like to add is we like to add a probiotic, one called metabolic.
- 21:19It's made by a company called Prodrome, I believe.
- 21:22And it's refrigerated, which is a little cumbersome because it's harder to travel with.
- 21:26But you just take one a day and it has uh acromansia in it, it has a number of other bacterial strains.
- 21:33I think it has five bacterial strains in it.
- 21:35And it actually will create a population of bacteria in your gut that increase your endogenous, your own GLP-1 producing capabilities.
- 21:44So now you're getting your own biological GLP-1 in a very physiologic way.
- 21:50So now you've reset the table, you decrease the carb consumption, you're eating the perfect aminos in the creatine for power and energy and production.
- 21:58And you've got this metabolic probiotic that's actually giving you some GLP-1 to help modulate it.
- 22:04We think that combined with protracted cardio and, you know, some weight training and things like that is a much better way to go because it's actually a durable plan.
- 22:17Right.
- 22:17And if you want to, if you want to repeat the fast-making diet in a month or six weeks or whatever, three months to kind of help you continue on your path, then go ahead and do
- 22:28that.
- 22:28You can do it monthly.
- 22:29Right.
- 22:30so we think that's a really sane way to go.
- 22:33Now let's say that
- 22:34you have diabetes or you have a metabolic disorder or problem or you're 60 pounds overweight and you want to use the GLP-1 to help you.
- 22:44We don't snivel at that because it's a big deal and there's a of health costs to carrying all that weight, right?
- 22:50So use the GLP-1, 2 or 3.
- 22:52We do think the GLP-3 is going to be the ticket once it's released.
- 22:55was gonna say, why would somebody use the GLP-1 if the GLP-3 seems to do...
- 23:00Yeah.
- 23:01Better.
- 23:01Right now you can't get the GOP3.
- 23:03That's the reason.
- 23:04Unless you're going outside of traditional things, right?
- 23:08Which as physicians with a medical license, you can't do.
- 23:11But people will find a way, right?
- 23:13So the GOP3 is probably the ticket.
- 23:15GOP2 is better than GOP1, right?
- 23:18And people can get the GLP too right now.
- 23:19That's right.
- 23:21Okay.
- 23:21So let's say you need that.
- 23:23So then you do that.
- 23:23But nonetheless, I would do it this way.
- 23:26I would start with a five day fast, maybe night.
- 23:29Why not make your life easier?
- 23:30Why not make the work of the GOP one or two easier?
- 23:33So start with a five day fast, maybe we the other things we talked about and then use it.
- 23:39And what you want to do is you want to titrate it up slowly because you get more muscle wasting when you go up faster.
- 23:46So you titrate it up slowly, you're doing the other things.
- 23:49Now you actually have a synchronized program.
- 23:52And when you actually want to come off of it, you can use the five day fast mimicking diet again to reset ghrelin, right?
- 24:00So you can actually come off of it and not have the rebound spike, right?
- 24:04Okay.
- 24:05So I have another question.
- 24:07and again, I'm referencing this only because of what I'm seeing so much in like the fitness world and who's using these.
- 24:14And it's of course people that like you said, 60 plus pounds, obviously that makes sense.
- 24:18But when I see people that have like that 10, 15 to 20 pounds, does every person that has 10, 15, 20 pounds to lose have a blood sugar problem?
- 24:27You know, it's shocking.
- 24:28They typically do.
- 24:29Okay.
- 24:29They don't know it.
- 24:30They, one of the things that we do here at Gladden Longevity is we measure a two hour glucose tolerance test, right, with an insulin curve.
- 24:38So what does that mean?
- 24:39Well, it means that when they come in, we check a fasting blood sugar, we check a hemoglobin A1C, we check something called fructosamine, which is a two week look back at
- 24:47whether they're spiking their sugar.
- 24:48Normally their doctor will just do a fasting sugar and a hemoglobin A1C, which is a three month look back at what's your average blood sugar.
- 24:54Right.
- 24:55But what we find is that when you check a fasting insulin, you can see if they're running higher insulin, they should be running insulins around two, three, maybe four as a
- 25:05baseline.
- 25:05We see a lot of people come in with that extra 15, 20 pounds, they're running insulins of nine, 10, right?
- 25:1012.
- 25:11Then you challenge them with 75 grams of glucose.
- 25:14It's like a stress test for your pancreas and your metabolic.
- 25:18I know you love it.
- 25:19Right.
- 25:20It's good for you.
- 25:21So I think it was an exercise.
- 25:24anyway, we challenged them with that and then we remeasured the insulin and the glucose at one hour.
- 25:29And then we remeasured the insulin and the glucose at two hours.
- 25:32And what we can see is if your baseline insulin is eight,
- 25:36It shouldn't go up by more than four to five times at one hour.
- 25:40And then it should be back down to eight again.
- 25:42So eight to 40 to eight, then that would be, that would be good.
- 25:46And if you, particularly if you're controlling the blood sugar, which means the blood sugar starts at, let's say 95 and it goes to 135 and then at two hours is back to 95.
- 25:56But what we see all the time is that it may start at 95, but it goes to 180, which is diabetic.
- 26:04The insulin goes from eight.
- 26:06Yeah, from eight instead of to 40, it goes to 85.
- 26:09And at two hours, it's still at 75.
- 26:12Okay.
- 26:12This is classic insulin resistance that's totally missed because they were just looking at the fasting sugar and hemoglobin A1C.
- 26:19And we can see the degree to which it's happening.
- 26:21Right.
- 26:21We can also tell people who have damaged their pancreas by overworking it so much that they've actually lost the capacity to even begin to gain.
- 26:32For example, their blood sugar maybe is 115 at rest, their insulin level maybe six.
- 26:38We give them the glucose, their insulin level can only go to 14, but their blood sugar goes to 250, right?
- 26:45So this means the pancreas is shot.
- 26:47So now they're actually looking more like insulin.
- 26:50So unless this is actually done, you can't really tease apart where somebody is.
- 26:55And it's really important to get this right because when you either have high blood sugars, which are damaging your arteries, accelerating aging, or you have high insulin
- 27:02levels, which are also stiffening your arteries, increasing senescent cells and accelerating aging, you kind of know about that.
- 27:10So what about all the people that are just sort of like jumping online and getting it sent to them and they don't know any of this?
- 27:17Right.
- 27:17They don't know actually if they have high blood sugar or not.
- 27:21They just know, I want to lose weight.
- 27:24What kind of risks are they maybe putting themselves at?
- 27:27Because if you don't have high blood sugar and it drops your blood sugar, are we in a danger zone?
- 27:32Well, here's the thing, you can develop pancreatitis with these drugs.
- 27:35You can also develop thyroid tumors, small tumors.
- 27:39That's probably not such a high risk, but pancreatitis is.
- 27:42We've had people that we've known about end up in the hospital, not that we were prescribing for, but people we know about that ended up in the hospital in the ICU with
- 27:51acute pancreatitis.
- 27:52Okay, so another big thing we're seeing is for women like me, I'm 45, I'm in perimenopause, and we're hearing it talked about to microdose as you go into
- 28:01perimenopause, because we know like blood sugar insulin shifts, that has a cascading effect on all of our...
- 28:08is interesting, right?
- 28:09So I think that can work well because you're really kind of avoiding a lot of the side effects when you're doing such low doses.
- 28:15Okay.
- 28:16And I think the microdose is probably okay.
- 28:18I will say this, there's another agent out there that I have yet to mention.
- 28:22Okay.
- 28:23It actually was discovered in New Zealand and it comes from hops, although you can't drink beer and get it.
- 28:29Okay.
- 28:31Sorry ladies.
- 28:32Yeah, so it's called callocurb, is the name of it, right?
- 28:35And it's basically, in New Zealand, they basically studied over 3,000 different compounds from various plants, trying to find something that would suppress appetite.
- 28:44And what they found with callocurb is that it activates bitter taste receptors throughout the GI tract.
- 28:50So you have some in your mouth.
- 28:51That's why when we eat bitter things, it's actually good for us, right?
- 28:56We've kind of been trained by the food industry to go after salty, sweet, savory, right?
- 29:02But actually, bitter things, bitter plants, bitter roots, anything that's bitter actually is good for us.
- 29:08It's nutritionally dense and it actually suppresses appetite.
- 29:12Anyway, with this hops thing, if you've ever tasted hops, it's pretty bitter, right?
- 29:16Yeah.
- 29:16So the thing is that when you have these taste receptors, these bitter receptors, they're in your mouth, they're in your esophagus, there are a lot of them in your stomach.
- 29:25the most of them in your small intestine and then there's even some in the colon.
- 29:28Okay.
- 29:29So it's really the whole GI tract that you're affecting.
- 29:32And what it does is it's also a GLP-1.
- 29:35In other words, it activates the release of your endogenous GLP-1.
- 29:39Okay, so it's not the same as injecting yourself, but and this is so this is a pill
- 29:45It's a pill.
- 29:45Okay.
- 29:46It's a pill.
- 29:46Calo curve.
- 29:47Yeah.
- 29:48So my other question is, obviously all of these suppress appetite.
- 29:52What if somebody is not an overeater or are they automatically an overeater?
- 29:59I will, so I'll use myself as an example.
- 30:01I'm not looking to take a GLP-1 or anything, but I've been the same weight for 18 years.
- 30:09110 pounds, give or take a pound or two.
- 30:12The only thing that has changed for me being in perimenopause is my fat mass to my muscle mass.
- 30:17And I still work out five days a week.
- 30:19I still lift incredibly heavy.
- 30:22I don't overeat.
- 30:25So what would somebody like me do where my fat mass is shifting to being a little bit, having a little bit more fat and a little bit more less muscle?
- 30:34It's optimizing your sex hormones at this point.
- 30:37So when we go through either menopause, perimenopause, andropause, periandropause, mean, men go through a similar situation.
- 30:45Body mass changes.
- 30:46This is why men start to look like this, right?
- 30:48Women tend to put it on more in the hips and thighs.
- 30:51So it's really important if you want to maintain body composition, which is the fat mass where you want it, the muscle where you want it, to keep your sex hormones optimized.
- 31:02So we think that, you know, initially hormones were only prescribed when women, let's say, had symptoms, hot flashes.
- 31:10Okay, we'll give you a bit of estrogen to calm those down.
- 31:13Super short-sighted, right?
- 31:14Progesterone actually protects you from cancer and many other things.
- 31:18Women without estrogen, their brains do not do as well.
- 31:21You know, lot of women will complain of memory loss as they go through perimenopause or menopause.
- 31:27And a lot of has to do with low estrogen.
- 31:29Men have the same thing.
- 31:31And one of the worst things that people can do for a man is to go to a tea clinic, a testosterone clinic, and put on testosterone and then something that blocks the production
- 31:40of estrogen from it.
- 31:41Because we want to be manly, so we're going to have all this testosterone.
- 31:44Well, guess what?
- 31:44Your brain needs the estrogen.
- 31:46Your bones need the estrogen, right?
- 31:48Even to burn fat, you need the estrogen.
- 31:50So men need estrogen and women need testosterone.
- 31:53It's just a question of balance.
- 31:54Right.
- 31:54So you really want to optimize that as you go through perimenopause.
- 31:59You don't want to wait for symptoms.
- 32:00You want to actually start optimizing, measuring your hormones throughout your cycle and making sure that you're on doses that will optimize them.
- 32:07There are also things like maca and things like that will actually help the body regulate the hormones.
- 32:13Right.
- 32:14So we're fans of something called Mighty Maca.
- 32:16We have a lot of people taking that.
- 32:18Men and women can benefit.
- 32:19The other thing about that, sexual hormones and uh brain fog is that if you have a gene like APOE4, right?
- 32:27Like this is Alzheimer's gene, APOE4.
- 32:30have it.
- 32:31Okay.
- 32:31APOE1, Tom 40.
- 32:33If you have that trifecta, you have APOE4, APOE1.
- 32:37So if you have that, estrogen is mandatory because in order for your brain to function properly, you have to have estrogen and it's even...
- 32:48accentuated when you have APOE4.
- 32:50So getting the sex hormones right not only saves your brain, but enables you to actually have the body comp that you're looking for.
- 32:55So I would say that you're probably a hundred dose if you're starting to notice changes in terms of the sex hormones at this point.
- 33:02a little uh
- 33:08of restridges too.
- 33:09There's E2 and E3 and E1 and it's E2 that does the work.
- 33:14Right?
- 33:14So if you're going to use topical, you want to make sure that it has plenty of E2 in it and not just E3 with a smidge of E2.
- 33:21Right?
- 33:21So it's really important to get the right combinations.
- 33:24Yeah.
- 33:24m
- 33:25So yeah, it's not always about just suppress your appetite.
- 33:29Right.
- 33:30And I think that's something that's overlooked too.
- 33:32It's like, oh, okay, I'm gaining weight.
- 33:34I must need to suppress my appetite.
- 33:36I just have to eat less.
- 33:37And it's not always that.
- 33:38I'll tell you another thing that's missed all the time.
- 33:40And that is something we do here routinely for every client is to measure their resting metabolic rate.
- 33:46So that's measured basically by wearing a mask and we measure O2N CO2 out.
- 33:51From that we can actually calculate if they're on a bike while they're doing that we could be O2 max anaerobic threshold.
- 33:56That's great.
- 33:57But in a chair, if you sit there for 20 minutes, we simply get what's your resting metabolic rate.
- 34:02And we find that many people
- 34:04are hypothyroid and they don't know it.
- 34:06And so if the set point on thyroid is lower than it should be, your metabolic rate is already slower.
- 34:12Right.
- 34:12Right.
- 34:13And so if you add to that, take out some of the hormones and the other things that cause you to be more anabolic and, you know, using calories to build stuff, all of a sudden now
- 34:23you're really shifting fast.
- 34:24Right.
- 34:25So I think if you're going to optimize weight, you have to start with a resting metabolic rate.
- 34:29And the reason is that thyroid blood work is not the Holy Grail.
- 34:33It's you cannot look at blood work and know what the metabolic rate is.
- 34:37You'd like to think you can, but you can't.
- 34:39We've looked at so many of these.
- 34:40We just know you can't.
- 34:41So a resting metabolic rate hormones are critical.
- 34:45You get that right.
- 34:46Then some nutrient values.
- 34:47Do you have enough chromium to actually make your insulin receptors work?
- 34:50You know, you've got to look at all the pieces of the F1 car.
- 34:54Why isn't it running right?
- 34:55Oh, it's not running right because of these.
- 34:57What if you six those four things?
- 34:59Maybe you don't need a GOP one.
- 35:00Maybe you just actually need
- 35:02to get your hormones optimized and your thyroid and you're good to go.
- 35:04Right.
- 35:05So what we've found is that precision in the diagnostics is typically what's missing.
- 35:12So you have people that like, need to lose weight.
- 35:14GLP-1, I'll grab it.
- 35:15Where were the diagnostics?
- 35:16Non-existent.
- 35:17Right.
- 35:18Non-existent.
- 35:19Right, and sometimes it's just clean up your lifestyle.
- 35:22Sometimes it's just that.
- 35:24move a little bit more.
- 35:25Sometimes it's a little bit deeper.
- 35:27We need to look at thyroid.
- 35:28We need to look at sex hormones.
- 35:29Yep, that's right.
- 35:31It's a really interesting topic.
- 35:32And I do think that there are good indications for GOP 1, 2, and 3, depending on, you know, any underlying medical need.
- 35:39I think the GOP 3 is probably going to be probably the best for weight loss.
- 35:44And I think in any of these situations, because when you stop it or try to taper off of it, you get this girl in rebound, which is going to drive your appetite real high.
- 35:53You want to have a strategy around that and you can't just stop it.
- 35:58So I think
- 35:59The five day fast-memaking diet, the sugar to fiber, using the metabolic probiotic and adding in the callocurb, right?
- 36:08That becomes a really nice way to transition out of a GLP one, two or three.
- 36:14so you're not on it forever.
- 36:15Right.
- 36:16And I think many people can actually get where they want to go just using that without ever needing to get the prescription.
- 36:22Well, I can speak from experience.
- 36:24Obviously, you know, I created a hormone health program for women of all ages, all phases, fitness, nutrition, stress management, and a couple of supplements.
- 36:33And we did all the labs and everything.
- 36:35And so for people like we, we showed you can balance your blood sugar, improve insulin sensitivity.
- 36:42And actually in those labs that we ran, we saw women's hormones.
- 36:46Balance out to improve estrogen go up.
- 36:49Yep.
- 36:49I were everything balance out.
- 36:52like, right.
- 36:53Before you jump into, have to do all these things.
- 36:55It's can you optimize your nutrition first?
- 36:58I know that's not the easiest thing because I know, look, we all want to have the glass of wine every night.
- 37:03We all want to have the dessert.
- 37:04love pizza too.
- 37:06Moderation portions, things like that.
- 37:09Getting the fiber in, our diet, getting those nutrients in, moving our bodies.
- 37:12sit so much.
- 37:14That's
- 37:14I think it's so important to have that part of that conversation too of, yes, there's all these other things that can help optimize, but if you're not looking at your lifestyle
- 37:23first.
- 37:24Here's the problem I think that people run into and you tell me if you think this is right is that we all have willpower in the morning and nobody has willpower at night.
- 37:34Right.
- 37:34So if there's ice cream in my refrigerator at night at eight thirty, I'm going to eat it.
- 37:39It's not like I have super willpower.
- 37:41Right.
- 37:42What I've done is I've architected my environment to support the mission.
- 37:46Yes.
- 37:46There's no alcohol in the house.
- 37:47There's no ice cream in the house.
- 37:49There may be some dark chocolate.
- 37:50Yes.
- 37:51You know, there's some stuff.
- 37:52But if I don't do that.
- 37:54I'm like everybody else.
- 37:55So I think you've got to architect your environment.
- 37:57If you're listening to this, you've got to architect your environment.
- 38:00Otherwise, nobody has enough willpower to do this.
- 38:04And then I think you can help yourself by resetting the whole system with like a five day fast-mimicking diet, using the Cali-Curb, the right probiotics, know, the sugar to fiber.
- 38:14Next thing you know, it's like, oh yeah, I'm not really...
- 38:16hungry for that anymore.
- 38:18then now it's reinforcing.
- 38:20Now you've got an environment that reinforces it.
- 38:22Now you've created a system around yourself, right?
- 38:25And it's only systems that win, right?
- 38:27It's not willpower.
- 38:28Willpower never wins.
- 38:30So uh you have a system that helps you win.
- 38:33And I think that's the key, no matter what strategy you're taking.
- 38:36Yeah, I tell people all the time, being prepared isn't half the battle, that's the battle.
- 38:41you prepare yourself, you have the healthy food on hand.
- 38:44That's right.
- 38:44You know what the easy go-to meals are.
- 38:46It drives me crazy when people are like, well, I was really hungry so I grabbed something fast.
- 38:50I'm like, an apple is just as fast as the cookies.
- 38:55But it has to be.
- 38:57Exactly.
- 38:58And same thing.
- 38:59I love ice cream.
- 39:00I love dessert.
- 39:01I don't really keep it in the house.
- 39:02I keep dark chocolate.
- 39:04It doesn't mean I don't ever have ice cream.
- 39:06It means that if I'm going to have it, it's a treat and I have to go get it.
- 39:09We're going to go somewhere.
- 39:10We're going to get really good quality.
- 39:12I'm going to enjoy that.
- 39:14That's exactly what I'm going to have.
- 39:15This is a lot of my son actually works at a gelato place.
- 39:18And that, but that's it.
- 39:19Like I'm not going to bring it in to the house where I would eat it every day.
- 39:23You know, I know when my workouts are going to happen.
- 39:26know when my rest days are going to happen.
- 39:27And I think that is where a little discipline comes in.
- 39:31That's where a little.
- 39:33motivation and willpower comes in to just set up those plans and those strategies.
- 39:38I think the discipline is in the structure you create.
- 39:42And then I think it's really important if you are using a GLP 1, 2, 3, and even if you're trying to lose weight in general, that you have this strategy of using the perfect aminos
- 39:51to creatine.
- 39:52You're doing the weight training that enables you to have the muscle.
- 39:55You're doing the cardio training that allows you to have VO2 max and a good respiratory capacity.
- 40:01And then when you piece all that together, now you can actually lose weight.
- 40:06But what's really important is you've lost fat, gained muscle, right?
- 40:11Like when I had my own health journey, right, and I was hormonally depleted.
- 40:15And I go and say, South Surround and DHEA.
- 40:17I lost 20 pounds of fat and put on 10 pounds of muscle.
- 40:20Amazing.
- 40:20Right?
- 40:20So that was what I was looking for.
- 40:23And that's, that's really what everybody's looking for.
- 40:25Right.
- 40:25Let me lose 20 pounds of fat and put on 10 pounds of muscle.
- 40:28I lost 10 pounds.
- 40:29I don't care what the scale says.
- 40:30I look good.
- 40:31I feel good.
- 40:31Right?
- 40:32You can't get married to a number on the scale.
- 40:34It's got to be body comp.
- 40:35Right.
- 40:36So.
- 40:36Huge yeah, like I said my number on the scale has been the same for 18 years, but my body doesn't look the exact same The composition is shifted now.
- 40:44I'm not unhappy about it, but it's but I'm always looking to Tweet it dial it check it out.
- 40:51There might have been a hot flash that happened while we were sitting here uh Okay I think this has been super helpful so that people can really understand
- 41:02Go ask the right questions, do their own research about what's going to be right for them.
- 41:07And do the diagnostics.
- 41:09my gosh.
- 41:09If you can do the diagnostics and actually understand who you are, where you are.
- 41:14Do I have insulin resistance?
- 41:16I don't.
- 41:16What's my body composition?
- 41:17It's really easy with an in-body.
- 41:19You can get a good estimate of it.
- 41:21You know, having
- 41:22a diagnostic that you can leverage off of to see what kind of progress you're making.
- 41:27Am I losing muscle mass?
- 41:28Am I losing fat mass?
- 41:29What's actually happening as you go through the process?
- 41:32The things that we've shared with you here today, Ben, things that we've seen enable people to lose weight, but also improve their body composition with more muscle mass and
- 41:42less fat mass.
- 41:43So that would be kind of the take home message.
- 41:45Yeah.
- 41:45think there's one thing that people should keep in mind.
- 41:49it is bio-individuality.
- 41:51It's that, you know, we're surrounded now with social media.
- 41:54You're seeing not just like the people in your community.
- 41:57You're literally seeing everybody around the world.
- 42:00And not everybody's always sharing exactly what they're doing.
- 42:03But even if they are, you're looking at it you're like, but this worked for them.
- 42:07And so this, this exact plan should work for me too.
- 42:10And it doesn't always work that way.
- 42:13In fact, it never works that way.
- 42:15It almost never works that way.
- 42:16places.
- 42:17So you gotta tweak it for yourself.
- 42:19Somebody might have some good strategies, they might have a good baseline plan, but you still have to figure out how to optimize for yourself.
- 42:26You still need to learn how to listen to your own body and your body's cue.
- 42:30and it comes back to the diagnostics.
- 42:32Who are you?
- 42:32What are your genetics?
- 42:33What are your fat genes?
- 42:35know, what are your genes around being able to control appetite and leptin?
- 42:39All these are genetically predetermined.
- 42:41So if you understand your genetics, if you really understand yourself, you can actually get where you want to go.
- 42:47If you just keep throwing stuff at the wall and it's not working, it's not working, and now I just lost 20 pounds, but now I regained it all.
- 42:53This is what happens when you don't fully understand what's going on.
- 42:56So I would really encourage you to do the deconstruction, understand your N of one, you, and then you can get a great plan.
- 43:03Yeah.
- 43:03oh
