- 0:03>> Dr. Terry Simpson: Hey, welcome back to Fork U Fork University, where
- 0:06we make sense of the badness, bust a few myths and show
- 0:09you how food may not be medicine, but it is
- 0:12important. I am your chief medical
- 0:14explanationist and host, doctor Terry Simpson.
- 0:20Today we're going to tackle the topic that is both timely
- 0:23and critical. We're going to talk about why
- 0:25chiropractors and other functional medicine practitioners
- 0:28take stool samples and recommend rather
- 0:31restrictive diets for autoimmune disease like Hashimoto's
- 0:34thyroiditis, rheumatoid arthritis and
- 0:37others. Let's start with the simple truth.
- 0:40Autoimmune diseases are complex,
- 0:43chronic conditions that require careful
- 0:45management. Conditions like Hashimoto's
- 0:48thyroiditis, where the immune system attacks the thyroid
- 0:50gland leading to hypothyroidism, are
- 0:53not easily fixed by dietary changes
- 0:56or gut health gimmicks. Yet there are people
- 0:59out there, often without a medical degree,
- 1:02claiming they can diagnose and treat these
- 1:05serious conditions by analyzing your
- 1:08stool or putting you on a restrictive
- 1:10diet. Let me be clear, these
- 1:13folks are quacks.
- 1:19Now don't get me wrong. Diet does
- 1:22play a role in your health, and there's growing
- 1:25evidence that our gut microbiome is important to
- 1:28overall well being. But when it comes to autoimmune
- 1:31disease, the science is far more
- 1:33nuanced than what these so called experts would have
- 1:36you believe. Let's break it down.
- 1:39First, let's talk about the chiropractors and other
- 1:42alternative health practitioners who claim they
- 1:45can diagnose autoimmune diseases
- 1:48and their treatments by analyzing stool
- 1:50samples. The idea here is by looking at the
- 1:53bacteria in your gut, they can identify
- 1:56imbalances or what we call
- 1:58dysbiosis, which claims to be the root
- 2:01cause of your autoimmune disease.
- 2:04They will tell you by adjusting your gut bacteria,
- 2:07usually with expensive probiotics, supplements
- 2:10and restrictive diets, you can cure or
- 2:13manage your disease. Heres the problem.
- 2:16While its true that our gut microbiome influences
- 2:19many aspects of our health, the science
- 2:22of specific gut bacteria to
- 2:25autoimmune disease is in its
- 2:27infancy. We do not yet have a
- 2:30clear understanding of how to manipulate the gut
- 2:33microbiome and treat autoimmune
- 2:36diseases effectively. The studies
- 2:39that do exist are often preliminary,
- 2:41done with mice or rats, or based
- 2:44on small sample sizes of human beings,
- 2:47and to extrapolate that limited amount of
- 2:50data into broad claims that gut health
- 2:53can cure or significantly
- 2:55change autoimmune disease is just
- 2:58not only premature, it's
- 3:00irresponsible. I'll give you an example.
- 3:04A study published in Nature Reviews Immunology in
- 3:062015, highlighted the complexity
- 3:09of gut immune interaction and noted
- 3:12that the gut microbiome likely plays a role
- 3:15in the development of autoimmune disease, but we are far
- 3:18from understanding how to target it for treatment.
- 3:21More recent research, such as that from the Journal of
- 3:24Autoimmunology in 2021
- 3:26supports that view, acknowledging that
- 3:29while gut dysbiosis is associated with
- 3:32autoimmune conditions, there is no one size fits
- 3:35all pattern, no one solution to fixing it
- 3:38through diets or gut probiotics.
- 3:41Furthermore, recent
- 3:43studies show that stool samples
- 3:46may not be helpful in termining the
- 3:49gut microbiome, especially in the context of
- 3:51determining autoimmune disease. A study by Lew
- 3:54et al found that fecal bacteria
- 3:57significantly different from the swab and tissue
- 4:00microbiota in patients with irritable bowel
- 4:02syndrome versus if we lavage the
- 4:05gut with like a colonoscopy so we can get
- 4:07truer to where anatomically
- 4:10the gut microbiome is. Let's be clear,
- 4:13the chiris factors will have you take a stool sample.
- 4:16However, the microbacteria which are in your
- 4:19gut that might be affecting it are going to be
- 4:21completely different if you go up further into the colon,
- 4:24as if you use a colonoscopy.
- 4:26Leviathan et al demonstrated stool samples
- 4:29are a poor proxy for what we call
- 4:32the inner colonic microbiome, or the bacteria of your
- 4:35gut. Their study showed that those samples that were
- 4:37taken very carefully significantly
- 4:40differ from those in a stool sample.
- 4:44It's just not the same
- 4:46thing. So in summary, stool samples may not be
- 4:49helpful for determining gut microbiome, as if gut
- 4:52microbiome could actually fix autoimmune
- 4:55diseases. So when someone takes
- 4:58your stool sample and say they can cure your
- 5:01hashimoto's or your lupus or your rheumatoid
- 5:04arthritis by tweaking your diet and selling you rather
- 5:07expensive stuff, theyre selling you
- 5:09snake oil. The truth is, managing
- 5:12autoimmune diseases requires a
- 5:15multifaceted approach and typically involves
- 5:17medications such as synthetic thyroid hormone. In the case of
- 5:20Hashimoto's and close monitoring by
- 5:23healthcare professionals, this is something that these
- 5:26people have no clinical idea about. They weren't
- 5:29trained clinically. Let's be clear, a
- 5:31chiropractor doesn't have the extensive
- 5:34clinical training that
- 5:37a physician does. Consider this. Your average medical
- 5:40student not only spends two years studying basic clinical
- 5:43medicine in a textbook, they then go on to the wards
- 5:46of hospitals where they see the most complex
- 5:49patients on the planet treated by the most
- 5:52well known physicians on the wards, then they
- 5:54go out and do a three year internal
- 5:57medicine internship in some of the brightest
- 6:00hospitals in the nation, taught by some of the best scientists.
- 6:03Then they go on to do a fellowship in rheumatology.
- 6:06Compare that to a chiropractor who has two years
- 6:09of neck cracking. Let's move on.
- 6:12Let's talk about the restrictive diets that are pushed by these
- 6:14practitioners. You've probably heard of the autoimmune protocol,
- 6:17or the AIP diet, which eliminates a wide
- 6:20range of foods, from grains and legumes to
- 6:22nightshades, dairy, eggs, nuts and seeds.
- 6:25The idea is that these foods could trigger some
- 6:28immune response, and by eliminating them, you can reduce
- 6:31inflammation and manage your autoimmune disease. Sounds
- 6:34promising, right? The problem is that the AIP
- 6:37diet is based on very, very weak
- 6:39science. While some people report
- 6:42feeling better on these diets, we have to ask,
- 6:45why? Is it limited by their eliminating
- 6:48food groups? Or is it simply
- 6:50that their disease comes and
- 6:53goes, relapses, returns,
- 6:57goes into remission, which is the nature of
- 6:59autoimmune disease. Let's look at a study published
- 7:02in nutrients in 2017. They looked at the
- 7:05AIP diet in the context of inflammatory bowel
- 7:08disease, another autoimmune disease, and found that while
- 7:11some patients experienced symptomatic
- 7:13relief, the study was small, lacked the control.
- 7:16Group, couldn't possibly say it was the AIP diethouse,
- 7:19etcetera. There are other
- 7:22diets that are used for this. Typically what they will do is, let's
- 7:25eliminate dairy. I can tell you, based on
- 7:28what we have seen and has been told to us, that
- 7:31their formula comes down to this. Besides selling you the
- 7:34really expensive supplements, the really expensive
- 7:36probiotics for which there is zero, and I
- 7:39mean zero evidence, they will tell you to
- 7:42eliminate dairy, eliminate gluten, and
- 7:45go on one of these highly restrictive diets, like, oh,
- 7:48the whole 30 diet or the AIP diet, and
- 7:51then they'll say, look, you're cured.
- 7:54Let's be very clear.
- 7:57When you have a relapsing disease like
- 7:59rheumatoid arthritis, you are going to
- 8:02have times when it's going to relapse for unknown reasons. It's not your
- 8:05gut microbiome. it will just does it over
- 8:08time. And if you have rheumatoid arthritis aren't
- 8:11getting appropriate clinical medicine,
- 8:14you will end up with more joint destruction and more
- 8:16problems. And let's put it this way, once the
- 8:19chiropractors have you out of their office, they're not going to
- 8:22be able to follow your inflammation. They're not going to be able to
- 8:25follow your joint destruction. They
- 8:27don't care. They just want to sell you
- 8:30their exceedingly expensive tests, which cost up to
- 8:33dollar 500 for a stool sample, and their supplements,
- 8:36which are like $90 a point at each.
- 8:40So what's the bottom line? When it comes to
- 8:42autoimmune diseases? Beware of anyone who promises a
- 8:45cure or significant management from
- 8:48diet and supplements alone. The science
- 8:51simply doesn't support these claims.
- 8:54Instead, focus on working with a
- 8:56qualified healthcare provider. If you have a
- 8:59rheumatologic disease, see a board certified
- 9:02rheumatologist. If you have a problem with your
- 9:04thyroid, see a board certified
- 9:06endocrinologist. Because these non qualified
- 9:09nurses, dentists and chiropractors who get their
- 9:12little functional medicine degree, which by the way, they focus
- 9:15on thyroid disease and they focus poorly on
- 9:18it, they simply don't lack the clinical
- 9:21experience, the comprehensive literature knowledge,
- 9:24and they end up keeping people from
- 9:27having support, the support you need
- 9:30for an exceedingly complex medical
- 9:33problem. Alright,
- 9:35managing autoimmune diseases is a
- 9:38marathon. It's not a sprint. It
- 9:41requires thoughtful, multifaceted
- 9:43approach, not a quick fix. Don't let
- 9:46those quacks steer you off courses with
- 9:50promises far too good to be
- 9:52true. I hope today's episode has shed a
- 9:55little light on the dangers of these so called miracle cures with
- 9:58miracle diets and the quackery. That
- 10:00sounds too good. This reminds me of one thing
- 10:03else. Do you remember when chiropractors
- 10:06used to say the cure of all their diseases is in
- 10:09sublaxations of the spine, which they could never
- 10:12see, could never demonstrate, and spent years telling you to come back
- 10:15every week? Now they're going to the gut. They can't
- 10:17see it. They have sort of a science y test, and they are trying to
- 10:20convince you they alone have the answer to the root cause
- 10:23of disease, as opposed to all of the
- 10:26people who seriously study this and follow their
- 10:29patients intently and publish in the
- 10:32literature, the only root cause of
- 10:35disease that these functional medicine practitioners have
- 10:37found is your wallet. I would be
- 10:40careful of them. See board certified people to make
- 10:42sure thanks for joining me on this episode of
- 10:45fork you where we try to make sense of the madness.
- 10:48I'm your host, doctor Terry Simpson. This has been written and

- 11:21Hey, Ivo, did you ever find some sort of
- 11:24thing that we could use to, cure everything in that herb garden of
- 11:27yours?
- 11:29>> Emily: Ivo is on vacation in Portugal. Terry, this is
- 11:32Emily filling in for him on the podcasting front, but he
- 11:35didn't mention anything about watering his plants.
