- 0:00Hello, everyone. This is Jacob Emerson with the
- 0:02Becker's Payer Issues podcast.
- 0:04Thrilled today to be joined by Emily Porter,
- 0:06who is the vice president of external affairs
- 0:09at Sidecar Health. Emily, thank you so much
- 0:11for taking the time to be with me
- 0:13on the podcast today.
- 0:14Sure. Thank you for having me.
- 0:16So before we dive into everything we wanna
- 0:17talk with you about, Emily, and, a big
- 0:19policy change going on in Washington, can you
- 0:21talk to us a little bit about your
- 0:23career background and what it is that you're
- 0:25doing today at Sidecar Health?
- 0:28Sure. So I,
- 0:30I got my my career start at a
- 0:32think tank in Washington DC.
- 0:35I won't say how many decades ago, but
- 0:37many years ago.
- 0:39And then I worked on Capitol Hill for
- 0:42about a decade. So I spent most
- 0:44of that decade working for,
- 0:47former congressman John Boehner
- 0:49and worked for him in the majority leader's
- 0:51office and in his his role in the
- 0:53leadership generally
- 0:55as minority leader, and then I I ended
- 0:58my role with him in the speaker's office.
- 0:59So I left The Hill, about twelve years
- 1:02ago, and then I went to work at
- 1:04a small boutique lobbying
- 1:06firm in Downtown
- 1:07DC where many of our clients were all
- 1:10in the health care sector, so anything from
- 1:12insurance to hospitals to medical device.
- 1:15And then one of those clients that we
- 1:17had was, Sidecar Health.
- 1:19And I was very intrigued at the outset
- 1:22about what Sitecore Health has to offer. It
- 1:24was truly the only thing I had seen
- 1:27in my career that
- 1:29actually creates
- 1:31a market incentive for consumers to be engaged
- 1:34in probably the most meaningful way,
- 1:36that they've ever been engaged in health care.
- 1:39And
- 1:40that was that was really exciting for me,
- 1:41having been on Capitol Hill during
- 1:44the Medicare part d program consideration,
- 1:47during the
- 1:48Affordable Care Act consideration and multiple other pieces
- 1:51of of big legislation that I'm sure we're
- 1:53all familiar with. But this was really something
- 1:55that
- 1:56I thought was the most interesting in terms
- 1:58of not requiring
- 2:00any sort of government
- 2:01subsidy or tax credit or some of the
- 2:03more,
- 2:04some of the more obvious
- 2:06carrots and sticks that we see in other
- 2:08legislation.
- 2:10Fantastic. So you've got a long career in
- 2:12in politics and in DC, Emily, and that's
- 2:14why we wanted to come to you to
- 2:16ask specifically about
- 2:17president Trump signing a new executive order in
- 2:21February aimed at cracking down on price transparency
- 2:24requirements for hospitals and insurers. And that, of
- 2:27course, follows his original executive order on health
- 2:30care price transparency back in 2019.
- 2:33So I think the natural question there is,
- 2:35what is the difference? What changes
- 2:38do insurers need to know about this new
- 2:41EO?
- 2:42What's what does it do differently?
- 2:44So as you as you articulated, Jacob, in
- 2:47the previous executive order, health plans and hospitals
- 2:50obviously were both swept up in that, but
- 2:51health plans were instructed to
- 2:54post their negotiated rates with providers and their
- 2:57out of network
- 2:58payments to providers and then the actual prices
- 3:00that they paid for prescription drugs.
- 3:03And then they had to maintain an online
- 3:05tool in order to permit
- 3:08customers to access that particular pricing information.
- 3:12This executive order,
- 3:14which we're very supportive of, obviously, at Sidecar
- 3:17Health because we we are a major medical
- 3:19product based on price transparency
- 3:21and giving consumer information. But this executive order
- 3:24requires
- 3:25the,
- 3:26requires the plans and hospitals
- 3:28to put up actual pricing. So
- 3:31previously, I think we've seen a lot of
- 3:33estimates.
- 3:33And so now there is a requirement that
- 3:36actual prices
- 3:38must be disclosed,
- 3:40and it requires that the department determine what
- 3:43kinds of enforcement policies
- 3:45they want to implement
- 3:46to ensure that the providers are compliant.
- 3:50So it will mean that not only will
- 3:52health care entities have to determine how they
- 3:54disclose a price in
- 3:56Toledo, Ohio
- 3:57or
- 3:58Jacksonville, Florida, but but that becomes, I think,
- 4:01a very meaningful and and different way that
- 4:03for the consumer
- 4:05and the plans to figure out how to
- 4:07share that pricing so that it's constructive and
- 4:09and useful.
- 4:11Absolutely. So more concrete information,
- 4:14actual prices, not estimates being required from insurers
- 4:17across the country.
- 4:19And so you mentioned insurers have been required
- 4:21to disclose a lot of this pricing information
- 4:24starting in 2022.
- 4:26It staggered from there over the years.
- 4:28So what's compliance been like, from your perspective,
- 4:31Emily, up until now across the industry?
- 4:34Are are insurers actually following these rules?
- 4:38So first, I'll say that, just
- 4:41go on with your question here in a
- 4:43second. But first, I I will say I
- 4:44do think
- 4:45that insurers are supportive of transparency. I think
- 4:48the crux of the issue has always been
- 4:51applicability
- 4:53and making sure that it's applicable in a
- 4:56uniform way and having the kind of data
- 4:58that
- 4:59that is displayed in a consistent
- 5:01consumer friendly format. I think that's where it
- 5:04can be a real game changer for the
- 5:05consumer, and we see this every day in
- 5:08what we do at Sycor Health. I think
- 5:11I think under these particular
- 5:14executive orders so first of all, the hospitals
- 5:16were required to disclose that information prior to
- 5:19insurers
- 5:20being required to in 2022.
- 5:22So I think you've seen some some delay,
- 5:25in enforcement
- 5:27really because I think in part, it was
- 5:28tied up in courts for a while.
- 5:30And but I think we all share the
- 5:32goals, obviously, of giving consumers the information to
- 5:35make good choices, but there has been a
- 5:38bit of a lag in the Biden administration.
- 5:40But I think there's always going to be
- 5:42different priorities
- 5:44depending on who is in the White House
- 5:46and what the directives
- 5:48are from the senior folks at those
- 5:50departments. And I think
- 5:52in this particular instance,
- 5:55you know, there's maybe not been
- 5:59some of the processing of pricing data
- 6:02that has been a priority
- 6:04when you think about
- 6:06all of the other issues that both hospitals
- 6:10and insurers are trying
- 6:12to work on every single day that really
- 6:14has nothing to do with price transparency, but
- 6:17is trying to is trying to work on
- 6:19just other issues related to Medicare or Medicaid
- 6:22and and different payments. So but I I
- 6:24do think,
- 6:25overall,
- 6:27hospitals and insurers, like, we very much value
- 6:29our hospital partners. And when it comes to
- 6:31giving consumers
- 6:32pricing information,
- 6:34we share those goals,
- 6:36but we really just wanna make the experience
- 6:38for the consumer smooth. And I think that's
- 6:40where
- 6:41sharing actual data
- 6:43is really gonna be a game changer in
- 6:45terms of, let's call this,
- 6:48executive order two point o, in terms of
- 6:50how this second executive order really sort of
- 6:53forces
- 6:54that positive direction, I think, forward for the
- 6:56next generation of of pricing in health care.
- 7:00Absolutely. Absolutely. And you mentioned some of the
- 7:02challenges and delays we've seen in enforcement. And
- 7:05and just so our audience is aware for
- 7:06context, we know that 18 hospitals have been
- 7:09fined for for noncompliance with these rules since
- 7:12they first came out. No insurers have been
- 7:14by CMS. So so that's where things currently
- 7:16stand.
- 7:17But but, Emily, you mentioned that this all
- 7:19comes back down to making things easier in
- 7:21health care for the consumer. So
- 7:23where do things stand in terms of, third
- 7:26party companies and consumers' ability
- 7:29to to use this data, and to know
- 7:31what their health care costs will be before
- 7:34they they go to the doctor's office?
- 7:36Sure. So I think it's always good for
- 7:39consumers and or for hospitals and insurers to
- 7:42provide this data, as you say, in a
- 7:43more actionable way. I think that helps move
- 7:46the ball forward and creates a more educated
- 7:49health care population. And this really is
- 7:52the next generation of evaluating what those next
- 7:54steps are. And in any sort of
- 7:56public policy,
- 7:59any sort of public policy development, there's always
- 8:01going to be room for progress.
- 8:04And I think this exercise
- 8:06from the Trump administration
- 8:08is
- 8:09a smart exercise in terms of
- 8:12how to make that public policy more effective
- 8:15for the consumer. I think we wanna make
- 8:17sure providers obviously,
- 8:20are providing that information
- 8:22in a way that is digestible. Consumers are
- 8:25savvy, and that's something that I've learned a
- 8:28lot about at SacroHealth, but they like having
- 8:30additional information to the extent that they're able
- 8:33and being able to share that information in
- 8:35a way that is
- 8:38is digestible.
- 8:39And like I said earlier, in a uniform
- 8:41manner,
- 8:42in a way that they're buying anything else
- 8:44throughout their daily lives, that's a real that's
- 8:47a real,
- 8:49step forward. And we wanna make sure, obviously,
- 8:51providers
- 8:52that they don't need to go through prior
- 8:54authorization and they don't have to pay,
- 8:57or that we don't have to pay with
- 8:58a middleman. There should be immediate payment, and
- 9:01there is a privilege for the customer to
- 9:02be able to choose what they want. And
- 9:04sharing
- 9:05pricing
- 9:06is part of the process or a step
- 9:09in giving them that privilege to be able
- 9:11to go wherever they wanna go. I do
- 9:14think sharing this kind of data also opens
- 9:16the door for other innovation
- 9:19in the healthcare sector, which we think innovation
- 9:21is is always a good thing.
- 9:23And it will probably result in more opportunities
- 9:26for,
- 9:27you know, think about the the Google Healths
- 9:30and the Amazons of the world to create
- 9:32other innovative models that help give consumers the
- 9:36tools they need to be
- 9:38smarter and better consumers. And whether it's through
- 9:42artificial intelligence
- 9:44or
- 9:45website
- 9:46tools or just other,
- 9:48more intuitive ways to get information,
- 9:51I think that that that is always gonna
- 9:53be a positive step forward.
- 9:55Absolutely. Yeah. It is very exciting to think
- 9:57about what could come from this new executive
- 10:00order within the private sector. Yeah. But, Emily,
- 10:03before we go, I think would you educate
- 10:04our audience on on the Sidecar Health model
- 10:07and how you all are using the price
- 10:09transparency
- 10:10data today?
- 10:12Sure thing. So Sidecar Health is a large
- 10:14group major medical insurance product.
- 10:18We give consumers
- 10:19pricing information so that they know what every
- 10:22single thing in the health care system
- 10:24can cost, and we give them that data
- 10:27on our website. And so they're able to
- 10:29compare,
- 10:30and we do this for all 170,000
- 10:32things you can do in the healthcare system,
- 10:34as well as for any FDA approved drug.
- 10:37So the consumers can compare
- 10:40and determine where they want to go because
- 10:42we don't have a lot of the more
- 10:45onerous
- 10:45or frustrating things that that more traditional insurers
- 10:49have. We don't have a network. We don't
- 10:51have prior authorization.
- 10:53We don't have set therapy or formularies. And
- 10:55again, we just don't have some of the
- 10:57more burdensome
- 10:58things. So we give the cons we give
- 11:00the consumer,
- 11:02a budget that they need to be able
- 11:03to shop, and then it's up to them.
- 11:05If they wanna go someplace that is over
- 11:07the budget that we're giving them, that's totally
- 11:10fine. That's their prerogative. But if they want
- 11:12to go someplace that that costs less, we're
- 11:15gonna let them keep part of that savings.
- 11:18And so it really is a game changer
- 11:19when you think about the consumer having more
- 11:22skin in the game
- 11:23and and being able to consume health care,
- 11:25like I said earlier, in a meaningful way,
- 11:27in a way they consume everything else in
- 11:29their lives. But we're really excited about what
- 11:31we've seen from consumers so far. Like I
- 11:33said, consumers are really smart and they like
- 11:36having the data. And we're seeing about 90%
- 11:38of our claims come back where consumers
- 11:41are getting their services either at the budget
- 11:44that we give them or slightly above with
- 11:47maybe what we consider like a co pay
- 11:50amount, like $15 or $20
- 11:52but they're choosing also to go to sites
- 11:53of care that are less costly and
- 11:56are finding a lot of success also at
- 11:58the pharmacy and saving a lot of money
- 12:00and being able
- 12:02to, you know, kind of almost compete a
- 12:05little bit sometimes with their coworkers. I know
- 12:06we certainly do that inside the company in
- 12:08terms of where we found good prices. And
- 12:11it really is an eye opening and exciting
- 12:13experience for both just us, but also the
- 12:16consumer. And we love hearing love hearing all
- 12:18the feedback.
- 12:19Well, appreciate that overview, Emily, and it'll be
- 12:21fascinating to watch as a sidecar continues to
- 12:24disrupt across the industry.
- 12:26Before we go, we've got a lot of
- 12:28health plan leaders listening in. Any final thoughts
- 12:31or final bits of advice you wanna offer
- 12:34them in terms of, this this new executive
- 12:36order and what that means for price transparency
- 12:39moving forward?
- 12:40Well, I think there's I do think sometimes
- 12:43there's an impression that perhaps
- 12:46there's, I think, a lot of opposition
- 12:48to price transparency depending on what industry. And
- 12:51and I think I'd like to maybe just
- 12:53share. I don't think that's true. I think
- 12:55there is a lot of support for transparency.
- 12:58I think that
- 12:59there is a lot of
- 13:01there are a lot of federal and state
- 13:03programs that both hospitals and insurers are trying
- 13:05to comply with. And so there's a little
- 13:07bit of a a push pull between
- 13:10between where some of the attention can be
- 13:12focused. But I do think that
- 13:14insurers and, like I said earlier, our hospital
- 13:17partners,
- 13:18they're supportive of giving the consumers
- 13:20more information
- 13:21and more,
- 13:23more information that'll make them better consumers because
- 13:27we want the consumer to have a smooth
- 13:29experience. And so I think we do all
- 13:31truly have,
- 13:32you know, shared goals in that area. This
- 13:34probably I'm maybe giving you more of a
- 13:37a kumbaya answer than you wanted, but but
- 13:39I do think sometimes in the field, there's
- 13:41a little bit of a, the sense that
- 13:43there's consternation. And I think in this case,
- 13:46there isn't. I think because the prices have
- 13:48been somewhat hidden for quite some time that
- 13:51it's
- 13:52a different administrative role to make some of
- 13:55that pricing more public, but we're really excited
- 13:57to have allies in that effort. And we're
- 13:59very grateful
- 14:01to have all the providers that that we
- 14:03have that that do take Sitecore Health customers
- 14:05because we wanna make that experience really
- 14:07enlightening and smooth for the consumer.
- 14:11Fantastic. Well, Emily, thank you so much for
- 14:13taking the time to sit down with us
- 14:14and for sharing your insights with our listeners.
- 14:16We really appreciate it. Thank you so much.
