- 0:38This is Laura Dirda with the Becker's Healthcare
- 0:40podcast. I'm thrilled today to be joined by
- 0:42Emily Giacchi, president of Northwestern Medicine, Delanor Hospital.
- 0:46Emily, it's a pleasure to have you on
- 0:48the podcast today. Thank you, Laura. Great to
- 0:50be here. Happy New Year. Absolutely. Now I'm
- 0:53excited for our conversation because I know there's
- 0:55so much happening in health care and, you
- 0:57know, you're doing some really unique and interesting
- 0:59things, especially around quality of care and everything
- 1:01you're doing to serve the community better in
- 1:032026. And so I'm looking forward to learn
- 1:05more about that. But before we dive in,
- 1:07can you tell me a little bit more
- 1:08about yourself and Del Norte Hospital?
- 1:11Certainly.
- 1:13I originally hail from the great state of
- 1:15Michigan,
- 1:17but have lived in the Chicagoland area since
- 1:19about 2008,
- 1:21working in health care,
- 1:24for
- 1:25the entire time,
- 1:27I've been in the Chicagoland area.
- 1:29I've been with Northwestern Medicine,
- 1:31about a year and a half.
- 1:33It's been wonderful,
- 1:35and really have just
- 1:37been so honored to serve here at Geneva
- 1:40at, Del Norte Hospital in the Fox Valley.
- 1:43We are a 159
- 1:45bed level two trauma
- 1:48hospital. I like to say that Del Norte
- 1:50acts like a bigger hospital than it is,
- 1:52with an incredibly robust,
- 1:55surgical and outpatient platform.
- 1:58We've got a incredibly
- 2:00beautiful, recently renovated,
- 2:03cancer center,
- 2:04where we're routinely serving over a 100 patients
- 2:07a day between infusion
- 2:09and radiation oncology services.
- 2:12We have a family medicine residency,
- 2:15program on this campus, so we're certainly doing
- 2:17some teaching here.
- 2:19And it's growing. We have
- 2:21renovation projects,
- 2:23you know, similar to a lot of other
- 2:24hospitals,
- 2:25underway every single day. So, it's a it's
- 2:28a wonderful place serving a a wonderful growing
- 2:30community.
- 2:32That's fantastic to hear. And, you know, I
- 2:34I think it's so important to have that
- 2:37ability,
- 2:37to serve the community and understand what they
- 2:39need to live there and really,
- 2:42be integrated into everything that that's happening on
- 2:45a local level. You know, when you think
- 2:47back the last year or so, what was
- 2:48the most important initiative you led? What did
- 2:50you do, and what were the results?
- 2:53Great question. So,
- 2:55I, I actually joined
- 2:58Northwestern and Del Norte in August 2024. So
- 3:012025
- 3:02was really my my first year here. So,
- 3:06I would be
- 3:08remiss if I didn't say going through,
- 3:11my team, this incredible leadership transition,
- 3:14so smoothly with such incredible results is something
- 3:17I'm so proud of.
- 3:19We have an incredible executive team here at
- 3:21Del Norte and
- 3:23had a lot of good opportunity to promote
- 3:25a lot of folks from within,
- 3:28make sure that we are developing in our
- 3:31leaders. We focused a lot on leadership development,
- 3:34just to make sure that, you know, the
- 3:36reality of a major leadership transition is a
- 3:39big,
- 3:40shift for a culture and for an organization.
- 3:43And I'm just incredibly proud of how well
- 3:45that went and,
- 3:47how warm of a welcome I received, and
- 3:49I think we're just,
- 3:51the Del Norte's done incredible work
- 3:54predating me, and I'm just glad to be
- 3:56a part of it going forward.
- 3:57One of the things that I'm most proud
- 3:59of,
- 4:00is, you know, what we're here to do
- 4:03is serve our patients,
- 4:05and our quality performance.
- 4:07My team,
- 4:09has a target, and then they look at
- 4:10the max target and set that as their
- 4:12target. So, we perform just incredibly well across
- 4:16the board on many of our our health
- 4:18outcomes and quality scores,
- 4:21namely mortality,
- 4:22safety,
- 4:24efficiency,
- 4:24equity.
- 4:26In our Visiant,
- 4:27cohort of complex,
- 4:29medical centers,
- 4:31we finished the year ranking number three out
- 4:33of 211
- 4:34hospitals.
- 4:35And, again, that is meeting top decile or
- 4:38above in all of our key metrics. So,
- 4:41it you know, when you when you score
- 4:43that high, maintaining it then becomes the next
- 4:47challenge. But it's something we're just incredibly proud
- 4:49of, and it's one patient
- 4:51at a time every day, and it's something
- 4:53we talk about every single day.
- 4:56Absolutely. I I love that. And, you know,
- 4:58being able to have that type of quality
- 5:00care and, you know, focus
- 5:03in in putting some of those,
- 5:05targets in place for the team and knowing
- 5:07that it's gonna improve,
- 5:09outcomes and make a big difference for the
- 5:11community is so, so helpful. I'm curious, you
- 5:14know, when you were looking at,
- 5:16putting some more of those scores in place
- 5:18and following them and tracking them and thinking
- 5:21about safety as well as outcomes and equity
- 5:24and everything else,
- 5:26how did you really keep your team focused
- 5:28and and help them,
- 5:30you know, work through any challenges so that
- 5:32they could come out the other end, making
- 5:34some big improvements?
- 5:36Yeah. There was a really great infrastructure here
- 5:39when I arrived,
- 5:40and I would say we just continue to
- 5:42sharpen the pencil.
- 5:44We have our, you
- 5:46know, safety and quality performance improvement committee.
- 5:49We have,
- 5:51a number of kind of department and and
- 5:54service line specific,
- 5:56group work groups as well. One thing that
- 5:58I think really helped us move the needle,
- 6:01we have a a top five, our safety
- 6:04and quality top five every single year. And
- 6:06those are the top five things that drive
- 6:08our focus and keep us focused.
- 6:11Not many have changed this year, but we
- 6:13have falls. We have ambulation,
- 6:15keeping our patients moving.
- 6:17We have safe handoffs.
- 6:19You know, our top five is, has
- 6:21a team around each one of our core
- 6:24measures,
- 6:25and I think that that allows us to
- 6:26take the number of things that are thrown
- 6:28at our leaders all the time,
- 6:30to really focus on what matters,
- 6:33not only for the experience and the outcomes
- 6:36of our patients, but can also drive the
- 6:38results.
- 6:39Got it. That makes a lot of sense.
- 6:41Thank you so much for digging a bit
- 6:42deeper there. Now looking ahead, what are some
- 6:45of the big priorities and headwinds you're focused
- 6:47on for 2026?
- 6:49Yeah. I mean, I think like many organizations
- 6:51and, you know, speaking from the Far West
- 6:54Suburbs of the Chicagoland area,
- 6:57all of our emergency rooms are just getting
- 7:00slammed right now.
- 7:02And it feels like and and the data
- 7:04is telling us that, you know, volume, whether
- 7:07it's stable or up,
- 7:09acuity is certainly up. So I would say
- 7:11a a big conversation
- 7:12on our leadership teams is around throughput. How
- 7:15are we managing?
- 7:16We're we're
- 7:17running out of beds faster than we thought
- 7:20we would, namely because our patients are sicker.
- 7:23So how are we managing those cases? How
- 7:25are we bringing, you know, a full team
- 7:28together to make sure we have a good
- 7:29care plan?
- 7:31Actually, our team in the ED is doing
- 7:33just such a phenomenal job. We're we're starting
- 7:36care in the in the waiting room.
- 7:38And not only do we have two kind
- 7:40of key triage spots, in our waiting room,
- 7:42but we're actually caring for patients and discharging
- 7:45patients who otherwise
- 7:47don't need to be there,
- 7:49from the waiting room. We are just trying
- 7:51to move care up as much as we
- 7:52can because, we're we're trying to manage that
- 7:55aspect of throughput. So I'd say that's a
- 7:58a a daily, hourly conversation,
- 8:02and making sure that both we we are
- 8:04staffed appropriately to manage, the the changing nature
- 8:07of this care, that we're doing it in
- 8:09a safe way,
- 8:10and that we have,
- 8:12the wellness of our staff in mind, making
- 8:14sure that we're appropriately
- 8:16staffed and and making sure that people are
- 8:19are getting what they need through every shift.
- 8:22That that's a a very
- 8:23big conversation, especially smack dab in the middle
- 8:26of a really tough flu season.
- 8:29So I'd say operationally, that's
- 8:31the the the, top priority. A top headwind,
- 8:36I'm I'm confident all of my colleagues around
- 8:38the country are keeping an eye on our
- 8:40our state and our federal health care policy
- 8:42changes.
- 8:43You know, just I think over the weekend,
- 8:45the rural transformation fund,
- 8:48out of Washington was announced.
- 8:51We're just starting to get information at the
- 8:53state level on what that means for the
- 8:55rural,
- 8:57the rural
- 8:58transformation, dollars that will go to some of
- 9:00the rural hospitals.
- 9:01I don't even know what that means for
- 9:02us locally yet,
- 9:04but I can tell you we're all keeping
- 9:06a really close eye on,
- 9:08what's changing,
- 9:09the status of our Medicaid patients,
- 9:12the status of our state budgets.
- 9:14These are all and I call them you
- 9:17know, when I we look at our financials,
- 9:19even if our volume is up, we have
- 9:21to understand the impact of these headwinds and
- 9:24what our managing our expenses
- 9:26looks like going forward just so we can
- 9:28navigate those waters.
- 9:31Absolutely. That's such a great point. And, you
- 9:33know, it's really interesting to hear you talk
- 9:34about all of these things because I know
- 9:36especially when you are thinking about,
- 9:39the legislative changes, those policies could be updated.
- 9:42At any point, you have to stay nimble.
- 9:44And then thinking about the financial challenges, but
- 9:47also, you know, potentially getting some, additional funds
- 9:50for the rural health transformation fund would be
- 9:52exciting. So
- 9:53I'm curious. How are you thinking about,
- 9:56you know, the next twelve months or so?
- 9:58What is the hardest thing you're gonna have
- 10:00to do? How are you looking at all
- 10:02of these things and, continuing to to pivot
- 10:05it and grow your teams?
- 10:07The hardest thing? I think all of us
- 10:09jump into health care because every day, is
- 10:12game day. That's what I like to say.
- 10:14Every day is interesting in our jobs. I
- 10:16think the hardest thing continues to be prioritization.
- 10:20There are so many areas, so many programs,
- 10:23so many teams that, you know, need investment.
- 10:26There are so many great innovations coming down
- 10:28the path.
- 10:29There's replacements we need to do.
- 10:33Prioritizing
- 10:35those projects, I think, for a leadership team
- 10:37becomes the hardest part of the job.
- 10:39There's a finite number of dollars, especially when
- 10:41you're in a health system. When you're thinking
- 10:43about things like strategic capital, you can't fund
- 10:46it all.
- 10:47So I think what we're trying to do
- 10:49is really roll up our sleeves and understand,
- 10:52the why. You can't always prioritize things that
- 10:56have a positive return on the investment.
- 10:59Some things are safety issues. Some things are
- 11:01about meeting needs of our community or meeting
- 11:03needs of our campus.
- 11:05So we're really focused on making sure we
- 11:07have all the information and all of the
- 11:09data
- 11:10so we can make the best informed,
- 11:13decisions when it comes to prior priorities on
- 11:15this campus.
- 11:17That's fascinating to hear. And and I really
- 11:19like your point about, staying focused and having
- 11:22the right priorities. And when you talk about
- 11:24having the information, having the data, it it
- 11:26more power at your fingertips there. What does
- 11:28it look like for you, or or has
- 11:30the journey been to,
- 11:32incorporate that data into your decision making process
- 11:34and making sure on the technology side,
- 11:37you know, all all the data and,
- 11:39technology that goes into it is really strong
- 11:42and dependable?
- 11:44Yeah. I mean,
- 11:45yeah,
- 11:46ask a lot of good questions,
- 11:49listen as best we can,
- 11:51and make sure we have the right people
- 11:52at the table.
- 11:54Again, you
- 11:55know, when you're a part of a health
- 11:57system like Northwestern,
- 11:58we're grateful to have a lot of new
- 12:01things vetted, at the system level. Will this
- 12:04integrate with our systems? You know? Is this
- 12:07product safe? Is it in our is it
- 12:09within kind of the usable function of our
- 12:12our supply chain? Whatever whatever the the new
- 12:15thing may be.
- 12:16A lot of vetting gets done at our
- 12:18system level.
- 12:19We need to make sure that it fits.
- 12:22So we need to make sure we have
- 12:24the right nurse leaders at the table, physician
- 12:26leaders at the table, operational leaders at the
- 12:28table,
- 12:30our support services, our ambulatory and outpatient functions.
- 12:34Our team likes to have that really comprehensive,
- 12:37diverse,
- 12:38numbers are numbers. Data
- 12:40can tell lots of different stories.
- 12:42It's the users
- 12:44to make sure that our teams help us
- 12:46understand the why
- 12:47that, start to really inform the prioritization.
- 12:52That is so helpful to know and understand.
- 12:53Thank you, Emily. And before we wrap up,
- 12:55I wanted to talk about growth as well.
- 12:57Where do you see some of the best
- 12:58opportunities for growth in the next year?
- 13:01You know, specifically for us in our market,
- 13:04we have an incredibly
- 13:05robust
- 13:06ambulatory and outpatient,
- 13:09platform.
- 13:10More and more,
- 13:12patients are requiring kind of a complex workup.
- 13:15So our imaging and diagnostic imaging modalities are
- 13:19busy.
- 13:19You know, I'm sure everyone is focused on
- 13:21this, but I'd say you can always do
- 13:23more at looking at how you're looking at
- 13:25your schedules, your access, and your ability,
- 13:29to make sure you're getting your patients in
- 13:31and through for
- 13:33your diagnostic imaging imaging department. It's just a
- 13:35huge growth opportunity for our campuses.
- 13:38Our surgical platform here,
- 13:40is robust.
- 13:42You know, we're a 159
- 13:44bed hospital,
- 13:45and we have a tertiary campus to our
- 13:48east that we refer higher acuity things to.
- 13:51But that makes us a really great bread
- 13:53and butter,
- 13:55operating facility where we start on time. We
- 13:58can, you know, be really efficient, we can
- 14:00add on cases, and that's been a really
- 14:02attractive thing for some of our surgeons and
- 14:04proceduralists. So I'd I'd say for us, we're
- 14:07we're really focused on growth,
- 14:09our procedural areas, our diagnostic imaging areas, and
- 14:13our ambulatory platform, including our our oncology and
- 14:16cancer center.
- 14:18That's amazing to hear. A lot of exciting
- 14:20things upcoming. So, Emily, thank you so much
- 14:22for joining us on the podcast today. This
- 14:24has been a a really fun discussion to
- 14:26have, and I'm looking forward to seeing you
- 14:29as well at our annual meeting. I I
- 14:30know coming up here in April, we'll you'll
- 14:32be speaking on a panel, and it'll be
- 14:34just really fantastic to hear a little bit
- 14:36more about some of the things we're talking
- 14:38about today as well as how it's continuing
- 14:40to make an impact on the communities you
- 14:42serve.
- 14:43Really looking forward to it. We always love
- 14:45the Becker's conference.
