- 0:35This is Gracelyn Keller with the Becker's Healthcare
- 0:37Podcast, and we are recording live at the
- 0:39thirty first annual business and operations of ASCs.
- 0:42I'm currently joined by Brenda Fernandez, who is
- 0:45the chief financial officer at Puerto Rico ASC
- 0:48Holding. So Brenda, thanks for being here. I'm
- 0:50gonna have you kick us off by sharing
- 0:52a little bit more about yourself and your
- 0:54work in the ASC space.
- 0:57Yeah. First, thank you for the invite.
- 1:00I currently serve as a chief financial officer
- 1:02of,
- 1:03network of ASCs in Puerto Rico. Our portfolio
- 1:07includes,
- 1:08two single specialty
- 1:09in ophthalmic ASCs and one multi specialty
- 1:13with the first outpatient
- 1:15total joint program on the island. My role
- 1:18focus on
- 1:19financial stewardship and sustainable growth,
- 1:22overseeing strategic investment, payer contracting, capital allocation, and
- 1:27operational
- 1:28efficiency.
- 1:29I work closely with the
- 1:31clinical and administrative leaders
- 1:33to ensure that
- 1:35every dollar spent
- 1:37aligns with our mission, which is delivering high
- 1:40quality, cost effective care in a compliant and
- 1:42scalable environment.
- 1:44Wonderful. Well, thanks for being here. And let's
- 1:46start our conversation by talking about the ASC
- 1:49market. In The US, this is projected to
- 1:52reach $60,800,000,000
- 1:54by 2030, and it continues to experience strong
- 1:56year over year growth. So from your perspective,
- 1:59what is the most significant trend and market
- 2:02forces driving this expansion? And how should ASC
- 2:05leaders be preparing today?
- 2:07Well, the growth, it's been fueled by a
- 2:09combination of payer payer incentive,
- 2:12patient preference, and innovation.
- 2:15From a financial lens, the shift of surgical
- 2:18volume from hospital to ASCs, especially orthopedics and
- 2:22ophthalmology,
- 2:23this creates meaningful cost saving for the system
- 2:27while maintaining quality.
- 2:29So
- 2:30there are
- 2:31opportunities in the market. So for ASC leaders,
- 2:33the priority should be scaling in a strategic
- 2:36way.
- 2:37For example, build strong financial motors around came
- 2:40case mix and payer mix to anticipate margin
- 2:43compression.
- 2:45Also, straighten revenue cycle performance,
- 2:48eligibility,
- 2:49coding, accuracy, and denials management.
- 2:52Another priority,
- 2:53might be aligned capital investment in technology and
- 2:56infrastructure with volume based
- 2:58return on investment projections.
- 3:00And the last one but not minor, diversity
- 3:03partnerships and bundled payment participation to stay competitive
- 3:06in a value based environment.
- 3:09Wonderful.
- 3:10And then from AI and robotic surgeries to
- 3:12advanced EHR systems, technology remains both a make
- 3:16or break factor and a critical driver of
- 3:17ASC operations at scale. So how do you
- 3:20see deeper tech integration shaping the ways ASCs
- 3:22deliver care and manage their business over the
- 3:24next few years?
- 3:26AI is a trending topic in this era.
- 3:29Technology is quickly becoming the great equalizer for
- 3:32a for ASCs.
- 3:34From my perspective,
- 3:36data driven decision making is now the differentiator
- 3:39between good and great centers.
- 3:41AI is,
- 3:43an automation are allowing us
- 3:46to actually predict case profitability
- 3:48and optimize staffing in real time.
- 3:51Also, doing some benchmark supply utilization and implant
- 3:55costs against national datasets.
- 3:59Also
- 4:00identify leakage points in billing or documentation
- 4:03that directly affect cash flow.
- 4:06Advanced analytics also make our board level reporting
- 4:09sharper. We can tie operational metrics
- 4:12directly to their financial impact like turnover, state,
- 4:15and infection rates among others.
- 4:18The next frontier
- 4:19is
- 4:20integrated
- 4:21dashboards that link quality outcomes, staffing, and cost
- 4:25per case on one platform.
- 4:28And with 60% of health systems considering ASC
- 4:30joint ventures and many ASCs already partnering with
- 4:33systems in their communities,
- 4:35what opportunities do you see for collaboration, whether
- 4:37with other providers or vendors, to strengthen patient
- 4:40care and operational efficiency?
- 4:43Partnerships
- 4:44are where where we are valued has been
- 4:46unlocked. Help system bring brand recognition
- 4:49and complex
- 4:51case pathways.
- 4:52ASCs brings efficiency and lower cost structures.
- 4:56There are too many opportunities in the market.
- 4:59So some of them include joint venture expansions,
- 5:03where hospital contribute case volume and capital, and
- 5:06ASCs provide operational excellence.
- 5:09Another opportunity
- 5:11might be vendor and implant partnerships
- 5:14tied to cost per case performance or volume
- 5:16based rebates.
- 5:18And, also, I think that cost provider networks,
- 5:21it could be another opportunity for bundled episodes,
- 5:24surgery,
- 5:25rehab,
- 5:26and follow ups to enhance con continuity of
- 5:29care and capture shared savings.
- 5:32The financial motor of the future is collaborative,
- 5:36share risk, share reward, and align transparency.
- 5:40Well, thank you so much for taking the
- 5:41time to join me today. Is there anything
- 5:43we didn't touch on or any final thoughts
- 5:44you'd like to share? Absolutely.
- 5:47Financial literacy, it's in the ASC world today
- 5:50means more than balancing books.
- 5:52It's about designing resilient systems.
- 5:55Our challenge is to remain lean while investing
- 5:58in the right innovations.
- 6:00The centers that
- 6:01thrive will be those that combine discipline and
- 6:03cost management with boldness in strategic growth.
- 6:07Technology and collaboration
- 6:08are not are not just enablers.
- 6:11They are capital multipliers when executed with precisions.
- 6:15Wonderful. Well, Brenda, thanks so much for joining
- 6:17me. And, again, we are recording live at
- 6:19the thirty first annual business and operations of
- 6:22ASCs. Thanks so much. Thank you.
