- Yoel Bakas: A question many of you are grappling with today is no longer whether you should start Pluvicto which, if you haven't, you should really be considering. It's whether you should build your own program or partner with someone else. Building your own program gives you full control, but it also is full capital outlay. you get the highest control with the longest payback. Vendors like it because the program absorbs all of the capital cost and the pharmaceutical companies still provide the radiopharmaceuticals. Ken Brooks and Christine Stone from WellStar in Atlanta recently published an awesome publication on how community centers can build their own Theranostics program specifically tailored for community practices. In their case, was a three year rollout. The first phase, which is the foundational phase, they complete it in about 6 months. But the critical parts of that phase teed up the work for the rest of it. In that phase, they considered financial analysis, staffing, they considered workflows. They considered regulatory compliance and reimbursement. and they estimated about $42,000 in Hot Lab equipment. 340B drug pricing was fundamental to the success of this community practice. their first patients have been treated successfully. That is exactly what a program that builds should look like. In a partnership, you share the capital expense with an outside vendor or outside investor. That means you also share margin. And when the partners commercial priority shift, your program shift with it. The type of Theranostics program you build has to be based on your specific program needs and goals and can only be done after a thorough evaluation. Do the homework first, start the program second.
